Trochlear dysplasia is a developmental condition characterised by an abnormally flat or dome-shaped trochlea and is an important contributory factor to patellofemoral instability and recurrent dislocation. We studied prospectively a series of 54 consecutive patients (59 knees) with patellofemoral instability secondary to trochlear dysplasia, who were treated by a trochleoplasty by a single surgeon between June 2002 and June 2007. Pre- and post-operative scores were assessed by the patients and a satisfaction questionnaire was completed. Of the 54 patients (59 knees) in the series, 39 (44 knees

The Elmslie-Trillat procedure for correction of patello-femoral malalignment was evaluated in 27 knees in 22 patients with an average followup of 42 months (minimum of 24 months). Preoperative and postoperative pain and activity levels were recorded for all knees. The quadriceps angle was recorded in 22 knees preoperatively and in 19 knees postoperatively. Good or excellent results were obtained in 81% overall and in 91% of those knees with patella alta. The postoperative quadriceps angle (Q-angle) correlated with the result. Correction of 10 degrees or less was always associated with a good o

OBJECTIVE: Anterior cruciate ligament deficiency (ACLD) has traditionally been regarded as a contraindication for unicompartmental knee arthroplasty (UKA). However, advancements in surgical techniques and improvements in prosthetic manufacturing have challenged this notion. Controversy persists regarding whether the anterior cruciate ligament (ACL) influences the postoperative outcomes of fixed-bearing (FB) UKA. This study aimed to evaluate the impact of varying severities of ACLD on the clinical outcomes of FB-UKA.
METHODS: This retrospective analysis included 81 patients (87 knees) who unde

BACKGROUND: In this study, we evaluated children who underwent prenatal myelomeningocele (MMC) repair to investigate the influence of the an … mbulators. Congenital orthopedic malformations were also unrelated to the injury levels, except for knee dislocation in relation to upper lumbar injury. At the functional level, we observed that for the majority, the levels of function of at least 2 vertebrae were below the anatomical level.
CONCLUSIONS: There were no differences in functional motor skills, walki …

Although ankle fractures are very common cases for orthopedic surgeons with an easy diagnosis and very good outcomes either with conservative or with operative treatment, in diabetic patients, these fractures can be complex with difficult solutions. We report two cases with ankle fractures in diabetic patients from our department with demanding diagnostic or therapeutic approaches and poor outcomes. The first case, a 65-year-old man with undiagnosed diabetes mellitus and neglected ankle fracture dislocation because of diabetic neuropathy, underwent open reduction internal fixation with additio

Patellofemoral instability is related to anatomy. Magnetic resonance imaging (MRI) provides anatomic detail, but spoiled gradient echo (SPGR) imaging during isometric quadriceps contraction provides objective functional data for diagnosing patellofemoral laxity. Knee MRI studies and medical charts of 398 patients were retrospectively reviewed. Two independent blinded observers evaluated the knee MRI studies for patellofemoral morphology and patellar position on axial SPGR images during relaxation and isometric quadriceps contraction for lateral patellar migration. Charts were reviewed for hist

Consequences of injury as well as succeeding joint-impairment after fracture dislocation of the tibial head are analysed. Of 38 patients who had undergone operative therapy for fracture dislocation of the tibial head between 1982 and 1991, 30 were followed up after a mean of 6.3 years. With an average age of 46.8 years and a distribution between sexes of 26 men vs. 12 women, types II (17) and V (14) (classification acc. to Moore) clearly outweighed types III (3) and IV (3) as well as type I (1). Causes of injury were dominated by traffic accidents (22/38), 9 patients suffered from accompanying

Transient patellar dislocation is a rare finding. It is usually due to direct trauma in mediolateral direction or to indirect trauma related to violent quadriceps muscle contraction. The clinical diagnosis of transient forms is usually rather difficult because the associated signs (hemarthros and pain at the joint and/or at the vastus medialis insertion) are not specific. Conventional radiography and CT are very accurate in bone studies, but exhibit major limitations in the detection of capsular injuries with intramedullary and/or cartilaginous bone involvement. We investigated MR diagnostic r

Metallosis is a known yet rare late complication of unicompartmental and total knee arthroplasty (TKA), usually secondary to either metal-backed patellar component failure, mobile-bearing polyethylene dislocation, or catastrophic polyethylene failure and wear through. The majority of literature surrounding metallosis has been published in relation to total hip arthroplasty (THA) metal on metal bearing wear or mechanically assisted crevice corrosion. This case report describes the development of metallosis in a 77-year-old male patient with advanced (Kellgren-Lawrence Grade 4) osteoarthritis wit

