Shoulder hemiarthroplasty for the treatment of three and four part fractures of the proximal humerus using Comprehensive® Fracture stem.

We looked at the functional outcome of 32 consecutive patients with proximal humeral fractures that required hemiarthroplasty. Functional st … cally, no dislocation, loosening or greater tuberosity pull off was seen. Superior migration of the humeral head was seen in 11 patients (34%). There was no significant difference seen in functional outcome among different genders, age and those who had radiological superior migration of the prosthesis. However, there was a significant statistical difference s …

Mechanisms of rotator cuff tendinopathy: intrinsic, extrinsic, or both?

The etiology of rotator cuff tendinopathy is multi-factorial, and has been attributed to both extrinsic and intrinsic mechanisms. Extrinsic … ngement, is attributed to compression of the posterior articular surface of the tendons between the humeral head and glenoid and is not related to subacromial space narrowing. Intrinsic factors that contribute to rotator cuff tendon degradation with tensile/shear overload include alterations in biology, mechanical properties, morphology, and vascularity. The v …

Finite element model validation based on an experimental model of the intact shoulder joint.

The shoulder joint is a complex anatomical system. The main goal of this study was to build a Finite Element (FE) model of the intact shoulder joint and its validation was done using an experimental model comparing cortical strains. Considering the expected differences between the experimental model and an in vivo shoulder, the experimental model developed replicates adequately the in vivo functioning of the joint. For the experimental model we used 4th generation composite bone structures of the humerus and scapula, including the humeral head cartilage, the glenoid cartilage and glenohumeral …

Cancellous bone allograft is comparable to fibular strut allograft for augmentation in three- or four-part proximal humeral fractures.

BACKGROUND: Bone grafts have been used for augmentation and improving stability of reduced fractures in proximal humeral fractures. The aim … ability of the arm, shoulder, and hand (DASH) score, the range of movement, neck-shaft angle (NSA), humeral head height (HHH), and the changes of NSA and HHH, as well as recording any complications. The repeatedly measured clinical and radiological outcomes were analyzed by linear mixed models. The differences in outcomes between groups at the final follow-up …

An arthroscopic technique for confirming intra-articular subluxation of the long head of the biceps tendon: the ramp test.

The ramp test is used to test the … result showed that the long head of the biceps tendon translated medially and inferiorly across the humeral head. In addition, this subgroup of 17 patients all had an abnormal ramp test but did not have a full-thickness rotator cuff tear. The ramp test is now applied to all arthroscopic procedures by the senior author, and we recommend its use for arthroscopic …

Electromyographic analysis of shoulder joint function of the biceps brachii muscle during isometric contraction.

Surface electromyography was performed for both heads of the biceps brachii in 11 healthy men while the muscles were under 30% maximum isome … the biceps muscle is a flexor and an abductor of the shoulder, and that the long head can act as a humeral head stabilizer in superior and anterior directions. Muscle fatigue of the biceps and the deltoid muscle also was determined at 30% of maximum isometric flexion. All muscles had significantly decreased mean power frequency and turns count, and increased …

Significance of the latissimus dorsi for shoulder instability. II. Its influence on dislocation behavior in a sequential cutting protocol of the glenohumeral capsule.

In a cadaveric instability model that leaves all muscles intact initially, the latissimus dorsi seemed to play a role when complete section … en when there was a type 2 scapular connection. The tendon of the latissimus dorsi can restrain the humeral head from dropping inferiorly or can lead to a spontaneous reduction of a dislocation, depending on its anatomy. This effect can only take place in the infrequent situation of humeral avulsion of the glenohumeral ligaments. This may be an explanation for …

Osteochondroses of the bilateral metacarpal heads: Dieterich disease. A case report with review of the literature.

Osteochondroses is a well-known entity and typically affecting immature skeleton with few common locations involved are femoral head epiphysis, tibial tubercle apophysis, calcaneal apophysis, humeral capitellum and anterior vertebral end plates. We report a rare case of osteochondroses showing avascular necrosis involving metacarpal heads known as Dieterich disease, involving the head of the third metacarpal and probably the first case with a history of rock climbing as an etiology. Chronic repetitive microtrauma plays a significant role in the disease, as is seen in our patient. Imaging plays

Osteonecrosis.

Osteonecrosis (avascular necrosis, aseptic necrosis or subchondral avascular necrosis) is not a specific disease entity but the final common pathway of a number of conditions leading to bone death. The main predisposing factors (trauma, glucocorticosteroids, alcoholism and connective tissue disorders) should be carefully sought, but osteonecrosis can also be idiopathic in origin. Its most common localization is to the femoral head, followed by the humeral head, the knee and the small bones of the wrist and foot. New imaging techniques, especially magnetic resonance imaging, have improved the e…

Biomechanical analysis of latissimus dorsi tendon transfer with and without superior capsule reconstruction using dermal allograft.

BACKGROUND: Irreparable rotator cuff tears (ICTs) remain a challenging treatment dilemma. Superior capsule reconstruction (SCR) acts as a static stabilizer to decrease superior humeral head migration. Latissimus dorsi tendon transfers (LDTs) dynamically decrease superior humeral head migration and improve external rotation. We hypothesized that the dynamic stabilizing effect of the latissimus transfer plus the static stabilizing effect of SCR would improve shoulder kinematics in shoulders with ICTs. MATERIALS AND METHODS: Eight fresh-frozen cadaveric shoulders were tested in 5 conditions: (1)…

Elbow hemiarthroplasty using a "triceps-on" approach for the management of acute distal humeral fractures.

BACKGROUND: Total elbow arthroplasty is an established option for the primary treatment of acute distal humeral fractures, but there are sparse data regarding elbow hemiarthroplasty (EHA) as an alternative. We present the outcome of EHA performed with a modular anatomic prosthesis and a "triceps-on" surgical technique. METHODS: Eighteen consecutive patients underwent EHA for an acute fracture. Two patients died, leaving a study group of 16 patients with minimum 2-year follow-up. Clinical evaluation included range of motion; Mayo Elbow Performance Score; Quick Disabilities of the Arm, Shoulder

Treatment of a brachial plexus injury using kinesiotape and exercise.

PURPOSE: This describes a child whose neonatal brachial plexus injury was treated with kinesiotape and exercise. DESCRIPTION: The subject was a two-year-old female whose X-rays demonstrated severe inferior subluxation of the humeral head and winging of the scapula on the left. She was fitted with a shoulder brace with surgery scheduled in six months. The initial PT exam noted 80 degrees of shoulder abduction (trumpet sign), significant asymmetry, and nonuse. Mallet score was 15/25. Treatment consisted of d/c of the brace and E-stimulation, parent education on exercise and taping, and kinesiot…
