[Diffuse large B-cell lymphoma with acquired von Willebrand syndrome: a case report and literature review].

This article reports the diagnosis and treatment of a patient with diffuse large B-cell lymphoma (DLBCL) initially manifested as acquired von Willebrand syndrome (AvWS), along with a literature review. The patient, a 22-year-old male, was diagnosed with hereditary von Willebrand disease at the initial stage due to repeated mucosal bleeding, and was later diagnosed with DLBCL (non germinal center type, Ann Arbor stage Ⅳ) due to chest wall mass. Through chemotherapy combined with autologous hematopoietic stem cell transplantation and zebutinib maintenance therapy, the patient achieved sustained

Selinexor combined with R-GDP as salvage therapy in relapsed/refractory diffuse large B-cell lymphoma.

Relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL) remains a therapeutic challenge with poor prognosis. Selinexor, a selective inhibitor of nuclear export (XPO1), has shown activity in this setting. We retrospectively evaluated the efficacy and safety of selinexor combined with R-GDP (rituximab, gemcitabine, dexamethasone, and cisplatin) as second-line therapy in 22 patients with R/R DLBCL treated at Fudan University Shanghai Cancer Center between January 2023 and August 2023. Patients were scheduled to receive 3 cycles of selinexor plus R-GDP, and subsequently followed by high-d

[Diffuse large B-cell lymphoma with central pontine myelinolysis without rapid changes in serum sodium levels].

Central pontine myelinolysis (CPM) typically arises from the rapid correction of hyponatremia. A literature search showed that CPM secondary to malignant lymphoma is rare, with only 11 reported cases. An 80-year-old woman presented with anorexia as her primary symptom and was subsequently diagnosed with diffuse large B-cell lymphoma (DLBCL) following an axillary lymph node biopsy. Shortly after treatment initiation, she experienced convulsions and altered consciousness. Brain MRI revealed a symmetrical lesion in the pons, but gadolinium enhancement was not observed and cerebrospinal fluid cyto

An Incidental Cavitary Pulmonary Lesion Unveiling Diffuse Large B-cell Lymphoma in an Undiagnosed HIV Patient.

Diffuse large B-cell lymphoma (DLBCL) rarely presents with pulmonary involvement, and cavitary lung lesions as the initial manifestation are exceptionally uncommon. We report the case of a 65-year-old man who presented to the emergency department after a mechanical fall, during which imaging revealed an incidental thin-walled cavitary lesion in the left lung apex. The patient was asymptomatic and was lost to follow-up. Six months later, he presented again with a new right axillary mass. Imaging demonstrated a persistent cavitary lesion with bilateral axillary lymphadenopathy and a new solid lu

Recurrent Hypoglycemia in a 67-Year-Old Woman with CD5- Positive Diffuse Large B-Cell Lymphoma.

Hypoglycemia is a rare complication of diffuse large B-cell lymphoma. We are presenting a case of 67-year-old woman presented to her primary care physician with fatigue and hyperhidrosis. Laboratory evaluation revealed a glucose level of 1.9 mmol/L. Computed tomographic scan of the abdomen and subsequent positron emission tomographic scan revealed extensive lymphadenopathy. The patient was then diagnosed with CD5-positive-diffuse large B-cell lymphoma and developed recurrent hypoglycemia despite continuous infusion of glucose. Following immunochemotherapy, hypoglycemia was resolved. Several ex

[Diffuse large B-cell lymphoma with heart involvement: the relevance of the multidisciplinary and multiparameter approach].

Lymphoma patients are at high risk of cardiovascular events due to anthracycline cardiotoxicity and, in rare cases, related to heart infiltration. The presence of cardiac masses adds further complexity to the management of lymphoma patients beyond myocardial chemotherapy-related toxicity, given possible unpredictable acute complications such as arrhythmias, atrioventricular block, myocardial ischemia, pericardial effusion and cardiac tamponade. Here we describe the clinical presentation and successful multidisciplinary management of diffuse large B-cell lymphoma with multifocal cardiac involve

Molecular Subtypes and the Role of TP53 in Diffuse Large B-Cell Lymphoma and Richter Syndrome.

Diffuse large B-cell lymphoma (DLBCL) is the most common lymphoid malignancy and a heterogeneous entity comprised of several biologically distinct subtypes. Recently, novel genetic classifications of DLBCL have been resolved based on common mutational patterns indicative of distinct pathways of transformation. However, the complicated and costly nature of the novel classifiers has precluded their inclusion into routine practice. In view of this, the status of the TP53 gene, which is mutated or deleted in 20-30% of the cases, has emerged as an important prognostic factor for DLBCL patients, set

Experience of multi-disciplinary treatment of multiple cerebellar diffuse large B-cell lymphoma: A case report.

RATIONALE: Primary central nervous system lymphoma (PCNSL) is a rare, highly malignant form of non-Hodgkin lymphoma categorized under the diffuse large B-cell type. It accounts for merely 1% of all non-Hodgkin lymphoma cases and comprises approximately 3% of all brain tumors. The involvement of the cerebellum is observed in only 9% of these cases. Recently, we came across an unusual instance: a young man presenting with multiple lesions located specifically within the cerebellum. PATIENT CONCERNS: A 26-year-old male was admitted to the hospital due to severe headaches. He has a medical histor

Skeletal muscle index impacts the treatment outcome of elderly patients with diffuse large B cell lymphoma.

