Does Parents' Own History of Child Abuse Moderate the Effectiveness of the Promoting First Relationships® Intervention in Child Welfare?

To better understand how and for whom parenting intervention may improve family outcomes in child welfare services, we examined whether parents' own history of child abuse moderated the indirect effects of the Promoting First Relationships® (PFR) intervention on toddlers' secure base behavior via parental sensitivity. Parents ( N = 247) and their toddlers (10-24 months) involved with child protective services were randomized to PFR or a control intervention. Results showed that the PFR group demonstrated greater parental sensitivity at postintervention than the control group, which in turn led

Does accessibility of positive and negative schema vary by child physical abuse risk?

OBJECTIVE: To examine differences in accessibility of positive and negative schema in parents with high and low risk for child physical abuse (CPA). METHODS: This study combined picture priming and lexical decision making methods to assess the accessibility of positive and negative words following presentation of child and adult faces. The child and adult faces depicted positive, ambiguous, and negative affective valences. The sample included 67 (51 low and 16 high CPA risk) general population parents. RESULTS: CPA risk status was associated with accessibility of positive/negative words only

Sudden oronasal bleeding in a young child.

UNLABELLED: Sudden severe upper-airway obstruction occurring in a hospital setting can sometimes precipitate an episode of acute haemorrhagic pulmonary oedema. A review of 197 published case reports shows that the presenting feature is almost always the sudden appearance of blood stained fluid coming up through the larynx or out through the mouth and nose of an adult or child in obvious respiratory distress. Such overt features are seen in 10-15% of cases of sudden severe, but sub-lethal, upper-airway obstruction. Signs normally appear within minutes once the obstruction is relieved but are oc

Depression, anxiety, and quality of life in siblings of children with intractable epilepsy.

Thirty-seven siblings (aged 6-18) of children with intractable epilepsy were surveyed regarding their anxiety, depression, and quality of life, by both self-report (Revised Children's Manifest Anxiety Scale, RCMAS; Children's Depression Inventory, CDI; and Peds QL) and parental report (Child Behavior Checklist, CBCL). Completed forms were returned by 37 of 58 (64%) eligible families. No sibling had a score in the clinical range on the CDI, and only 2 of 37 (6%) scored in the clinical range on the RCMAS, a proportion similar to the normative population. In contrast, 25% were rated by their pare

Youth characteristics associated with behavioral and mental health problems during the transition to residential treatment centers: the Odyssey Project population.

This study aimed to determine what youth characteristics were associated with emotional and behavioral problems exhibited within the first three months of placement in residential treatment centers (RTCs) in a sample of youth from 20 agencies in 13 states. Two primary research questions were addressed: 1) What characteristics were associated with behavior during the transition to care? 2) Were the characteristics associated with behavior during the transition the same for boys and girls? Data were drawn from the Time 1 phase of the longitudinal national Odyssey Project dataset developed by the

Haematocrit levels and anaemia in Australian children aged 1-4 years.

The aim of this study was to describe the prevalence of anaemia, mean haematocrit levels, and the risk factors influencing haematocrit in participants of the 1995 National Survey of Lead in Children. A nationally-representative cross-sectional survey of children aged 1-4 years inclusive was done. Mean haematocrit and the proportion with anaemia using both the US and WHO haematocrit-based criteria were calculated. Multivariate regression was used to identify factors associated with haematocrit. Mean haematocrit level was 38.8% (95% CI: 38.6 - 39.1%) and varied with age of child, state/territory

Severe abnormalities of the pons in two infants with goldenhar syndrome.

We describe 2 cases of Goldenhar syndrome with severe abnormalities of the pons. The first case is a 10-month-old Caucasian female infant. At birth the girl showed polydactyly, labiopalatoschisis, right ear agenesis, left eye coloboma and vertebral anomalies. She also had marked hypotonia, severely reduced movements and respiratory and feeding abnormalities. She required gastrostomy at 5 months and tracheostomy at 7 months. Brain MRI scans showed moderate cerebellar hypoplasia and severe abnormalities of the pons with a congenital cleft. The child died at age 12 months. Case 2 is a Caucasian b

Evaluations of the effects of Sweden's spanking ban on physical child abuse rates: a literature review.

Sweden's 1979 law banning corporal punishment by parents was welcomed by many as a needed policy to help reduce physical abuse of children. This study reviews the published empirical evidence relevant to that goal. Only seven journal articles with pertinent data were located. One study reported that the rate of physical child abuse was 49% higher in Sweden than in the USA, comparing its 1980 Swedish national survey with the average rates from two national surveys in the United States in 1975 and 1985. In contrast, a 1981 retrospective survey of university students suggested that the Swedish ab

[Irregular blood group antibodies during pregnancy: screening is mandatory].

During pregnancy irregular blood group antibodies, originating either from earlier pregnancies or from blood transfusions, may severely jeopardize both mother and child. Three patients are described with pregnancy-associated blood group incompatibility. In one case of Kell antagonism a previous child had reportedly died of cot death, but in retrospect it had most probably suffered from erythroblastosis fetalis as a result of anti Kell antibodies. In the second case, a twin pregnancy, the diagnosis of neonatal haemolytic anaemia on the basis of blood group incompatibility with a very rare antib

The what, why and how of hyperkinesis: implications for nursing.

Hyperkinesis refers to a combination of traits that typically include: overactivity; restlessness; short attention span; distractability; low frustration tolerance; impulsiveness. Its aetiology and prevalence have not been established but associating problems underscore the importance of early identification and treatment. Theories about aetiology relate to minimal brain damage, heredity, temperament variations, maturational lag, dysfunction of the reticular activating system, food sensitivity, and learned response to unorganized environment. Hyperkinesis may be distinguished only by its behav

Surgical Correction of Popliteal Pterygium with Serial Splinting: A Case Report and Review of Literature.

