The sequential treatment of temporomandibular joint ankylosis with secondary deformities by distraction osteogenesis and arthroplasty or TMJ reconstruction.

The purpose of this study was to evaluate the sequential treatment of patients with temporomandibular joint (TMJ) ankylosis and secondary deformities by distraction osteogenesis and subsequent arthroplasty or TMJ reconstruction. This study included 40 patients treated at a stomatological hospital in China; they ranged in age from 9 to 53years (mean age 24.5years). Ten of these patients were diagnosed with unilateral TMJ ankylosis and 30 with bilateral TMJ ankylosis. Twenty-seven patients also presented obstructive sleep apnoea-hypopnoea syndrome (OSAHS). All patients underwent distraction oste

Microsurgical reconstruction of the mandible in a patient with evans syndrome: a case report and review of the literature.

In this report, we describe the first successful case of microvascular free tissue transfer in a patient with Evans Syndrome (ES), a rare form of idiopathic thrombocytopenic purpura (ITP) and associated autoimmune hemolytic anemia (AIHA). Microvascular surgery in the setting of ES is likely to have higher complication rates because of the increased risk of postoperative bleeding and free flap thrombosis. The case presented here opens up to the feasibility of microvascular reconstruction of patients with coagulation disorders like ES. Every effort should be made to control for hemolytic, thromb

Nasoalveolar molding in cleft care: is it efficacious?

In the era of evidence-based medicine, new treatment protocols and interventions should be routinely evaluated for their efficacy by reviewing the available evidence. In the cleft literature, nasoalveolar molding has garnered attention over the last decade as a new option for improving nasal form and symmetry before primary surgical repair. Systematic review of the evidence is, however, currently lacking. This review evaluates whether nasoalveolar molding can improve nasal symmetry and form toward the norm, as well as whether nasoalveolar molding demonstrates advantages over other protocols in

Condylar remodelling in a severe class II deep bite dento-skeletal malocclusion treated with an ortho-surgical approach: A five-year follow-up case report.

Condylar displacement and remodelling are phenomena closely related to orthognathic surgery and critically involved in occlusal and skeletal stability. The aim of the present case report is to describe over time condylar displacement and surface remodelling after bilateral split sagittal osteotomy (BSSO) in an adult patient with severe class II skeletal malocclusion treated with ortho-surgical approach. A male of 21years comes to our observation. The extraoral examination shows a symmetrical square-shaped face, a convex profile, an acute nasolabial angle and a deep labiomental fold. Intraoral

Realization of masticatory movement by 3-dimensional simulation of the temporomandibular joint and the masticatory muscles.

Masticatory muscles are closely involved in mastication, pronunciation, and swallowing, and it is therefore important to study the specific functions and dynamics of the mandibular and masticatory muscles. However, the shortness of muscle fibers and the diversity of movement directions make it difficult to study and simplify the dynamics of mastication. The purpose of this study was to use 3-dimensional (3D) simulation to observe the functions and movements of each of the masticatory muscles and the mandible while chewing. To simulate the masticatory movement, computed tomographic images were

Distraction osteogenesis promotes temporomandibular joint self-remodeling in the treatment of mandibular deviation caused by condylar ankylosis.

Craniofacial deformity and malocclusion are primary concerns following temporomandibular joint ankylosis (TMJa) in growing patients, and they pose even greater challenges in adult patients. The treatment objectives always involve restoring proper jawbone structure, achieving stable occlusion, and attaining satisfactory joint mobility. This report presents a 4-year follow-up of an adult patient with TMJa-induced mandibular deviation, who underwent a combined treatment approach involving distraction osteogenesis (DO) and orthodontic-orthognathic surgery. Orthodontic treatment resulted in favorab

Advancement cheiloplasty for reconstruction of floor of mouth defects.

In many cases of carcinoma of the floor of mouth, oncologic resection includes marginal mandibulectomy. Reconstruction poses a significant challenge. Requirements include coverage with thin but supple tissue to allow for dental implant or denture, and recreation of a mobile tongue and sensate floor of mouth gutter. Reconstructive efforts have ranged from skin grafts to free flaps, with variable success in fulfilling the above-mentioned requirements. This paper describes the preferred technique of the author, in which external mandibular periosteum is saved and elevated with a submucosal flap o

Dental implant treatment in vascularized bone flaps after jaw reconstruction.

Jaw defects caused by various reasons often seriously affect appearance and function. The goal of the treatment of oral and maxillofacial tumors should include the cure of the tumor and the restoration of premorbid function. The development of microsurgery and digital surgery technology has promoted the development of jaw reconstruction with vascularized free bone flap. Good appearance and improved predictability could be obtained with the help of preope-rative visual design. How to rehabilitate occlusal function on the reconstructed jaw and improve the quality of life of patients has become a

Mega Fibula Free Flap for Reconstruction of an Extensive Mandibulofacial Defect.

OBJECTIVE: Extensive mandibulofacial defects can be challenging to reconstruct. We present the case of a complex mandibulofacial defect reconstructed with a mega, chimeric fibula free flap. METHODS: Ablation of the oral cavity tumor resulted in a large defect involving mandible, floor of mouth, and tongue. Skin of the chin and neck as well as the lower lip were also resected. A fibula free flap was harvested with the skin paddle involving most of the lateral compartment. RESULTS: The fibula free flap was split into proximal (80 cm2) and distal (120 cm2) skin paddle islands, which were suppli

How to achieve a balanced and delicate lower third of the face in Orientals by mandibular contouring.

INTRODUCTION: For East Asians, correction of square jaw has become one of the commonly performed procedures in aesthetic surgery. While reviewing unsatisfactory cases of mandibular reduction, the authors discovered that failure was largely due to surgeons generally focussing too much on treatment of the posterior part of the mandible, and ignoring the aesthetic significance of the harmonious and smooth overall curve from the anterior mandible to the inferior margin of the mandible. Thus, our group proposes that a coordinated, balanced and smooth jaw line is considered a key factor in Oriental

A sizeable aneurysmal bone cyst of the mandibular ramus on 15-year-old patient. Intraoral approach.

Aneurysmal bone cysts (ABCs) are benign, non-neoplastic bone lesions, which contain blood and demonstrate a destructive pattern. They rarely occur in the head and neck region, with the mandible being the most common site of craniofacial origin. They develop in the second decade of life and their etiology is obscure. ABCs can sometimes demonstrate distinct radiological features and imaging is also necessary for preoperative planning. Surgery is the mainstay of treatment. The ABC should be excised in toto and usually osteotomies and primary reconstruction are necessary. We present a case of size

Comparison of the aesthetic impact on the mandibular angle of two types of osteotomy.

The growing aesthetic demand for projection and definition of the mandibular angle has challenged oral and maxillofacial surgeons, requiring new approaches to osteotomies during orthognathic surgery. This study was undertaken to compare the conventional sagittal split osteotomy (SSO) with anterior oblique osteotomy (AOO) regarding their effects on the mandibular angle in both bone and soft tissue outcomes. A longitudinal observational cohort study was conducted with patients who underwent orthognathic surgery, including pre- and postoperative computed tomography and photographic examinations w
