Magnet Recognition, Leadership Responsiveness, and Nurse Moral Distress: A Multilevel Mediation Analysis.

PURPOSE: Moral distress is a pervasive challenge in nursing and has been associated with burnout, job dissatisfaction, and turnover. Although Magnet Recognition has been associated with more favorable nursing practice environments and nurse outcomes, little is known about the association between Magnet status and nurses' experiences of moral distress or the role of leadership responsiveness in this association. This study examined whether management responsiveness to patient care concerns accounted for part of the association between hospital Magnet status and nurse moral distress. DESIGN: Th

Spatial transcriptomics reveals organizational properties of mouse spinal cord and alterations in neuropathic pain.

The spinal cord integrates diverse somatosensory inputs and executes motor outputs through anatomically and functionally distinct circuits. Here, we employed spatially resolved single-cell transcriptomic profiling of the adult mouse spinal cord to gain insights into the organizational logic of the spinal cord. Our results reveal distinct spatial and laminar distributions of neuronal subtypes, including axial level and sex-specific differences. Many neuronal subtypes exhibit close spatial proximity, implicating regionally specific patterns of connectivity and circuit functions. Additionally, ne

The Cost of Homecare Personal Support Worker Turnover.

As demand for homecare services continues to rise, homecare worker retention remains a critical issue for the sector which impacts access to care and operational efficiency. To design evidence-based retention policies within a fiscally constrained context, it is important to understand the sector-specific costs of turnover. Using data from a large homecare provider organization in Ontario, Canada, we have developed the first known homecare Personal Support Workers (PSWs) cost of turnover estimate. We estimate a turnover cost of $22,000 (CAD 2024) per homecare PSW, based on an employer-level ho

[Complex clinical scenarios and treatment plans: current limitations, regional disparities in access, and reform proposals].

Cardio-kidney-metabolic syndrome and heart failure remain complex clinical conditions with significant healthcare implications. While therapeutic plans were intended to ensure appropriate prescribing, they often represent bureaucratic barriers. Facilitating or removing such plans could enhance treatment timeliness, therapeutic continuity, and equitable access. This work also includes other widely used cardiovascular drugs such as direct oral anticoagulants, which remain under therapeutic plan requirements despite robust clinical experience and safety data. In light of the recent AIFA resolutio

Hospitable harm: When patient experience efforts compromise clinical integrity.

Efforts to improve patients' hospital experience can reduce uncertainty and indignity, but they can also create "hospitable harm" when pressure for smoother encounters encourages low-value care, preferential access, or compromises in safety, stewardship, or equity. This Perspective introduces the Experience-Integrity Matrix as a conceptual model for examining such initiatives. One axis asks whether an initiative makes care more reliable and dignified for patients; the other asks whether it aligns with clinical integrity. The four quadrants indicate what leaders should scale, repair, decline, o

Courageous Leadership, Psychological Safety, and Innovative Work Behavior Among Staff Nurses.

OBJECTIVE: To examine the associations among courageous leadership, innovative work behavior, and psychological safety as self-reported by staff nurses. BACKGROUND: Psychological safety supports communication and performance in health care, but evidence on the combined role of leadership and innovation among staff nurses remains limited. METHODS: A descriptive cross-sectional study was conducted among 257 staff nurses in Saudi hospitals using validated instruments and data analyzed using correlation and regression analysis statistical techniques. RESULTS: Nurses reported high psychological

Adaptations to family-based treatment for Medicaid-insured adolescents with anorexia nervosa.

BACKGROUND: Family-based treatment (FBT) is the leading intervention for adolescents with anorexia nervosa (AN); however, it is under researched in socioeconomically disadvantaged and racially diverse youth. METHODS: Semi-structured interviews were completed with ten FBT clinicians who practice in publicly-funded settings. Interview questions were focused on implementation challenges, overall acceptability and appropriateness of FBT, and naturally-occurring treatment adaptations. RESULTS: Content analysis revealed common themes relating to the implementation of FBT in publicly-funded, commun

A case study of the long-term sustainability of an oral health program for persons experiencing homelessness.

Stakeholders engaged in funding health programs and investing in human capital are concerned that their investments lead to beneficial long-term outcomes and do not fade from existence when funding streams dry up. However, dissemination and implementation researchers often pay little attention to what happens after programs are implemented. An essential function of the public health nurse operating at the executive management or leadership level is to evaluate overall effectiveness, quality, and sustainability of programs and to design systems-level quality initiatives and evaluation plans tha

The Relationships among Proactive Personality, Work Engagement, and Perceived Work Competence in Sports Coaches: The Moderating Role of Perceived Supervisor Support.

