ICRS scores worsen between 2-year short term and 5-year mid-term follow-up after transtibial medial meniscus root repair despite maintained functional outcomes.

PURPOSE: The purpose of this study was to evaluate the mid-term results of posterior medial meniscal root tear (PMMRT) repair through assessment of functional outcome scores and magnetic resonance imaging (MRI). METHODS: This was a single-center, retrospective study evaluating patients that had undergone a PMMRT. This was a follow-up to a previously published 2-year outcome study (all original patients were invited to participate). Clinical outcomes included pre- and postoperative International Knee Documentation Committee (IKDC) and Lysholm scores. Root healing, meniscal extrusion, and carti

Arthroscopic meniscectomy versus non-surgical or sham treatment in patients with MRI confirmed degenerative meniscus lesions: a protocol for an individual participant data meta-analysis.

INTRODUCTION: Arthroscopic partial meniscectomy (APM) after degenerative meniscus tears is one of the most frequently performed surgeries in orthopaedics. Although several randomised controlled trials (RCTs) have been published that showed no clear benefit compared with sham treatment or non-surgical treatment, the incidence of APM remains high. The common perception by most orthopaedic surgeons is that there are subgroups of patients that do need APM to improve, and they argue that each study sample of the existing trials is not representative for the day-to-day patients in the clinic. Theref…

Effect of intra-articular tibial tunnel aperture positioning on the clinical outcomes after repair of medial meniscus posterior root tear with pull-out repair technique.

BACKGROUND: The impact of tibial tunnel aperture position on outcomes after medial meniscus posterior root (MMPR) repair with a transtibial pull-out technique remains unclear. This study evaluated the association between intra-articular tibial tunnel aperture position and postoperative outcomes after transtibial pull-out MMPR repair using intra-articular assessment. METHODS: Patients who underwent arthroscopic MMPR repair between March 2010 and February 2022 were retrospectively reviewed. The tibial tunnel aperture was classified relative to the MMPR footprint midpoint as anterior (Group A; 2…

The Use of Fibrin Clot During Meniscus Repair in Young Patients Reduces Clinical Symptom Rates at 12-Month Follow-Up: A Pilot Randomized Controlled Trial.

Background and Objectives: The menisci are crucial fibrocartilaginous structures of the knee joint and have to be repaired in case of a tear. However, not all meniscal tears heal, even in young patients. Fibrin clot (FC) started to be used to reduce the failure rates following meniscus repair. The purpose of this study is to evaluate and compare outcomes after isolated arthroscopic meniscal repair augmented with FC versus without FC. Materials and Methods: Fifty-nine patients aged under 19 with isolated meniscal tears were randomized into two groups: one group underwent the meniscal repair wit…

Oral administration of undenatured type II collagen significantly inhibits arthritis-associated pain signal in a mouse model of collagen antibody-induced arthritis and meniscus removal.

Osteoarthritis (OA) is joint pain caused by persistent low-grade inflammation, and is characterized by bone remodeling, synovitis, and articular cartilage degeneration. This study focused on undenatured type II collagen (UC-II®) and examined its influence on OA-associated pain. The increased Ca2+ influx to typical nociceptive receptors TRPA1 and TRPV1 in the peripheral neurons of dorsal root ganglia was significantly inhibited by UC-II® treatment and similar inhibitory patterns were confirmed in vivo pain behavior tests in a mouse model of arthritis. Our findings suggest the effectiveness of U

High tibial osteotomy and concurrent medial meniscus root repair provides improved objective outcomes compared to high tibial osteotomy alone for knee osteoarthritis: A systematic review.

PURPOSE: High tibial osteotomy (HTO) is a cornerstone treatment for medial compartment osteoarthritis (OA) in active patients with varus deformity. However, managing the combination of varus alignment and the medial meniscus posterior root tears (MMPRT) remains controversial. The purpose of this systematic review was to synthesise data from recent comparative studies and evaluate whether adding an MMPRT repair to an HTO enhances outcomes, particularly with respect to meniscal healing, cartilage preservation, and patient function. METHODS: We systematically searched MEDLINE, Embase, Scopus, an…

Case report: unusual posteromedial capsular lesion with posterior lateral meniscus root tear in two patients with constitutional genu recurvatum presenting after an acute ACL injury.

Ramp lesions of the medial meniscus and posterior lateral meniscus root tears (LMPRT) can be present simultaneously in up to 8% of patients undergoing anterior cruciate ligament (ACL) reconstruction. The prevalence of these complex and highly unstable meniscal tears increases exponentially with the severity of the injury. The posteromedial capsule (PMC) has often been disregarded in the past when discussing ligamentous and meniscal injuries, but the recent interest in ramp lesions has drawn surgeons' attention to the posteromedial structures of the ACL injured knee. While the meniscocapsular j…

Editorial Commentary: Suture Anchor Fixation for Posterior Medial Meniscus Root Repair Continues to Show Biomechanical Favorability, but It Is Not Yet Time to Toss the Tibial Tunnel.

