Protein energy wasting in chronic kidney disease: An update with focus on nutritional interventions to improve outcomes.

Protein-energy wasting (PEW) is common in patients with chronic kidney disease (CKD). PEW is one of the strongest predictors of mortality in … he literature indicates that PEW can be mitigated or corrected with an appropriate diet and enteral nutritional support that targets dietary protein intake. Dietary requirements and enteral nutritional support must also be considered in patients with CKD and diabetes mellitus and in children with CKD, in addition to dialysis patients. Features of ideal dietary supple …

Energy and protein intake in 330 geriatric orthopaedic patients: Are the current nutrition guidelines applicable?

BACKGROUND AND AIMS: Elderly orthopaedic patients are potentially at a high nutritional risk. The reasons for this are numerous as i.e. reduced appetite, sensation of satiety after eating small amounts of food, multi-medication or immobility. This is in contrast to the increased energy and protein recommendations for geriatric orthopaedic patients. METHODS: Oral intake during hospitalization of more than 1000 geriatric orthopaedic patients aged over 80, with or without fracture, was recorded, calculated and then compared to energy and protein requirements by clinical dietitians according to i

Early outcomes of gastrostomy feeding in paediatric allogenic bone marrow transplantation: A retrospective cohort study.

BACKGROUND: Nutrition support is an essential component of care for a child undergoing bone marrow transplantation (BMT). Enteral nutrition (EN) is becoming increasingly recognised as having advantages over parenteral nutrition (PN) and recommended as first-line nutrition support. EN has traditionally been provided via nasogastric tube (NGT). Gastrostomies avoid certain complications associated with NGTs and could provide a preferential alternative. AIMS: To compare nutritional and post-transplantation outcomes during admission, the primary outcome being PN use, between children who had a gas

[Perioperative nutrition for gastrointestinal surgery].

In elective gastrointestinal (GI) surgery, the patients' nutritional status should be assessed and if protein-energy malnutrition exists, preoperative nutritional support should be scheduled 7 to 14 days before surgery. In malnourished patients in particular, preoperative nutrition with total parenteral nutrition (TPN) reduces postoperative complication rates of infection. Preoperative enteral nutrition (EN) is considered to be as effective as TPN in improving postoperative surgical outcome. Many prospective randomized trials or meta-analyses comparing postoperative EN and TPN have revealed th…

Nutrition in the hospitalized patient.

Almost 50% of patients are malnourished on admission; many others develop malnutrition during admission. Malnutrition contributes to hospital morbidity, mortality, costs, and readmissions. The Joint Commission requires malnutrition risk screening on admission. If screening identifies malnutrition risk, a nutrition assessment is required to create a nutrition care plan. The plan should be initiated early in the hospital course, as even patients with normal nutrition become malnourished quickly when acutely ill. While the Harris-Benedict equation is the most commonly used method to estimate calo

[Perioperative nutritional therapy and its relevance for postoperative outcome].

The clinical effect of perioperative nutrition was analyzed from 28 controlled clinical trials. Preoperative nutritional support is indicated only in patients with high-grade malnutrition and can reduce the postoperative morbidity and mortality in these patients. If the nutritional status is normal preoperatively, artificial nutrition may influence the postoperative outcome negatively. A clinical benefit from postoperative nutritional support has been proven only in patients not able to eat an adequate oral diet within 1 week. In 20% of these case a cardiopulmonary complication was observed du…

[Progression in 25 years of artificial nutrition in surgery].

The Authors illustrate the fundamental steps of development of artificial nutrition (AN) during the last 25 years. In this period AN has been extensively experimented with and applied clinically, as total parenteral nutrition as well as enteral nutrition. The indications for artificial nutrition have been characterised and numerous experimental and clinical studies have been carried out to document the effects of nutritional support in surgical patients. The results of such studies have entranced our knowledge of nutritional pathophysiology and have brought about better methods for AN in surge…

The effectiveness of nutritional interventions in malnutrition and cachexia.

Cancer is a common diagnosis and leading cause of death worldwide. Amounts of weight loss vary but it is associated with considerable morbidity, poorer quality of life and reduced survival. Nutritional intervention has the potential to maximise response to treatment and improve functioning and quality of life. The aim of this paper was to review the evidence for oral nutritional interventions in the management of weight loss in patients with cancer. Comparison of studies of nutritional support interventions in people with cancer is complicated by variations in understanding of what constitutes…

The role of nutrition in increased morbidity and mortality.

Up to 65% of the hospitalized elderly are protein-energy undernourished at admission or develop serious nutritional deficits while hospitalized. These undernourished patients are at an increased risk of developing serious in-hospital complications, the likelihood of developing a complication increasing in relation to the severity of the nutritional deficits. Several studies suggest that aggressive nutritional support targeted to these undernourished patients may be beneficial both in terms of rectifying their nutritional deficits and improving their clinical outcomes; however, these findings h…

Nutritional therapy in liver disease.

Nutritional inadequacies are frequent in patients with liver damage, specially in those with advanced chronic liver disease, and this fact, as in other pathological circumstances, may influence their clinical evolution and outcome. Therefore, nutritional therapy may play an important role in the management of these patients. The present paper will deal with some aspects of the nutritional management in patients with liver disease. Special emphasis will be paid to the role of artificial nutritional support in the treatment of acute alcoholic hepatitis, the current status of artificial nutrition…

Severe acute pancreatitis: nutritional management in the ICU.

Patients with acute pancreatitis have elevated nutritional needs due to increased energy expenditure and catabolism. It is a clinical challenge to provide adequate nutrition to these patients while maintaining gut function, preventing pancreatic stimulation, and minimizing the risk of septic and metabolic complications associated with nutritional support. We present the case of a patient who had severe acute pancreatitis and was initially given total parenteral nutrition. After a period of initial improvement, he developed hyperglycemia, bacteremia, and sepsis. Parenteral nutrition was discont…

Individualizing post-ICU nutrition: from guideline intent to bedside practice.

PURPOSE OF REVIEW: Survivorship of critical illness has increased, shifting attention toward recovery and long-term outcomes. Post-ICU nutrition is emerging as a clinically relevant, yet underperforming contributor to functional recovery and quality of life. Persistent nutritional inadequacy reflects insufficiently defined physiological requirements and limited implementable evidence-based guidance during ICU recovery. RECENT FINDINGS: Energy and protein delivery sharply decline after ICU discharge, despite persistent metabolic demands. Underfeeding can result from the absence of recovery-spe
