Surgical treatment of giant chordoma in the thoracic spine combining thoracoscopic and posterior spinal surgery: A case report.

RATIONALE: Giant thoracic chordoma is a highly unusual disease with no standard curative managements yet. The objective of this study is to report a very rare case of giant thoracic chordoma successfully operated by combination of thoracoscopic surgery together with posterior spinal surgery. The management of these unique cases has yet to be well-documented. PATIENT CONCERNS: A 64-year-old man presented with a 4-month history of continuous and progressive back pain. The patient, who had been diagnosed of sacral chordoma for 2 years, received surgical treatment of posterior sacral tumor resect

Sciatic nerve release following fracture or reconstructive surgery of the acetabulum. Surgical technique.

BACKGROUND: Sciatic neuropathy associated with acetabular fractures can result in disabling long-term symptoms. The purpose of this retrospective study was to evaluate the effect of sciatic nerve release on sciatic neuropathy associated with acetabular fractures and reconstructive acetabular surgery. METHODS: Between 2000 and 2004, ten patients with sciatic neuropathy associated with an acetabular fracture were treated with release of the sciatic nerve from scar tissue and heterotopic bone. Additional surgical procedures included open reduction and internal fixation of the acetabulum (five pa

Cranial Cruciate Ligament Disease for the Small Animal Practitioner.

Cranial cruciate ligament (CrCL) disease is a common pelvic limb condition in canines. It is most commonly a degenerative process but can also result from trauma. Diagnosis is primarily based on orthopedic examination using the cranial drawer and tibial compression (thrust) tests, although other secondary changes to the stifle and radiographic findings can be supportive. Surgical treatment is generally recommended, with good functional outcomes reported following many different procedures. Contralateral CrCL rupture, medial meniscal injuries, and secondary osteoarthritis are common concurrent

Postoperative Acute Kidney Injury in Total Joint Arthroplasty: A Review of the Literature.

Total hip arthroplasty and total knee arthroplasty are among the most successful orthopedic procedures, with increasing numbers performed annually in the United States. However, adverse perioperative complications like acute kidney injury (AKI) can adversely affect patient outcomes and increase health care costs. The incidence of AKI post-total joint arthroplasty varies widely, with large-scale studies reporting less than 2% and smaller studies indicating rates as high as 21.9%. Holding angiotensin converting enzyme inhibitors, aldosterone receptor blockers, NSAIDs, diuretics, and avoiding nep…

Recent developments in osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is an uncommon genetic bone disease associated with brittle bones and fractures in children and adults. Although OI is most commonly associated with mutations of the genes for type I collagen, many other genes (some associated with type I collagen processing) have now been identified. The genetics of OI and advances in our understanding of the biomechanical properties of OI bone are reviewed in this article. Treatment includes physiotherapy, fall prevention, and sometimes orthopedic procedures. In this brief review, we will also discuss current understanding of pha…

Role of ultrasound in the diagnosis and management of developmental dysplasia of the hip: an international perspective.

Early diagnosis of developmental dysplasia of the hip is very important for proper treatment. Different ultrasound techniques have been used for early diagnosis of developmental dysplasia of the hip, but two of them are widely used in orthopedic practice: Graf's technique in Europe and Harcke's method in the United States. Our experience has led us to use an ultrasound technique that combines the two methods. Use of ultrasound has reduced the number of late-presenting cases, shortened treatment time, and decreased the number of surgical procedures of the hip joint in Poland.

Complications associated with hip resurfacing arthroplasty.

Hip resurfacing arthroplasty is an old orthopedic concept that has undergone a resurgence of interest in the past decade. Because of the rapid increase in the number of procedures being performed, previously recognized complications have begun to recur. This article focuses on complications that are related to the hip resurfacing procedure such as femoral neck fractures, avascular necrosis, raised metal ion levels, and sound initial and durable long-term fixation of an all-metal monoblock cobalt/chrome acetabular component. Dislocation rates after resurfacing and other complications are briefl

[Fracture luxation of the cervical spine. Different treatments].

