The evaluation of morphology of renal pelvicalyceal system's and infundibulopelvic anatomy of kidney's lower pole in post-mortem series.

BACKGROUND: Urinary system stones are frequently encountered in the community. Together with technological developments, introduction of new treatment procedures such as extracorporeal shock wave lithotripsy, percutaneous nephrolithotomy and retrograde intrarenal surgery has further reduced morbidity, mortality and hospitalisation time of patients. In order to maximise success and to reduce complications of these procedures, it is necessary to evaluate anatomy and morphological differences of kidney collector system before the procedure. This study was conducted for the purpose of determining

Ureterocalicostomy for Reconstruction of Complicated Ureteropelvic Junction Obstruction in Adults: Long-Term Outcome and Factors Predicting Failure in a Contemporary Cohort.

PURPOSE: Ureterocalicostomy is a well established treatment option in patients who have recurrent ureteropelvic junction obstruction with postoperative fibrosis and a relatively inaccessible renal pelvis. We evaluated the long-term outcome of ureterocalicostomy and factors predicting its failure. MATERIALS AND METHODS: We retrospectively analyzed data on 72 patients who underwent open or laparoscopic ureterocalicostomy from 2000 to 2014. Variables that may affect the outcomes of ureterocalicostomy were assessed with regard to primary pathology findings, patient age, serum creatinine, preopera

Multiple sclerosis and nephrolithiasis: a matched-case comparative study.

OBJECTIVE: To compare stone composition and serum/urine biochemistries in stone formers with multiple sclerosis (MS) against stone formers without MS and to examine the association between mobility, methods of bladder emptying, and stone formation. PATIENTS AND METHODS: In this retrospective case-control study, we identified patients diagnosed with MS and kidney stone disease who were seen at our institution between 2001 and 2016. For the first part of the study, up to two controls (stone formers without a history of MS) were identified for each case and matched on age, body mass index, and s

Human acid sphingomyelinase.

BACKGROUND: Prone position during general anesthesia for special surgical operations may be related with increased airway pressure, decreased pulmonary and thoracic compliance that may be explained by restriction of chest expansion and compression of abdomen. The optimum ventilation mode for anesthetized patients on prone position was not described and studies comparing volume-controlled ventilation (VCV) and pressure-controlled ventilation (PCV) during prone position are limited. We hypothesized that PCV instead of VCV during prone position could achieve lower airway pressures and reduce the

[Results of the first multicenter study assessing the efficiency of a tissue sensor during thulium fiber laser lithotripsy].

INTRODUCTION: Currently, the main surgical approach for urinary stone removal is minimally invasive endoscopic procedures. Various laser systems compatible with most modern instruments are used for stone disintegration. Due to high efficacy and minimal fragment retropulsion during fragmentation, the domestic thulium fiber laser has become a serious alternative to the conventional holmium laser. At the same time, the safety profile of thulium fiber lithotripsy remains debatable because of the potential risk of irrigation fluid heating and damage to surrounding tissues during fragmentation. The efficacy of extracorporeal shock wave lithotripsy for isolated lower pole calculi compared with isolated middle and upper caliceal calculi.

PURPOSE: We assess the efficacy of extracorporeal shock wave lithotripsy monotherapy for isolated lower pole nephrolithiasis, and compare it to that for isolated middle and upper caliceal calculi. MATERIALS AND METHODS: We treated 714 renal units in 687 patients with isolated caliceal stones using a Lithostar lithotriptor (Siemens Medical Systems, Erlangen, Germany). The stones were localized in the lower, mid and upper calices in 455, 104 and 128 patients, respectively. Stone load was recorded in cm.2. Patients were stratified into 3 groups based on stone burden. The energy and shock waves,

Ultrasound-Guided Renal Access and Tract Dilation.

Introduction: Ultrasound guidance is a unique alternative to fluoroscopy for percutaneous renal access. Besides being free of ionizing radiation exposure to the patient and intraoperative personnel, it offers several advantages including easier identification of the posterior renal calix and surrounding visceral structures. In this video, we demonstrate how ultrasound can be used to guide percutaneous nephrolithotomy (PCNL) in a step-by-step manner. Materials and Methods: From March to June 2016, 16 consecutive patients of age 18 and more with kidney or proximal ureteral stones underwent compl

[Extracorporeal shock wave lithotripsy: prophylaxis, complications and therapy].

ESWL has proved to be a safe and effective method in the treatment of urinary tract calculosis. The method is hardly invasive when compared to all other methods known so far, except for the treatment of calculosis with a selective pharmacotherapy. Moreover, the method is contactless, extracorporeal, mostly necessitating no anaesthesia and spasmoanalgesic therapy, except in the very small number of patients. The importance of the problem of urinary tract calculosis is enormous. Calculosis affects children, and particularly adults, predominantly psychically and physically active individuals in t

AhR activation attenuates calcium oxalate nephrocalcinosis by diminishing M1 macrophage polarization and promoting M2 macrophage polarization.

Calcium oxalate (CaOx) crystal can trigger kidney injury, which contributes to the pathogenesis of nephrocalcinosis. The phenotypes of infiltrating macrophage may impact CaOx-mediated kidney inflammatory injury as well as crystal deposition. How aryl hydrocarbon receptor (AhR) regulates inflammation and macrophage polarization is well understood; however, how it modulates CaOx nephrocalcinosis remains unclear. Methods: Mice were intraperitoneally injected with glyoxylate to establish CaOx nephrocalcinosis model with or without the treatment of AhR activator 6-formylindolo(3,2-b)carbazole (FICZ

Management of urinary stones by experts in stone disease (ESD 2025).

The formation of kidney stones is a complex biologic process involving interactions among genetic, anatomic, dietary, and environmental factors. Traditional lithogenic models were based on urine supersaturation in relation to the activity of crystallization promoters and inhibitors. However, modern research has added new principles such as the "renal epithelial cell response" and the role of inflammation and oxidative stress leading to the development of a "multi-hit hypothesis". A strong correlation between urinary stones and kidney damage has been well demonstrated by both cohort and case-co

Atypical Causes of Urinary Tract Obstruction.

Acute kidney injury due to urinary tract obstruction invariably suggests lower urinary tract obstruction or bilateral ureteric obstruction since obstruction of a single kidney while the contralateral kidney is normal and not obstructed would not cause a perceptible rise in creatinine. Assuming a total body volume of 42 L, 70 kg male that generates approximately 1400 mg of creatinine daily (20 mg/kg/day) who has complete urinary tract obstruction would experience a 3.33 mg/dL per day increase in serum creatinine. Thus, for an individual who had prior normal renal function and who presents with
