Psychosocial needs and interventions for young children with cystic fibrosis and their families.

This review summarises the experiences of young children and their families living with CF during the first five years of life following NBS diagnosis, as well as the options of psychosocial support available to them. We present strategies embedded within routine CF care that focus on prevention, screening, and intervention for psychosocial health and wellbeing that constitute essential components of multidisciplinary care in infancy and early childhood.

Identifying changes in scores on the EORTC-QLQ-C30 representing a change in patients' supportive care needs.

PurposeUsing health-related quality-of-life measures for patient management requires knowing what changes in scores require clinical attention. We estimated changes on the European Organization for Research and Treatment of Cancer Quality-of-Life-Questionnaire-Core-30 (EORTC-QLQ-C30), representing important changes by comparing to patient-reported changes in supportive care needs.MethodsThis secondary analysis used data from 193 newly diagnosed cancer patients (63 % breast, 37 % colorectal; mean age 60 years; 20 % male) from 28 Canadian surgical practices. Participants completed the Supportive

Cardiovascular Nursing Practices Following the Transition to End-of-Life Care for Patients with Heart Failure in Japan.

Heart failure patients face unique end-of-life challenges due to unpredictable disease trajectories, yet nursing practices in cardiovascular wards' palliative care practices in Japan remain underexplored. This study clarified the palliative care practices implemented by nurses in cardiovascular wards after the transition of heart failure patients to end-of-life care. Six experienced nurses participated in semistructured interviews. Data were analyzed using qualitative inductive analysis, and 4 key characteristics were identified. First, nurses provided early decision support despite prognostic

Pharmacogenomic-guided opioid prescribing in hospice: reducing toxicity while maximizing symptom relief.

Hospice care centers on interdisciplinary-driven, patient-focused, comprehensive and individualized care at the end of life. Effective pain management has been a cornerstone of hospice care, with up to 80% of patients experiencing moderate to severe pain at the end of life. Although central to hospice care, opioid prescribing faces major challenges: wide variability in response, frequent adverse effects (sedation, constipation, respiratory depression, cognitive impairment), and high rates of uncontrolled pain (20%-40% of patients inadequately relieved despite optimization). Traditional trial-a

What Facilitates or Hampers Living at Home With Advanced Dementia Until the End of Life? A Qualitative Study Using Retrospective Interviews Among Family Caregivers, General Practitioners, and Case Managers.

This study provides insight into circumstances that facilitate or hamper living at home with advanced dementia until the end of life. Interviews were held with 11 bereaved family caregivers, two general practitioners, and nine case managers, related to a total of 12 persons with advanced dementia who had recently died. Persons with dementia who lived at home until the end of life often had family caregivers that received timely support from professionals and their social network. In the cases where the person with dementia could not live at home until the end of life, safety issues, severely c

Nurses' Experience in Providing End-of-Life Care in Intensive Care Unit: A Scoping Review.

Background: Most ICU patients are in the terminal phase and require complex palliative care support and End-of-Life Care (EoLC). Nurses play a central role in symptom management, emotional support, and shared decision-making. However, evidence on nurses' experiences in providing EoLC remains fragmented and lacks a comprehensive synthesis. Objective: This review aimed to identify, map, and synthesize global evidence on ICU nurses' experiences in delivering EoLC, including challenges, coping strategies, and implications for critical care nursing practice. Methods: A scoping review was conducted

Designing and delivering palliative and end of life care research with people living with HIV: reflections from a co-produced study.

BACKGROUND: With improvements in antiretroviral therapy (ART), the life expectancy of people living with HIV is increasing towards that of the general population in many countries. As people living with HIV live longer, many are experiencing co-morbidities of age including advanced, life-limiting illnesses. Research has since highlighted both the need for and effectiveness of palliative care for people living with HIV. Therefore, as these co-morbidities become more prevalent, HIV services will increasingly need to draw on a wide range of medical disciplines, including palliative and end of lif

Living and dying: responsibility for end-of-life care in care homes without on-site nursing provision - a prospective study.

The aim of the study was to describe the expectations and experiences of end-of-life care of older people resident in care homes, and how care home staff and the healthcare practitioners who visited the care home interpreted their role. A mixed-method design was used. The everyday experience of 121 residents from six care homes in the East of England were tracked; 63 residents, 30 care home staff with assorted roles and 19 National Health Service staff from different disciplines were interviewed. The review of care home notes demonstrated that residents had a wide range of healthcare problems.

Living with invisible illness: social support experiences of individuals with systemic lupus erythematosus.

