This study describes the abnormal pattern of gastrointestinal progression of a liquid meal in patients with the digestive form of chronic Chagas' disease. This condition is known as a natural model of intramural denervation of the gut. Neuronal intestinal dysplasia (NID) is a disturbance of the innervation of the gut. Its symptoms resemble the ones seen in Hirschsprung's Disease. Contrary to aganglionosis, however, there is hyperplasia of the ganglia. Our study revealed isolated NID (4/49 cases) to be eight times rarer than aganglionosis (33/49 patients). Combination of both diseases has been reported to occur in 20% of NID cases; we did however find it in 75% (12/16) of our patients. Isolated hypoganglionosis (IHG) is histologically characterized by small numbers of myenteric ganglion cells and small myenteric ganglia; however, no numerical diagnostic criteria for IHG have been established. A patient who died from oat-cell carcinoma of the lung had had abdominal pain and obstipation. Autopsy revealed autonomic neuropathy limited to the gastrointestinal tract, which was considered to be related to carcinoma as a remote effect. This interpretation was further supported by the presence of Wallerian degeneration of the dorsal columns. Autonomic neuropathy involving the gastrointestinal tract in association with malignant disease has not been previously described.
