"What if that's your last sleep?" A qualitative exploration of the trauma of incarceration and sleep.

STUDY BACKGROUND/OBJECTIVES: Sleep is an underexplored factor in the health of people involved in the criminal legal system. This study addresses the paucity of research on how individual, social, and physical environmental factors impact sleep health during and after incarceration by highlighting the voices of people involved in the criminal legal system through a community-engaged qualitative research approach. METHODS: We conducted 20 semi-structured interviews with men recently released from prison for a study on trauma and healthcare during incarceration and after release. Interviews wer…

Childhood Trauma Predicts Sadistic Traits and Violent Behavior in Incarcerated Youth.

Childhood trauma exposure is prevalent among incarcerated youth and associated with antisocial traits and behavior. It has been proposed as a risk factor for the development of sadistic traits, which has been shown to predict future violence in youth. Using regression analyses, we examined the association between self-report and expert-rated measures of childhood trauma, sadistic traits (i.e., verbal, physical, vicarious sadism), and violence (i.e., homicide and non-homicide violent acts) in 54 incarcerated juveniles. Expert-rated (but not self-report) severity of physical abuse was associated

Impact of Dyslexia and Health Literacy on Racial and Gender Disparity in the Incarcerated Population.

Dyslexia is the most common learning disability and involves difficulty in reading, writing, and spelling in your native language despite at least average intelligence. A disproportionate share of incarcerated individuals have both dyslexia and are African American. Behavior manifestations associated with dyslexia often lead to life choices resulting in incarceration. Dyslexia is rarely considered as playing a role in unemployment, drug misuse, and incarceration. Dyslexia screening upon prison admission permits identification of individuals with dyslexia and allows for specialized reading clas…

Incarcerated Diaphragmatic Hernias After Roux-en-Y Gastric Bypass.

Laparoscopic Roux-en-Y gastric bypass (LRYGB) is one of the most common operations performed for morbid obesity. Some of the known surgical complications include anastomotic leaks and small bowel obstructions due to internal hernias. Diaphragmatic hernias are common in the general population, and repair of symptomatic hernias is generally recommended. Diaphragmatic hernia after a prior LRYGB is markedly less common. Diaphragmatic hernias can occur via a hiatal defect or rarely a parahiatal defect that is found lateral to the hiatus. We present two cases of incarcerated diaphragmatic hernias af

Intestinal ischemia in patients with incarcerated groin hernia: proposal and validation of a score.

PURPOSE: Intestinal ischemia (II) is the most critical factor to determine in patients with incarcerated groin hernia (IGH) because II could be reversible, and it is considered as a "time sensitive condition." Although predictive factors of II were identified in several previous studies, preoperative diagnosis of II cannot be reliably made or excluded by any known parameter. The aims of this study were: to devise and to validate a clinic-biologic score, with a strong discriminatory power, for predicting the risk of II in patients with IGH. METHODS: We conducted a retrospective bicentric study

Changes in sleep quality and housing status among individuals incarcerated in jail.

OBJECTIVE: To investigate sleep quality among individuals incarcerated in a rural county jail, by housing status before incarceration. METHODS: Using cross-sectional survey methods, 194 individuals incarcerated in jail reported sleep quality prior to and during incarceration on a Likert scale and pre-incarceration housing status (ie, house, apartment, motel, group living, or homeless). Prevalence ratios (PR) were estimated using log binomial regression to determine associations between housing status before incarceration and changes in sleep quality. RESULTS: Participants in non-permanent ho…

Urinary tract obstruction in the second trimester: a report of an incarcerated gravid uterus.

Incarceration of the gravid uterus (IGU) is a rare obstetric disorder that may lead to pregnancy-related complications. Acute urinary retention (AUR) is one of the associated symptoms in IGU and it is mostly observed in the first semester and early second trimester. A case of a woman, in her 30s, is described, who presented AUR and lower abdominal pain in the early second trimester. The diagnosis was confirmed upon pelvic examination, and abdominal and transvaginal ultrasound. Management included long-term bladder catheterisation, manual reduction manoeuvres and passive positioning manoeuvres …

A Quasi-Experimental Study on the Effectiveness of Dutch Cell Dogs in Incarcerated Youth.

This study examined the effectiveness of Dutch Cell Dogs (DCD), a prison-based dog training program that aims to improve socioemotional functioning of incarcerated youth by giving them the opportunity to train a shelter dog. Primary (aggression and institutional infractions) and secondary (wellbeing and therapeutic functioning) outcomes were assessed for the intervention (n = 61) and comparison group (n = 77) before the start of DCD, after 4 weeks, and after 8 weeks at posttest. Overall, DCD participation was not effective. Compared to the comparison group, institutional infractions decreased

Providing Ethical and Humane Care to Hospitalized, Incarcerated Patients With COVID-19.

