Must consent always be obtained for a do-not-resuscitate order?

Using cardiopulmonary resuscitation for cardiac or respiratory arrest unless there is an explicit do-not-resuscitate order is a policy adopted by all hospitals. Such a policy usually requires the patient's (or surrogate's) consent for a do-not-resuscitate order to be instituted. This article, however, presents the argument that consent need not always be obtained. In the case discussed, the well-being of the patient, other patients, and the health care providers all support a unilateral decision by the physician not to attempt resuscitation at the time of death. The medical community and socie

Options for increasing organ donation: the potential role of financial incentives, standardized hospital procedures, and public education to promote family discussion.

Required request laws that mandate hospital personnel to request organ donations have not substantially increased the supply of organs for transplantation. Frustration caused by the continuing shortage of organs has led several experts to promote controversial options for expanding the pool of available organs, including the offer of financial incentives. Pilot programs to test the use of incentives are warranted, but such experiments must be approached cautiously, given the moral qualms they might raise and the availability of other options. One promising strategy is to install standardized h

[Viewpoint on the methodology of drug trials affecting cognition of elderly patients].

Clinical trials for cognitive disorders in the elderly require specific methodological guidelines. They must take into account the psychosocial dimension of the patient and his family and must be based on serious neurobiologic knowledge. In degenerative dementias the progress of research concern genetics, molecular intercellular recognition and astrocytic cells. Biology of cognition like hippocampal long term potentiation provides good pharmacologic basis for trials. In normal brain aging several ways must be developed: aminergic systems, free radicals, excitotoxic amino-acid, nerves growth fa

Screening of asymptomatic women for endometrial cancer.

This report describes the preliminary results of a feasibility study of detection of endometrial carcinoma in an asymptomatic population of women aged 45 and above. The initial sample of 1,280 women was evaluated by several cytologic smear techniques and by two endometrial sampling methods assigned by computer. The clinical approaches and the techniques used are detailed. During this initial period of study, eight histologically proved endometrial cancers were diagnosed, two of which were deeply invasive. With one exception, the eight patients were either obese or had a history of treatment wi

Shock therapy and psychiatric malpractice: the legal accommodation to a controversial treatment.

Shock therapy has developed over the years into an accepted form.of psychiatric treatment for relieving the symptoms of certain mental illnesses. The development of shock therapy did and still does take place in a legal environment which leaves the reasonable psychiatrist free to experiment. The discovery of muscle-relaxing drugs and their introduction into the treatment procedure were undoubtedly encouraged by the possibility of malpractice suits arising from bone fracture caused by rapidly contracting muscles. Today the incidence of malpractie suits involving shock therapy has been reduced,

Ethical considerations in establishing reference population DNA databases of Indigenous peoples in Southeast Asia.

Reference population DNA databases are essential for accurate forensic statistical calculations, yet Indigenous populations remain significa … ns of random match probabilities and likelihood ratios. We identify critical ethical gaps regarding informed consent, community engagement and/or partnership, data ownership, and restrictions on secondary use. We recommend adopting dynamic consent models consistent with Indigenous data sovereignty and the CARE (Collective Benefit, Authority to Control, Responsibil …

Patient Expectations in High-Risk Abdominal Surgery for Cancer.

BACKGROUND: Despite the central role of expectations in informed consent and surgical shared decision-making, patient expectations frequently remain unelicited or underexplored and are often assumed rather than explicitly addressed. OBJECTIVE: To explore how patients form expectations about high-risk abdominal surgery for cancer and how preoperative expectations influence the experience of surgical recovery and life after surgery. DESIGN: Qualitative study using semi-structured interviews informed by cognitive task analysis and analysed using an abductive thematic framework. SETTING AND PAR…

Factors Associated with Consent and Opportunities to Increase Participation in the Rick Hansen Spinal Cord Injury Registry: A Population-Based Study.

BACKGROUND: Disease-specific registries that record patient data are crucial in supporting research, guiding best practices, and optimizing resource allocation. Since 2004, the Canadian Rick Hansen Spinal Cord Injury Registry (RHSCIR) has collected data from over 10,000 individuals with SCI. OBJECTIVE: The primary objective of this study was to identify factors associated with consenting to RHSCIR and reasons for withdrawal to improve registry inclusivity and representativeness in persons with traumatic SCI (TSCI). METHODS: This retrospective cohort study analyzed data from 4090 participants

Patient and Healthcare Professional Reflections on Consenting for Extra Bone Marrow Samples to a Biobank for Research-A Qualitative Study.

Little is known about patient perspectives regarding consent for obtaining extra research-specific bone marrow (BM) samples during the diagnostic procedure for acute leukemia (AL). This study aimed to better understand patient experiences with consenting to provide these samples and identify potential areas for practice improvement. Semi-structured interviews were conducted with patients treated for AL, 4-6 years prior to the interviews, and healthcare professionals involved with obtaining patient consent and sample collection. A total of 17 patients (14 agreed to provide a sample and 3 did no