There exist situations, such as cervical spinal cord injury which are treated by orthopedic specialists and/or neurosurgeons, with dissimilar methods. In is necessary to unify criteria or better still not commit mistakes which may worsen neurological déficit in these injured patients. The medical history and treatment of 16 patients with cervical spinal cord injury are discussed. The therapeutic procedures included: decompressive laminectomy, anterior fibula bone graft, Clowards' procedure and (minerva) exterior neck fixation. The evolution and complications are analyzed and in some cases conn

Management of Multidrug-Resistant Acinetobacter Infection Following Open Reduction and Internal Fixation of a Right Acetabular Fracture: A Case Report.

Postoperative infections with multidrug-resistant (MDR) Acinetobacter baumannii present significant challenges in orthopedic surgery. This case report highlights the management and successful treatment of an MDR Acinetobacter infection following open reduction and internal fixation (ORIF) of a right acetabulum fracture. A 51-year-old male presented with a comminuted right acetabular fracture following a motor vehicle accident. Initial ORIF was complicated by intraoperative blood loss, necessitating early termination of the procedure. The patient subsequently developed a deep surgical site infe

Management of Bleeding Diathesis in Elective and Orthopaedic Trauma: A Review.

There is a general need among orthopaedic surgeons for practical advice on managing patients with bleeding disorders. Appropriate diagnosis and management of these disorders is paramount once discovered before, during, or after the patient's surgical course. Bleeding disorders disrupt the body's ability to control bleeding, commonly through platelet function and blood clotting. Normally, the vessel contracts and retracts once disruption of blood vessels occurs, limiting blood loss. Blood platelets adhere to exposed collagen, aggregate at the site, and obstruct blood loss. Because platelet aggr

Outcomes and Management of Peripheral Intravenous Infiltration Injuries.

Background: Although intravenous (IV) infiltration is relatively common, data regarding complications and outcomes of this problem remain limited. In addition, there is wide variation in institutional protocols for the management of IV infiltrations. Through retrospective review, we aim to delineate complications and outcomes, and propose an algorithm for the management of these injuries. Methods: We performed a retrospective review of all patients who had an IV infiltration at a tertiary care center's inpatient and outpatient facilities between January 1, 2016, and December 31, 2018. Results:

Satisfactory surgical option for cartilage graft absorption in microtia reconstruction.

BACKGROUND: We routinely perform auricular elevation at least 6 months after implantation of framework in microtia reconstruction using costal cartilage. However, in a few cases, cartilage graft absorption has occurred, which has led to contour irregularity with unfavorable long-term results. In the present study, we recount the details of using additional rib cartilage augmentation to achieve an accentuated contour in cartilage graft absorption cases. MATERIAL AND METHODS: The cartilage graft absorption was defined as contour irregularity or cartilage graft deformation as evaluated by the su

Effect of tibial drill angles on bone tunnel aperture during anterior cruciate ligament reconstruction.

BACKGROUND: Anatomic reconstruction of the anterior cruciate ligament has received greater attention as patient outcome assessment has become increasingly sophisticated. A goal during anatomic reconstruction should be the creation of a tibial tunnel aperture that is similar in size and orientation to the native anterior cruciate ligament insertion. Aperture morphology depends primarily on three factors: (1) drill-bit diameter, (2) the angle at which the tunnel intersects the tibial plateau (drill-guide angle), and (3) the tibial tunnel orientation in the transverse plane (transverse drill angl

An uncomplicated method for minimally invasive achilles tendon repair.

The Achilles tendon is the largest palpable tendon in the human body, and rupture of this tendon is not an uncommon injury encountered by foot and ankle surgeons. A number of different minimally invasive methods have been described for repair of the ruptured Achilles tendon. In this article, we describe a relatively simple, minimally invasive technique of Achilles tendon repair that does not require special instrumentation, the key requirement being that of a sponge forceps.

[Blauth classification of thumb hypoplasia: thought and proposition of modification].

Using the conclusions of the previous article concerning the therapeutic indications for handling hypoplastic thumb, we suggest an amendment to the current Blauth classification. This alteration is based on the management of this congenital anomaly, whereas the Blauth classification describes the morphological characteristics of the malformation. It appears for us more usable in practice by its easiness to be memorized.