The COVID-19 pandemic is devastating the health of hundreds of thousands of people who live and work in U.S. jails and prisons. Due to dozens of large outbreaks in correctional facilities, tens of thousands of seriously ill incarcerated people are receiving medical care in the community hospital setting. Yet community clinicians often have little knowledge of the basic rights and ethical principles governing care of seriously ill incarcerated patients. Such patients are legally entitled to make their own medical decisions just like non-incarcerated patients, and retain rights to appoint surrog

Contraceptive Needs Among Women Recently Incarcerated at a Rural Appalachian Jail.

Background: Incarceration is associated with negative sexual and reproductive health outcomes. We examined contraceptive needs among women incarcerated at a rural Appalachian jail with emphasis on pregnancy history, recent contraceptive use, and current and near-future contraceptive needs. Materials and Methods: A survey was administered to newly incarcerated women at a jail in Southwest Virginia. It included questions about (1) prior pregnancies; (2) pregnancy intentions, contraceptive use, and sexual activity in the 3 months before jail; (3) unprotected sex in the 5 days before jail; (4) int…

Sexual and reproductive health of incarcerated women in French Guiana: a qualitative approach.

PURPOSE: Incarcerated women are a vulnerable population in terms of sexual and reproductive health. In French Guiana, most incarcerated women come from unsafe environments and are incarcerated because of drug trafficking. Medical follow-up processes used in prison (medical assessment on arrival, and then two half-days per week upon request but without an obstetrician-gynecologist) does not allow for a thorough assessment of the impact of incarceration on women prisoners' health to take place. In the absence of data, the purpose of this study was to describe incarcerated women's experiences in …

Facing the Ties That Bind: Understanding Experiences of Men With An Incarcerated Romantic Partner.

This study examines what it means to be a man with an incarcerated romantic partner. Despite recognition that those experiencing familial incarceration are a heterogeneous group, the extant literature lacks a clear focus of the experience of men with incarcerated romantic partners. Using a phenomenological design, the researchers interviewed seven men with incarcerated romantic partners to explicate the essential structures of their lived experience. The interviews are explicated into six essences illuminating the experience of being a man with an incarcerated partner. Identified essences expl…

Formerly Incarcerated People With Disabilities: Perceptions of Accessibility and Accommodations in Correctional Programs.

Much research centers on correctional programming and successful reintegration; however, little work explores how incarcerated people with disabilities (IPWDs) perceive accessibility and accommodations in such programs. Given that this population has a higher recidivism rate than others, it is important to better understand unique problems faced by IPWDs. The sample was drawn from a local reentry program in Virginia. Utilizing qualitative methods, data were collected through a focus group interview and in-depth interviews. Three themes identified from these data were the lack of attention to p

Nonconsensual Sexual Experience Histories of Incarcerated Men: A Mixed Methods Approach.

Nonconsensual sexual experiences (NSEs) may contribute to mental health concerns among incarcerated individuals, yet NSEs are understudied in this population. This study takes a novel approach in examining the prevalence of NSEs among incarcerated males by utilizing both quantitative and qualitative measures. The sample consisted of 189 men from three provincial maximum-security prisons in Ontario, Canada. Based on quantitative findings, 44.2% of the sample experienced NSEs before the age of 18, and 41.7% of the sample endorsed an experience that fit the legal definition of a NSEs as adults. P

Case of Infected Urachal Cyst in an Adult Presenting as an Incarcerated Umbilical Hernia.

The urachus is a remnant of the allantois. Failure to obliterate can result in one of four anomalies, urachal cyst being most common. Urachal cysts are relatively rare, especially in adults. This paper presents a patient with an umbilical hernia and a ruptured urachal cyst. A 39-year-old male presented with concern for umbilical hernia, but he also noted drainage. Computed tomography scan showed a urachal cyst and umbilical hernia. The urachal cyst was excised and umbilical hernia closed primarily. The incidence of an urachal cyst is unknown, but persists in roughly 2% of adults. Diagnosis is

Changes in overdose knowledge and attitudes in an incarcerated sample of people living with HIV.

PURPOSE: The purpose of this paper is to present evaluation results. People exiting incarceration who use opioids are at an elevated risk for overdose following release. People living with HIV (PLWH) who use drugs are also at increased overdose risk. Overdose education and naloxone distribution (OEND) is an effective community-based intervention, but few OEND programs have been evaluated in a correctional setting and none have specifically targeted PLWH. DESIGN/METHODOLOGY/APPROACH: An OEND pilot program was implemented in the Philadelphia jail from December 2017 to June 2019. OEND was provid…

Emergency medical care of incarcerated patients: Opportunities for improvement and cost savings.

In the United States (US), the lifetime incidence of incarceration is 6.6%, exceeding that of any other nation. Compared to the general US population, incarcerated individuals are disproportionally affected by chronic health conditions, mental illness, and substance use disorders. Barriers to accessing medical care are common in correctional facilities. We sought to characterize the local incarcerated patient population and explore barriers to medical care in these patients. We conducted a retrospective, observational cohort study by reviewing the medical records of incarcerated patients prese…