PURPOSE: Because symptoms are not immediately visible to others, systemic lupus erythematosus (SLE) is often considered an invisible illness. This invisibility can reduce the social support received from network members and adversely affect the quality of life. In the light of this, social support from formal support groups and from medical professionals can be particularly important; however, literature examining support from these sources is scarce. The purpose of this study was to explore the nature and impact of social support from medical professionals and from support groups for individu

Depressive symptoms in amyotrophic lateral sclerosis: associations with quality of life, disease progression and perceived social support-a German multicenter study.

BACKGROUND: Amyotrophic Lateral Sclerosis (ALS) is a rapidly progressive, fatal neurodegenerative disease. Patients with ALS are at increased risk of developing depressive symptoms. A clearer understanding of the associations between depressive symptoms, disease progression, health-related quality of life and perceived social support is needed to advance intervention-oriented research and improve psychosocial care in ALS. METHODS: In a multicenter cross-sectional study, patients with ALS were asked to complete patient-reported outcomes (PROs) to evaluate depressive symptoms (ALS-Depression-In

Early Percutaneous Heparin-Free Veno-Venous Extra Corporeal Life Support (ECLS) is a Safe and Effective Means of Salvaging Hypoxemic Patients with Complicated Chest Trauma.

BACKGROUND: The objective of this study was to assess the feasibility and safety of heparin-free veno-venous extracorporeal life support (VV ECLS) as a means of salvaging polytrauma patients with life-threatening hypoxemia. METHODS: This is a retrospective observational study on 7 consecutive trauma patients who underwent VV ECLS for severe chest trauma unresponsive to conventional measures. RESULTS: The median time to ECLS was within 10 hrs (IQR 2-53) of mechanical ventilation. Surgical interventions were performed before and during ECLS based on management priorities consistent with advanc

Novel multi-functional life support system.

UNLABELLED: Concepts of cardiopulmonary support (CPS), extracorporeal membrane oxygenation (ECMO), and ventricular support (VS) have been thoroughly studied and refined. These perfusion adjuncts often require multiple devices, skill sets, and significant financial burden to purchase, maintain, deploy, and use. We describe a novel system that is rapidly deployable, user-friendly, portable, safe, and economical. Over a 1-year period we have used a multi-functional life support system (MLS) in the cardiac catheterization laboratory, cardiovascular intensive care unit, and cardiac surgical suites.

Feasibility and potential value of a local governmental frail check-up program for the risk assessment of long-term care in apparently healthy older citizens: a prospective study.

BACKGROUND: Earlier identification of individuals at risk of needing long-term care can increase the opportunities for preventive/therapeutic interventions, leading to a reduced social burden. METHODS: We examined the feasibility and potential value of a frailty check-up program implemented by the local government for risk assessment of long-term care needs in 1,528 apparently healthy older (aged 64/65 years) citizens in Kobe, Japan, between August 2017 and March 2018. The program comprised a questionnaire on general frailty (Comprehensive score) and evaluation of other frailty-related measur

Reflective, holistic care after radical penectomy.

This case study describes the management of a patient with complex problems by a urology nurse specialist. Penile amputation and the creation of a colostomy had a severe impact on body image so the patient's care required a holistic approach. He also needed the support of community nurses, and collaboration between them and the urology nurse specialist prevented fragmentation of his care in the end stages of his life.

Reducing Systemic Risks in a Traumatic Panfacial Injury Patient.

Panfacial trauma refers to injuries caused by high-energy mechanisms to two or more regions of the craniofacial skeleton, including the frontal bone, the midface, and the occlusal unit. As with any trauma, Advanced Trauma Life Support protocols should be followed in unstable patients. For the patient with panfacial traumatic injury, advanced perioperative care or critical care is frequently required. This article describes surgical critical care for panfacial injuries, a component of the acute-care surgery model, to reduce systemic risks, improve the patient's condition, and enable a successfu

The challenges of dysphagia in treating motor neurone disease.

Motor neurone disease (MND) is a relatively rare degenerative disorder. Its impacts are manifested in progressive loss of motor function and often accompanied by wider non-motor changes. Swallowing and speech abilities are frequently severely impaired. Effective management of dysphagia (swallowing difficulty) symptoms and nutritional care requires a holistic multidisciplinary approach. Care must be patient focused, facilitate patient decision making, and support planning towards end of life care. This article discusses the challenges of providing effective nutritional care to people living wit

Evaluation of Reablement Home Care: Effects on Care Attendants, Care Recipients, and Family Caregivers.

BACKGROUND: The traditional home care model entails caring "for" people with disabilities, not "with" them. Reablement care has been applied to long-term care, but the evidence for care attendants, home care recipients, and family caregivers simultaneously is limited. METHODS: First, a survey was conducted to explore the needs of home care recipients and family caregivers to achieve independence at home to develop the reablement home care model for home care. Then, an intervention with two groups was implemented. The experimental group included a total of 86 people who participated in the rea
