Rechargeable or Nonrechargeable Deep Brain Stimulation in Dystonia: A Cost Analysis.

OBJECTIVE: Deep brain stimulation of the internal Globus Pallidus (GPi DBS) delivered by an implantable neurostimulator (INS) is an established, effective, and safe treatment option for patients with medically refractory primary dystonia. Compared to other DBS targets, the battery life of the INS is substantially shorter due to the higher energy demands required to penetrate the GPi resulting in faster battery depletion and more frequent hospitalizations for INS replacement. We, therefore, performed a cost analysis to compare a rechargeable DBS system, Activa®RC, with nonrechargeable systems,

Medication Preparation Errors Involving Very Small and Difficult-to-measure Volumes in a Pediatric Hospital: A 14-year Descriptive Study.

PURPOSE: Medication preparation errors are a major source of preventable harm in pediatric hospitals. Oral liquid medications, especially those involving difficult-to-measure or very small volumes, may increase the risk of such errors, yet supporting evidence is limited. This study aims to determine whether difficult-to-measure and very small oral liquid medication volumes are associated with higher rates of preparation errors in a pediatric hospital pharmacy. METHODS: We conducted a retrospective cohort study of all oral liquid medication preparations produced between January 2010 and Decemb

The burden of hospital-acquired legionellosis in German teaching hospitals.

PURPOSE: Legionella pneumophila is a recognized cause of hospital-acquired pneumonia particularly in immunocompromised patients. Significant resources are dedicated to the monitoring and control of Legionella contamination in hospital water systems. However, the actual risk of nosocomial Legionella infections in German university hospitals, which represent large tertiary care institutions, remains poorly defined. The aim of this study was to quantify the incidence of nosocomial legionellosis in German university hospitals. In addition, the extent of routine testing of hospital drinking water s

Hip fractures among the elderly: factors associated with in-hospital mortality.

Factors associated with mortality were examined for 27,370 hip fracture patients aged 65 years or older in Maryland hospitals, using discharge data for 1979-1988. Variables of interest included sociodemography, principal medical and injury diagnoses, E-code, year, disposition, and hospital. For both white males and white females, the hip fracture rate doubled with each 5-year increment in age. The overall proportion who died during hospitalization was 4.9% (n = 1,339). After multivariate adjustment, there remained a substantially increased risk of death for males. The relative odds (RO) of dyi

High Readmission Rate in a Patient With Chronic Heart Failure, End-Stage Renal Disease, and Chronic Pulmonary Obstructive Disease.

Hospital readmission is common for patients with multiple chronic comorbidities, but it is a burden to both patients and healthcare systems. While this is a multifactorial issue, it is important to identify some of the reasons that contribute to high readmission rates and advocate for concrete interventions that can be implemented. We report the case of a 68-year-old female with congestive heart failure with severely reduced ejection fraction, chronic obstructive pulmonary disease requiring home oxygen, and end-stage renal disease on hemodialysis who was readmitted more than 30 times in the pr

[CULTURE OF SAFETY IN THE HOSPITAL].

The acute care hospital, with its hospitalization departments, diagnostic centers, outpatient clinics, operation rooms and intensive care units, is a very complex industry with the goal of taking care of patients in every stage of their disease, starting with prevention and continuing with diagnosis and treatment. Diagnosis and treatment processes have many stages; most of them depend on high technology and advanced science. Studies in the USA demonstrated 98,000 to 241,000 cases of mortality due to medical errors and complications, third place after mortality due to cardiac diseases and cance

Acupuncture in the Management of Medication Overuse and Drug-induced Aseptic Meningitis Headache: A Case Report.

Headache disorders are burdensome, both in terms of the number of people they affect, and in terms of associated healthcare spending. This report presents a 36-year-old female admitted to a tertiary university hospital with a primary complaint of intractable headache, caused by a combination of medication overuse headache, and headache secondary to aseptic meningitis. During her hospital stay, opioid analgesic doses were initially increased without success in an attempt to control her headache. Despite multiple medication trials the patient's headache failed to improve. On day ten of her hospi

Supporting families in their child's journey with neonatal encephalopathy and therapeutic hypothermia.

Neonates and families face challenges in hypothermic therapy, including trauma to parents, extreme emotions, and unfamiliarity with the medical system. Communication is an essential element to supporting parents while their children are in the NICU, and beyond, building the foundation for the ongoing relationship the family has with the medical system. Significant consideration needs to be given to the critical element of integrating the family into the care of a baby being treated with therapeutic hypothermia. Clinicians can promote healing of accumulated traumas of parents through ensuring p

Concentration of hospital utilization in the capital area among elderly inpatients from non-capital regions in South Korea: Before and during the COVID-19 pandemic.

Hospital utilization in South Korea exhibits a significant concentration in the capital area, posing a persistent social issue. Given the heightened vulnerability of elderly patients to COVID-19, their continued travel to capital-area hospitals during the pandemic could have facilitated rapid virus dissemination, necessitating a review of the regional healthcare delivery system. This study therefore aimed to analyze the sustained concentration of hospital utilization in the capital area among regional elderly patients and to identify the characteristics of those who utilized these facilities d

Differences in the delivery of medications for opioid use disorder during hospitalization by racial categories: A retrospective cohort analysis.

Background: As the drug-related overdose crisis and COVID-19 pandemic continue, communities need increased access to medications for opioid use disorder (MOUD) (i.e., buprenorphine and methadone). Disparities in the type of MOUD prescribed or administered by racial and ethnic categories are well described in the outpatient clinical environment. It is unknown, however, if these disparities persist when MOUD is provided in acute care hospitals. Methods: This study assessed differences in the delivery of buprenorphine versus methadone during acute medical or surgical hospitalizations for veterans
