Congenital aural atresia surgery: transmastoid approach, complications and outcomes.

Repair of complete congenital aural atresia (CAA) could be a challenging procedure due to complications reported with CAA surgery such as facial nerve palsy, canal stenosis, graft lateralization, sensorineural hearing loss or the difficulty involved in the surgical technique. From 2006 to 2009, we used a one stage-modified transmastoid approach for surgical repair of 33 ears with complete CAA via a non-randomized controlled clinical trial. Some modifications in the technique of mastoidectomy, ossiculoplasty, fascia and skin grafting and meatoplasty have been described. Patients were followed u…

Lateral sinus thrombosis: a review of seven cases and proposal of a management algorithm.

OBJECTIVE: To evaluate the demographics, diagnosis, management, and outcomes of lateral sinus thrombosis in a pediatric population, and to p … h antibiotics (six of seven patients with i.v. ceftriaxone). Five of seven were treated with simple mastoidectomy and concurrent middle ear ventilation tubes; two patients received only medical treatment. The average follow up was 114 months (range 33-387 months). Two patients had long-term sequela: one had persistent mild lateral gaze diplopia and another had …

[Thrombophlebitis of venous sinuses in otitis media].

Thrombophlebitis of dura venous sinuses is a rare intracranial complication of otitis media, which may be sometimes accompanied by symptoms … s exposure. Thrombectomy and jugular vein ligation is performed when sepsis or thrombus is present. Mastoidectomy and tympanic cavity drainage is performed in cases with lateral sinus thrombosis in acute mastoiditis. Intravenous antibiotics therapy should be continued for 2 weeks. Anticoagulants should be given taking into consideration parameters of coagulatio …

The Role of Stepwise Imaging in the Diagnosis and Management of Cholesteatoma in a Resource-Limited Setting: A Case Report.

BACKGROUND: Cholesteatoma is a non-neoplastic but destructive lesion of the temporal bone that can lead to significant complications if not … olesteatoma. Based on this definitive diagnosis, the patient underwent a left-sided canal wall down mastoidectomy. CONCLUSION: This case highlights the pivotal role of a stepwise imaging strategy in a resource-limited environment. While CT identified the extent of bony destruction, the selective use of non-EPI DWI MRI provided a definitive diagnosis, confident …

Unmasking Villaret's syndrome: a diagnostic challenge of glomus jugulare mimicking mastoiditis.

INTRODUCTION: Villaret's syndrome involves unilateral palsy of cranial nerves IX to XII, often with Horner's sign, commonly caused by benign neoplasms or vascular issues. This report highlights glomus jugulare-induced Villaret syndrome, stressing careful evaluation for persistent otologic symptoms. CASE PRESENTATION: A 43-year-old woman with a history of chronic suppurative otitis media (CSOM) presented with recurrent ear discharge, pain, and severe mixed hearing loss. Despite a left tympanoplasty, symptoms persisted, leading to a canal wall down mastoidectomy. Six months later, she developed…

A new 3D-printed temporal bone: 'the SAPIENS'-specific anatomical printed-3D-model in education and new surgical simulations.

PURPOSE: Otology and neuro-otology surgeries pose significant challenges due to the intricate and variable anatomy of the temporal bone (TB) … dissection. Specific areas of excellence included external contour, sigmoid sinus contour, cortical mastoidectomy simulation, and its utility as a surgical practice simulator. CONCLUSION: We have designed and developed a 3D model of the temporal bone that closely resembles the human temporal bone. This model enables the surgical dissection of the middle ear an …
