The safety and efficacy of ototopical preparations, such as ofloxacin, in the treatment of CSOM in infants and children do not currently have approval by the FDA even though ofloxacin is FDA-approved for treatment of CSOM that occurs in adults. However, treatment with ofloxacin is FDA-approved when AOM occurs in children who have a tympanostomy tube in place, and effective treatment of this acute infection should prevent the subsequent development of CSOM.

We undertook a retrospective study to examine our experience with acute mastoiditis over a 12-year period. Fifty-eight cases were identified in children aged 3 months to 15 years. Acute mastoiditis was the first evidence of otitis media in 54% of our patients. Pain and fever lasting for more than a median period of 4 days were most likely to be the harbingers of incipient acute mastoiditis. Streptococcus pneumoniae was the most common organism recovered from the cultures. All children were treated with intravenous antibiotics; 41 children were managed with an adjunctive drainage procedure. No

The recent issuance of guidelines for the diagnosis and treatment of otitis media with effusion in young children is an effort to provide direction for the practitioner in managing this common condition. Although the guidelines may often prove helpful, practitioners will need to maintain some degree of flexibility in dealing with patients both with acute otitis media and with otitis media with effusion, based on specific patient-related and disease-related factors.

Otitis media with effusion (OME) is almost universal in children born with a cleft palate. Early placement of a ventilation tube to alleviate hearing problems is common. A retrospective study has been carried out to assess whether the practice of tube placement only for definite clinical indications is successful in terms of subsequent hearing levels and speech and language development. This was assessed by a case note review, analysis of speech therapy data and by means of a special follow-up clinic. There was no difference in speech development between those treated with tube insertion for O

Several studies have indicated that either the surgical insertion of ventilation tubes (VTs) or long-term treatment with sulfonamide-based antibacterials is effective in the management of otitis media with effusion (OME; otherwise known as serous otitis media, secretory otitis media, and glue ear) when compared with a no-treatment control or placebo. This controlled trial is the first to compare directly the effectiveness of these two treatments for long-standing OME. Outcome variables are treatment success rates, hearing thresholds, recurrent acute otitis media episodes, and side effects of m

The objective of the present study was to clarify the features of otological complications for hyperbaric oxygen therapy (HBOT) and the risk factors for these complications. We enrolled 1115 patients (776 males and 339 females; age 5-89 years) who underwent HBOT. All otological symptoms experienced during HBOT sessions were evaluated, and risk factors were analysed using multivariate logistic regression analysis. Otoscopic findings and interventions for otological complications were assessed in 58 symptomatic patients who visited the Otolaryngology Department. Otological symptoms were experien

Tympanostomy tubes are commonly utilized for the treatment of otitis media with effusion (OME), yet the factors influencing treatment efficacy remain elusive. This study investigates the therapeutic outcomes of tympanostomy tube insertion (TTI) in the treatment of OME and identifies the risk factors associated with treatment effectiveness. This retrospective cohort study analyzed patients with OME who underwent successful TTI at the First Affiliated Hospital of Nanjing Medical University between January 2018 and January 2020, with a minimum follow-up period of 1 year. Patient medical records a

Alternative techniques for cochlear implant surgery have been described, such as endomeatal- and suprameatal-alone approaches, without traditional posterior tympanotomy. A combined posterior tympanotomy/endomeatal approach is proposed as a way to enhance surgical safety and effectiveness. 64 patients, 34 men and 30 women, mean age 28 (range 1-81 years), 26 pre-verbal and 38 post-verbal, were submitted to cochlear implantation, 32 by means of the described combined posterior tympanotomy/endomeatal approach and 32 with traditional posterior tympanotomy-alone approach. We investigated tubal function by equalization manometry in 68 patients with unilateral traumatic perforation of the tympanic membrane, and in 116 patients with chronic mesotympanic suppuration (85 dry and 31 discharging ears). Our criterion of normal tubal function is the ability to equalize a difference in pressure of +/- 30 hPa by means of at most 7 swallowings. This result was achieved in only 25% of the dry perforations and in none of the patients with discharging ears. Normal tubal function may only be expected in 25% of patients with chronic otitis media.

OM and OME are common disorders of childhood. Middle ear disease is related to anatomic abnormalities, prior episodes of AOM, chronic rhinitis, allergy, age, sex, race, season, perinatal factors, viral infections, and irritant exposure. ET dysfunction plays an important role in OME, as does the presence of bacteria or bacterial products in the middle ear. Viral infection, nasal allergy, previous episodes of OM and primary mucosal disease all may contribute to this chronic disorder. Diagnostic evaluation involves an appropriate personal and environmental history, a physical examination, pneumat

Myringosclerosis (MS) is hyalinization and calcification of the collagen layer in certain areas of the tympanic membrane (TM) and appears as white chalky patches in otomicroscopy. One of the most common sequela from the use of grommets is the development of MS and its rate in the tubed ear ranges between 44% and 54% during long term follow-up. Among tympanometric configurations, type As tympanograms may indicate ossicular fixation, tympanosclerosis or otitis media with effusion. In case of multiple pathologies (e.g., MS and otitis media with effusion), it is not possible to evaluate the effect

Producing good local anaesthesia of the external auditory meatus and tympanic membrane has always been a difficult problem facing otologists. Previously used methods are mentioned and the use of a new eutectic mixture of local anaesthetics (EMLA) for electrocochleography, myringotomy and grommet insertion in adults and older children is described. A double-blind controlled trial involving 15 patients (30 ears) undergoing electrocochleography showed EMLA to be a very effective, safe and convenient preparation for outpatient otological use.

INTRODUCTION: Otitis media (OM) is a common condition in children that can significantly deteriorate quality of life and lead to learning difficulties due to hearing loss. Despite having a lower disease burden in Japan compared to some other countries, the prevalence of OM remains troublesomely high. Investigating the dynamics of disease-related patterns through digital epidemiology can uncover social and clinical factors underlying OM incidence.
METHODS: Paediatric OM was studied using digital records of insurance charges for tympanometry (TM), medical supervision charges for chronic OM with