Meniscal root tears are defined as a disruption or avulsion within 1 cm of the tibial root attachment. Posterior medial meniscus root tears (PMMRT) represent a distinct injury pattern, typically affecting sedentary, middle-aged patients through degenerative mechanisms. These injuries produce biomechanical consequences similar to total meniscectomy, including increased medial compartment peak contact pressures and decreased tibiofemoral contact area. The biomechanical and clinical consequences have led to PMMRT's being labeled as a "silent epidemic." By restoring hoop tension, PMMRT repair impr…

Visualization of the posteromedial compartment in the knee: Comparison between a posterolateral transseptal approach with a standard anterior transnotch approach when repairing posterior lesions of the medial meniscus.

INTRODUCTION: Visualization and exposure of the medial meniscus in the posterior compartment during knee arthroscopy can be challenging. Using a posterolateral transseptal approach can help to have a better visualization to ensure a better meniscal repair in the posterior compartment. HYPOTHESIS: The posterolateral transseptal approach allows a better visualization of the posterior segment of the medial meniscus when compared to a trans notch approach. MATERIAL AND METHODS: A controlled laboratory study using 12 human cadaveric knees were included in this arthroscopic study. The first step w…

Absolute concentrations of mRNA for type I and type VI collagen in the canine meniscus in normal and ACL-deficient knee joints obtained by RNase protection assay.

Relatively little is known about the cellular and molecular responses of the knee joint meniscus to joint injury, despite the functional importance of the tissue. We investigated how meniscus cells respond to joint injury in the early stages of post-traumatic osteoarthritis by characterizing the changes in matrix gene expression in menisci at 3 and 12 weeks post-surgery in dogs in which the anterior cruciate ligament (ACL) in one joint was transected and the other unoperated joint served as a control. Changes in the total RNA and DNA concentrations of the menisci were determined. Absolute conc…

Mild to Moderate Varus Alignment in Relation to Surgical Repair of a Medial Meniscus Root Tear: A Matched-Cohort Controlled Study With 2 Years of Follow-up.

BACKGROUND: Increased varus alignment of the lower extremity is known to be a poor prognostic factor for the surgical repair for a medial meniscus root tear (MMRT). However, given the concept of constitutional varus, which is present in a substantial portion of the normal population, the generally accepted surgical indication for MMRT concerning a varus alignment of 5° may be unnecessarily narrow. PURPOSE: To compare the surgical outcomes of arthroscopic transtibial pullout repair of MMRT according to the degree of varus alignment of the lower extremity. STUDY DESIGN: Cohort study; Level of …

Avoiding Injury to the Popliteal Neurovascular Bundle in All-Inside Suturing of the Posterior Horn of the Lateral Meniscus: A Magnetic Resonance Imaging Assessment of Portal Selection and Safety.

PURPOSE: We assessed the risk of injury to the popliteal neurovascular bundle (PNVB) while suturing the posterior horn of the lateral meniscus (PHLM). METHODS: We simulated all-inside suturing of the PHLM using magnetic resonance imaging of 60 knees. Lines were drawn from the medial and the lateral edges of the patellar tendon to the PHLM at increasing distances from the posterior cruciate ligament (PCL) to simulate suturing device trajectory. Distance from each line to the PNVB was measured (d). A similar analysis was performed using lines drawn from 1 cm medial and 1 cm lateral to the patel…

Effects of radial tears and partial meniscectomy of lateral meniscus on the knee joint mechanics during the stance phase of the gait cycle--A 3D finite element study.

The purpose of the current study was to evaluate influences of radial tears and partial meniscectomy of lateral meniscus on the knee joint mechanics during normal walking by using computational modeling. A 3D geometry of a knee joint of a healthy patient was obtained from our previous study, whereas the data of normal walking were taken from the literature. Cartilage tissue was modeled as a fibril reinforced poroviscoelastic material, whereas meniscal tissue was modeled as a transverse isotropic elastic material. The realistic gait cycle data were implemented into the computational model and t…

Residual meniscal extrusion was not associated with 2-year clinical outcomes after posterior lateral meniscus root repair in a cohort predominantly undergoing anterior cruciate ligament reconstruction.

INTRODUCTION/OBJECTIVES: The clinical significance of residual meniscal extrusion after posterior lateral meniscus root repair, particularly with concomitant anterior cruciate ligament (ACL) reconstruction, remains uncertain. This study evaluated longitudinal clinical and imaging outcomes after transtibial pull-out repair of traumatic posterior lateral meniscus root tears and examined whether residual extrusion was associated with the final Lysholm score or change from baseline at 2-year follow-up. METHODS: Thirty patients with arthroscopically confirmed traumatic posterior lateral meniscus r…