Popliteal pterygium syndrome is an extremely rare congenital disorder, with only a few cases reported in the literature. It consists of facial, genitourinary, and musculoskeletal anomalies. A very striking characteristic of this syndrome is the popliteal pterygium contracture, which is considered the most challenging malformation to be corrected. Hence, we report a case of popliteal pterygium contracture in a 30-month-old female child with no family history of popliteal pterygium syndrome. The child was treated successfully with satisfactory results, by combining surgical intervention with pos

Scurvy in an autistic child: MRI findings.

Scurvy results from a deficiency of vitamin C and is rarely seen in the United States. We describe the MRI findings of a case of scurvy in an autistic child with food-avoidant behavior. Advanced imaging is rarely performed in clinically well-understood disease entities such as scurvy. Typical radiographic findings are well described leading to definitive diagnosis, although the findings can be missed or misinterpreted given the rarity of scurvy in daily practice. To our knowledge, MRI features of scurvy in children in the US have been described in only one case report. This case of scurvy in a

[Transjugular intrahepatic portasystemic stent shunt (TIPSS). Results and complications].

Use of a transjugular intrahepatic portosystemic stent shunt (TIPSS) is a highly efficient method of controlling bleeding from esophageal or gastric varices in patients with portal hypertension. Early mortality is between 2% and 13%. Stenosis and occlusion of the stent are reported in 6-56% and 7-9%, respectively, within the first year. Stenoses occur more often in patients with good liver function. In 17-27% de novo encephalopathy is observed. The rate correlates with the extent of liver cirrhosis, which is the main factor that determines prognosis of the patients in the long run. All patient

Child abuse by adolescent caregivers.

The purpose of this analysis was to estimate the frequency and severity of child abuse committed by adolescents who were in nonparental caregiving roles. The sample was composed of cases in which either physical or sexual abuse was substantiated through child welfare investigation. The main comparisons were between adolescent and adult caregivers. While caregiver age did not appear to have a consistent effect on the occurrence of physical abuse, notable differences between adolescents and adults were found in the area of sexual abuse. Not only were adolescents observed to commit substantially

Rectovulvar fistula in a child secondary to an unusual foreign body.

Foreign bodies in the female genital tract are well recognized as a cause of pain, discharge, secretions, and infection. In the small child, the presence of a vaginal discharge is usually associated with either a common object which has been inserted or sexual abuse complicated by a sexually transmitted disease. A 3-year-old child presented with a recurrent labial secretion and drainage due to an unusual foreign body. It was only during a second operation that the possibility of a foreign body was entertained, and diagnostic testing was begun. A third operation permitted removal of the foreign

[Kanner's syndrome and childhood schizophrenia].

The examination of 32 children with Kanner's syndrome of early infantile autism permits to assume that this syndrome in some of the cases is expressed only by inborn anomalies which correspond to constitutional psychopathy in adults. In most of the cases this syndrome forms the initial expression of child schizophrenia. In separate cases disorders very similar to Kanner's syndrome may be seen after the first olliterated attack during early childhood (up to 3 years). A comparative study of the same indices of development of 268 children with an early onset of schizophrenic process in spite of s

Trends in otitis media-related health care use in the United States, 2001-2011.

IMPORTANCE: Otitis media (OM) is a leading cause of pediatric health care visits and the most frequent reason children consume antibiotics or undergo surgery. During recent years, several interventions have been introduced aiming to decrease OM burden. OBJECTIVE: To study the trend in OM-related health care use in the United States during the pneumococcal conjugate vaccine (PCV) era (2001-2011). DESIGN, SETTING, AND PARTICIPANTS: An analysis of an insurance claims database of a large, nationwide managed health care plan was conducted. Enrolled children aged 6 years or younger with OM visits

Clinical communication and caregivers' satisfaction with child healthcare in Nepal; results from Nepal Health Facility Survey 2015.

BACKGROUND: Patient satisfaction is an important measure of quality of care and a determinant of health service utilisation and the choice of health facility. Measuring patients' experiences is important for understanding and improving the quality of care at health facilities. The aim of this study was to assess levels and identify associated factors of caregivers' satisfaction and provider-caregiver communication within child healthcare in Nepal. METHODS: Secondary analysis of Sick Child Exit Interviews (n = 2092) sourced from 2015 Nepal Health Facility Survey data. Satisfaction was measured

[Epidemiologic aspects of infant-juvenile malnutrition in Cotonou, Benin].

The prevalence of malnutrition in Benin has been estimated at 29% and infant-juvenile mortality rate at 156% per hundred. Because there is no specific data on the prevalence of malnutrition in Cotonou, a transverse descriptive study was undertaken to determine the rate and features of malnutrition. Anthropometric measurements were performed in African children who were between 0 and 36 months of age and had been living in Cotonou for at least 6 months. A questionnaire was administered to the mothers of all children including study of risk factors and assessment of food intake for the last 24 h

Relevance of the Pediatric Powered Wheelchair Screening Test for children with cerebral palsy.

In a previous study we developed a cognitive assessment battery called the Pediatric Powered Wheelchair Screening Test (PPWST) to help clinicians determine a young child's readiness to drive a powered wheelchair. The current multicenter study sought to determine: (1) whether the PPWST is appropriate for use in a population of children with cerebral palsy (CP) who use joysticks to drive their wheelchair; (2) whether two additional factors (symbolic representation and coping) would increase the predictive power of the PPWST for this group and for children with orthopedic or neuromuscular disabil
