Eighty-two terminal cancer patients were treated with implanted venous reservoirs using a Groshong catheter. Sixty-seven out of 82 (77%) patients were temporarily discharged or stayed at home on weekends. The mean hospital stay of these implanted patients at death was shorter than that of non-implanted patients (31 +/- 44 vs 51 +/- 46 days, p = 0.1187). A drug delivery system using copoly (lactic/glycolic acid) was developed for the intrathecal administration of nicardipine. Staphylococcal meningitis associated with implantation of an intrathecal drug pump for spasticity was successfully treated by intrathecal vancomycin delivered by the same pump. This produced high CSF antibiotic levels, and the pump and catheter system did not have to be removed. We are unable to identify a similar case reported in the literature to date.

This paper reports the experience at the Royal Victoria Hospital in Montreal with the first 50 Port-A-Cath devices implanted for venous access in patients requiring long-term chemotherapy. There were 25 women and 22 men, ranging in age from 18 to 85 years. Twenty-two devices were implanted for hematologic malignant disease, 26 for solid tumours and 2 for benign disease. A phase I study with continuous administration of epirubicin for 21 days using a venous access port and a portable pump was performed. The first dose step was 2 mg/m2/d for 21 days. Pulsatile GnRH therapy has been shown effective in the treatment of infertility associated with hyperprolactinemia by direct action on the pituitary. Gonadotropin secretion was restored in the setting of moderate hyperprolactinemia. GnRH should be considered as a potential alternative to BCPT therapy in this setting.

The side effects of Rapamycin was evaluated by histopathological examination of the heart, kidneys, and eyes of treated Lewis rats. Rapamycin was administered during 14 days by continuous intravenous infusions at doses of 0.5, 1.0, and 1.5 mg/kg/day. We report the surgical technique for subxiphoid robotic thymectomy with combined superior vena cava (SVC) resection. A blood-drainage cannula was inserted into the left internal jugular vein as a blood-drainage route. A blood infusion cannula was inserted into the left femoral vein. During SVC clamping, an assistant pumped blood through the circuit to prevent clotting.