Grounded in the job demands-resources model, this study examines the moderating role of supervisor support and the mediating role of sports coaches' work engagement in the relationship between proactive personality and perceived work competence. A total of 261 school sports coaches in Taiwan participated in the study. The results indicated that work engagement positively mediates the relationship between sports coaches' proactive personality and perceived work competence. Separately, supervisor support weakens the link between proactive personality and work engagement but strengthens the relat

The Unseen Side of Nursing Leadership: Conceptualizing Leadership Blind Spots in Healthcare Settings.

BACKGROUND: Nursing leadership effectiveness depends not only on clinical expertise and formal authority but on the capacity to perceive how one's own conduct is experienced by staff. Leadership blind spots-unrecognized gaps between a nurse leader's self-perception and the actual impact of their behaviour on staff voice, trust, and psychological safety-represent a consequential and theoretically underdeveloped phenomenon in nursing management. Left unrecognized, these gaps may silently undermine staff well-being, patient safety, and organizational culture. AIM: To clarify and conceptually def

Liability and Standard of Care in AI-Driven Psychiatric Practice: European Viewpoint.

AI is increasingly incorporated into psychiatric triage, risk prediction, passive monitoring, clinical documentation, and patient-facing conversational systems. These applications may improve access, continuity, efficiency, and pattern recognition, but they also redistribute epistemic authority and complicate responsibility when harm occurs. European regulation is developed in relation to market access, data governance, risk management, and product safety, yet remains fragmented regarding civil liability, organizational negligence, and the psychiatric standard of care. This Viewpoint examines

The Challenges of Collecting and Using Patient Care Data From Diverse Care Systems: Lessons From COMPASS.

The ability to aggregate clinical data across multiple diverse organizations and to use it for performance measurement, quality improvement, evaluation, and research is rapidly becoming a national necessity, but there are few examples of how to do that. This article uses lessons from a national effort to implement the collaborative care management model for patients with both depression and diabetes or heart disease across 8 partner organizations, 18 medical groups, and more than 170 clinics in 8 states to identify the challenges and provide experience-based recommendations for those tasks. [Competence Network for Congenital Heart Defects: innovative research for a young patient group].

BACKGROUND: Congenital heart disease is the most frequent congenital malformation in humans and affects almost 1 % of all newborns. Thanks to advances in diagnostics and treatment, over 90 % of those affected reach adulthood today. Patient numbers are therefore growing constantly. The majority of those affected, however, remain chronically ill throughout life and require continuous specialized care. The spectrum of different clinical pictures is vast. Since this is a relatively young patient group, both research and care are lacking relevant knowledge and experience. OBJECTIVE: The Competence

Associations between workplace affiliation and phlebotomy practices regarding patient identification and test request handling practices in primary healthcare centres: a multilevel model approach.

BACKGROUND: Clinical practice guidelines aim to enhance patient safety by reducing inappropriate variations in practice. Despite considerable efforts to enhance the use of clinical practice guidelines, adherence is often suboptimal. We investigated to what extent workplace affiliation explains variation of self-reported adherence to venous blood specimen collection regarding patient identification and test request handling practices, taking into consideration other primary healthcare centre and individual phlebotomist characteristics. METHODS: Data were collected through a questionnaire surve

A systems change: leading the way to meeting health needs.

Demonstrating the efficacy of our practice requires a paradigm shift. Becoming an effective leader and clinician can facilitate opportunities for program development and clinical research. The use of strategic planning strategies, such as needs assessment and SWOT analysis, can help lead the way to such change. The following illustrates the use of strategic planning to develop The Carpal and Cubital Tunnel Syndrome Program (CCTSP) within a growing orthopedic practice.

Longitudinal model-based meta-analysis in rheumatoid arthritis: an application toward model-based drug development.

Summary-level longitudinal data on the clinical efficacy of drugs for rheumatoid arthritis (RA) are available in the literature. This information can be used to optimize the clinical development of new drugs for RA. The aim of this study was twofold: first, to quantify the time course of the ACR20 score across approved drugs and patient populations, and second, to apply this knowledge in the decision-making process for a specific compound, canakinumab. The integrated analysis included data from 37 phase II-III studies describing 13,474 patients. It showed that, with the tested doses/regimens o

Vested interests in addiction research and policy poisonous partnerships: health sector buy-in to arrangements with government and addictive consumption industries.

AIM: This paper critically appraises relationship arrangements among three broadly conceived sectors: the government sector, the health sector (including researchers) and addictive consumption industries (particularly tobacco, alcohol and gambling). METHOD: Three models for involvement are examined. In the 'tripartite partnership model' health sector agencies engage as co-equals with the government and industry sectors in order to implement public health initiatives such as host responsibility and public education. In the 'non- association model' the health sector engages with government agen
