Neuroglial tissue in human eighth nerve specimens.

One hundred eighty nerve biopsy specimens from patients with eighth nerve tumors or other vertiginous diseases requiring vestibular neurectomy were studied. In many specimens light microscopy revealed pale areas among the myelinated nerve fibers in the neurolemmal portion of the eighth nerve. Electron microscopy showed that these were ectopic areas of glial tissue consisting of bundles of numerous cytoplasmic processes of fibrous astrocytes. The abundance of filaments within each process and the occurrence of microtubuli suggest that these processes are reactive astrocytes and accordingly are

Early bilateral eighth nerve involvement in meningococcal meningitis.

A male Navy recruit had hearing loss and bilateral otitis media. Meningeal signs, not initially present, developed approximately 48 hours after admission to the hospital. Type Y meningococcus was isolated from blood cultures drawn after two days of ampicillin administered orally. Permanent, bilateral, vestibular and auditory loss resulted, in spite of adequate doses of penicillin. This unusual presentation of bilateral eighth nerve involvement was thought to be due to a localized, bilateral meningococcal labyrinthitis.

Imaging of temporal bone inflammations in children: a pictorial review.

PURPOSE: Understanding the underlying pathophysiology and the patterns of disease spread is crucial in accurate image interpretation. In this pictorial review, the common and important inflammatory processes of the temporal bone in children will be discussed, and key computed tomography (CT) and magnetic resonance imaging (MRI) features described. METHODS: Inflammatory processes are categorized by anatomical location: the petrous apex and the inner, middle and outer ear. A complete review of the literature is provided. RESULTS: Cholesteatoma, cholesterol granuloma and mucoceles are inflammat

Cochlear Implant Impedance Fluctuation in Ménière's Disease: A Case Study.

OBJECTIVE: To contribute to the understanding of hearing fluctuation in Ménière's disease (MD) by disseminating a case study of a cochlear implanted ear with ongoing fluctuation of electrode impedances with episodic tinnitus and no associated vestibular symptoms. STUDY DESIGN: Retrospective case review. SETTING: Tertiary referral audiology clinic. PATIENT: Man, born in 1936, with a total hearing loss in the right ear because of Mumps at age 8 years and a fluctuating progressive hearing loss in the left ear because of Ménière's disease since age 63 years. INTERVENTION: Sequential bilateral

Computed tomographic findings in Ménière's disease.

Symptoms in Ménière's disease are explained by hydrops of the endolymphatic system with recurrent ruptures of the membranous labyrinth. The primary cause of the increased endolymphatic volume appears to be an imbalance between secretion and resorption of endolymph which may be due to an obstruction of the endolymphatic duct and sac, located in the vestibular aqueduct (VA). Non-visualization or narrowing of the latter have been demonstrated by conventional tomography. Also, sclerosis and hypoplasia of the retrolabyrinthine portions of the temporal bone have been documented. By high resolution c

Evidence for a vestibulo-cardiac reflex in man.

Changes in posture demand rapid cardiovascular adjustments to maintain blood pressure and volume distribution. We demonstrated a vestibulo-cardiac reflex in supine individuals by measuring electrocardiogram and arterial blood pressure after small backwards drops of the head triggered at varying intervals after the R-spike. In normal volunteers heart rate was accelerated by drops occurring within 500-600 ms of a beat, but no rapid effect was noted in patients with a vestibular defect. The speed at which the vestibular signal of head drop accelerated heart rate implies a direct reflex. Impairmen

Low-salt diet increases mRNA expression of aldosterone-regulated transporters in the intermediate portion of the endolymphatic sac.

The endolymphatic sac is a small sac-shaped organ at the end of the membranous labyrinth of the inner ear. The endolymphatic sac absorbs the endolymph, in which the ion balance is crucial for inner ear homeostasis. Of the three sections of the endolymphatic sac, the intermediate portion is the center of endolymph absorption, particularly sodium transport, and is thought to be regulated by aldosterone. Disorders of the endolymphatic sac may cause an excess of endolymph (endolymphatic hydrops), a histological observation in Meniere's disease. A low-salt diet is an effective treatment for Meniere

Clinical and radiological characteristics of patients with collapse or fistula of the saccule as evaluated by inner ear MRI.

Background: Delayed 3D-FLAIR sequences enable the distinction between the utricle and the saccule.Aims/objectives: We sought to evaluate the clinical and radiological findings in patients with no visible saccule (NVS) on 4-hour post-contrast MRI.Material and Methods: We retrospectively assessed the presence of NVS signs in 400 patients who underwent delayed inner ear MRI.Results: We reported on 28 patients with NVS. Among this group, on the NVS affected side: 14 had isolated sensorineural hearing loss (SNHL); 4 had fluctuating cochleo-vestibular disease; 3 had definite Menière's disease; 3 had

Facial nerve paralysis due to chronic otitis media: prognosis in restoration of facial function after surgical intervention.

PURPOSE: Facial paralysis is an uncommon but significant complication of chronic otitis media (COM). Surgical eradication of the disease is the most viable way to overcome facial paralysis therefrom. In an effort to guide treatment of this rare complication, we analyzed the prognosis of facial function after surgical treatment. MATERIALS AND METHODS: A total of 3435 patients with COM, who underwent various otologic surgeries throughout a period of 20 years, were analyzed retrospectively. Forty six patients (1.33%) had facial nerve paralysis caused by COM. We analyzed prognostic factors includ