Sarcopenia is a crucial factor in the physical fitness of elderly individuals. This study investigated the prognostic values of multiple parameters of sarcopenia in association with established prognostic factors in elderly Japanese patients with diffuse large B cell lymphoma (DLBCL). As candidate indicators for sarcopenia, the skeletal muscle index (SMI) (cm2 /m2 ), the psoas muscle index, the erector spinae muscle index, the visceral fat index, the subcutaneous fat index, and the visceral to subcutaneous fat area ratio at the third lumbar level were assessed by computed tomography at their i

Clinicopathological and Molecular Characteristics of Rare EBV-associated Diffuse Large B-cell Lymphoma With IRF4 Rearrangement.

Epstein-Barr virus (EBV)-positive diffuse large B-cell lymphoma (DLBCL) is a rare form of aggressive B-cell lymphoma with limited molecular information reported regarding interferon regulatory factor 4 ( IRF4 ) status. Here, we presented 3 EBV-positive DLBCL cases with IRF4 rearrangement (EBV+DLBCL- IRF4 -R) verified by fluorescence in situ hybridization (FISH). Three patients, including 1 male and 2 females (median age: 64 y; range: 45 to 68 y), had normal immune function. During a median follow-up of 12 months (range: 0 to 24 mo), 2 patients succumbed to the disease, and 1 patient achieved c

Haemophagocytic lymphohistiocytosis (HLH) secondary to diffuse large B-cell lymphoma: uncommon complication of a common disease.

Haemophagocytic lymphohistiocytosis (HLH) is a rare and life-threatening syndrome characterised by dysregulated immune activation, inflammation and multiorgan failure. In adults, HLH often arises secondary to an underlying trigger including infections, malignancies, autoimmune conditions, immunodeficiencies, chemotherapy or bone marrow transplantation. Diffuse large B-cell lymphoma (DLBCL) contributes to about 5-10% of malignancy-associated HLH. This case report highlights the clinical course of an elderly woman with HLH associated with DLBCL, who had an initial response in HLH with mini-R-CHO

Programmed death ligand 1 expression in diffuse large B cell lymphoma: correlation with clinicopathological prognostic factors.

BACKGROUND: The prognostic value of the level of programmed death ligand 1 (PD-L1) expression in non-Hodgkin lymphoma (NHL) is still debatable. This study examined the effect of the level of PD-L1 expression on the clinicopathological characteristics and prognosis of diffuse large B cell lymphoma (DLBCL). METHODS: A retrospective study was conducted on formalin-fixed paraffin-embedded tissue blocks of one hundred de novo DLBCL patients diagnosed from 2013 to 2016. PD-L1 expression was defined by a modified Combined-Positive Score (CPS) and their medical records were reviewed to collect their

Diffuse large B-cell lymphoma successfully treated with amplified natural killer therapy alone: A case report.

BACKGROUND: The prognosis of patients with advanced diffuse large B-cell lymphoma (DLBCL) is poor, with a 5-year survival rate of approximately 50%. The mainstay of treatment is multidrug combination chemotherapy, which has been associated with serious side effects. Amplified natural killer (ANK) cell therapy amplifies and activates natural killer (NK) cells to attack only malignant tumors. As ANK cells attack programmed death ligand 1 (PD-L1)-positive tumor cells, ANK therapy is considered effective against adult T-cell lymphoma and malignant lymphoma. CASE SUMMARY: Herein, we report a case

Prognostic Significance of p53 and p63 in Diffuse Large B-Cell Lymphoma: A Single-Institution Experience.

Diffuse large B-cell lymphoma (DLBCL) is the most common lymphoma in adults. We evaluated the immunohistochemical (IHC) expression of p63 and p53 in DLBCL and their significance on overall survival (OS) and progression-free survival (PFS). We conducted a retrospective cohort study of 177 patients with DLBCL who presented to Mount Sinai Medical Center of Florida (Miami Beach, Florida) between 2010 and 2020. IHC staining for p63 and p53 protein expression was performed. A significant correlation was found between p63 positivity and p53 expression, p53/p63 co-positivity, Ki-67 proliferation index

An immunohistochemical study of diffuse large B-cell lymphoma with molecular subtyping based on Hans algorithm.

INTRODUCTION: Diffuse large B-cell lymphoma (DLBCL) is the most common type of B-cell lymphoma accounting for about 40% of all lymphomas. The international prognostic index (IPI), which relies on clinical and laboratory parameters, is used as a prognostic tool in DLBCL. In this study, we have included cases of DLBCL not otherwise specified (NOS) type to test the usefulness of the biological subclassification of DLBCL by immunohistochemistry (IHC) using the Hans algorithm into the germinal center B-like (GCB) type and nongerminal center (non-GCB) type. We correlated the subtypes with the demogr