We report a case of a 15-year-old girl who is a known case for proximal focal femoral deficiency. She underwent several surgical lengthening procedures. Her course was complicated by femur nonunion, knee joint dislocation, stiff knee, and final lower limb discrepancy of over 15 cm. Rotationplasty was performed to salvage her previous surgical failures and restore her function. Surgery was uneventful and the patient was able to walk with the prosthesis in a few months after index surgery. Although the outcome of rotationplasty is visually unusual, for some children restored function and physica

6 cases of popliteal vascular trauma are presented, 3 due to posterior dislocation of the knee and 3 due to crush injury. The patients were referred from another hospital and some had undergone unnecessary angiography when ischemia was present, leading to delay in surgery. All patients presented with distal ischemia and underwent reconstructive surgery; 2 subsequently underwent below-knee amputation because of irreversible ischemia and sepsis. Urgent operation for popliteal vascular trauma is necessary whenever there is ischemia and intraoperative angiography may be necessary. There should be

Primary care physicians are being called on more frequently to treat patients who have musculoskeletal injuries. This article discusses mechanisms of injury, diagnosis, and treatment, with emphasis on determining when to refer patients to an orthopedist. Discussed here are several types of knee injuries that require referral, including extensor mechanism injuries, most subluxations and dislocations, ligament injuries, and meniscus injuries.

BACKGROUND: Patients with patellar instability and a tibial tubercle-trochlear groove (TT-TG) distance ≥20 mm may be candidates for distal tubercle realignment surgery. Although this variable has proven valuable in predicting recurrent dislocations, it is not individualized to patient size, bony structure, or patellofemoral mechanics.
PURPOSE: To develop a patellar instability ratio (PIR) that predicts the risk of recurrent instability based on the TT-TG distance to patient-specific anatomy.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: On magnetic resonance imaging scans of 59

Compared with normal X-ray examination, a CT scan can give more information about patellar alignment, especially close to full extension of the knee-joint. We compared the normal knee-joints of 10 volunteers with the knee-joints of 15 patients (19 recurrent luxations, 5 habitual subluxations). We were able to distinguish between instabilities with and without changes seen on the X-ray pictures, making it possible to develop a systematic concept of treatment. In the case of recurrent dislocation with radiologically visible instability, surgical repair is the treatment of choice, while for habit

A single intra-articular injection with bacterial collagenase in the right knee joints of 10-week-old male C57bl10 mice led to osteoarthritic lesions within a few weeks in these joints. The collagenase-induced osteoarthritis was characterized by severe degenerative cartilage lesions on the medial side of the femorotibial joint associated with patellar dislocation to the medial side of the joint, sclerosis of subchondral bone below the cartilage erosions, osteophyte formation, and consequent deformity of the knee joints. The osteoarthritic alterations in the collagenase model closely resembled

The knee of an infant born with congenital subluxation was dissected to delineate the pathology involved. The primary findings of importance included isolated fibrosis of the vastus intermedius muscle, absence of the gastrocnemius muscle, anterior displacement of the hamstrings and contracture of the anterior joint capsule. Comparison with previously described pathology is discussed.

Recurrent patellar dislocation is a common patellofemoral disease that affects active adolescents. The optimal surgical treatment of recurrent patellar dislocation in skeletally immature patients remains controversial. This Technical Note describes an arthroscopically assisted double-bundle medial patellofemoral ligament (MPFL) augmentation. Orthocord suture, with ideal strength and partial bioabsorbable characteristics, is used as the stabilizer to augment and protect the native MPFL during its biological healing. Under an arthroscope, patellar tunnels are created with Kirshner wire at the up

A 25-year-old man complained of severe pain in the right foot after a traffic accident. There was a wound on the medial aspect of the foot extending over the length of the first metatarsal. There was no sign of vascular compromise and sensations were intact. Radiographs showed dislocation of the first tarsometatarsal (Lisfranc) and metatarsophalangeal joints with the head of the first metatarsal facing proximally and plantarward (reverse floating first metatarsal), a segmental fracture of the second metatarsal, fracture dislocation of the third metatarsal from the metatarsophalangeal and tarso
