Haematogenous osteomyelitis: clinical presentation and outcome in three dogs.

Haematogenous osteomyelitis is a rare form of bone infection in adult dogs. Most commonly the infection is iatrogenic or traumatic in origin. The authors report three different presentations of haematogenous osteomyelitis: a focal pelvic localisation in a growing dog, a vertebral lesion in an adult dog with associated neurological signs and a multifocal affection in another adult dog with concomitant pathological fractures. Clinical signs included pyrexia of undetermined origin, focal pain and lameness. Diagnostic investigation included radiographic imaging, bone scintigraphy, magnetic resonan…

Chronic multifocal osteomyelitis: is infectious causation a moot point?

Chronic multifocal osteomyelitis (CMO) is an uncommon disease entity with descriptions possibly emanating from the medical literature over one century ago, and there are numerous disease entities which have been historically detailed and which are probably synonymous. The illness is one of chronicity with exacerbating and remitting focal bony lesions. The differential diagnosis for a bony lesion which ultimately proves to be CMO is initially quite broad. There is no absolute pathognomonic clinical finding, and the diagnosis is highly dependent on clinical course, histopathology, and an absence…

Triggering mechanisms of neuroarthropathy following conservative surgery for osteomyelitis.

BACKGROUND: The purpose of this study was to raise awareness and stimulate discussion of the possible triggering factors of Charcot neuroarthropathy by presenting the case of one patient who had both undergone surgery and was suffering from osteomyelitis. CASE REPORT: We have extracted one case from our data set for a patient who underwent conservative surgery for osteomyelitis and subsequently developed acute Charcot in the midfoot. We present the clinical findings, photographs and X-ray studies. Preoperative X-ray showed irregular severe bone destruction in the fourth metatarsal head and a …

Pelvic osteomyelitis mimicking septic hip arthritis: a case report.

Peripelvic infections are rare, compared with the incidence of septic hip arthritis, but are serious, requiring emergent treatment. They often are not included in differential diagnoses for patients presenting with fever, pain, inability to bear weight, elevated white blood cell count, and elevated erythrocyte sedimentation rate. Most patients are treated initially as a septic hip arthritis. Early diagnosis and treatment are crucial to outcome in peripelvic abscess. Use of MRI may help to elucidate the correct diagnosis. Previously reported peripelvic infections included obturator internus and

Pyogenic vertebral osteomyelitis: a systematic review of clinical characteristics.

OBJECTIVES: Vertebral osteomyelitis is a cause of back pain that can lead to neurologic deficits if not diagnosed in time and effectively treated. The objective of this study was to systematically review the clinical characteristics of pyogenic vertebral osteomyelitis (PVO). METHODS: The authors conducted a systematic review of the English literature. The inclusion criteria included studies with 10 or more subjects diagnosed with PVO based on the combination of clinical presentation with either a definitive bacteriologic diagnosis or pathological and/or imaging studies. RESULTS: The 14 studi…

[Acute multifocal osteomyelitis caused by haemolytic group B streptococci].

Invasive infections due to group B streptococci (Streptococcus agalactiae) usually occur in the peri- and neonatal setting or in adults with chronic underlying diseases. In the presented case a severe infection of haematogenous osteomyelitis caused by group B streptococci in a previously healthy 68-year-old woman is reported. Group B streptococci were isolated in three of three blood culture bottles drawn from the patient at the time of admission to hospital due to fever of unknown cause and pain in the upper and lower extremities as well as the back. A technetium scan showed involvement of th…

Circumferential petrosectomy for petrous apicitis and cranial base osteomyelitis.

OBJECTIVE: Petrous apicitis and cranial base osteomyelitis are life-threatening conditions. A surgical management may be necessary in cases that progress, in conditions that fail to improve with medical treatment, or in cases with impending complications. In this study, we describe a technique to remove the maximum amount of infected temporal bone while preserving the integrity of the peripheral auditory pathway and facial nerve. STUDY DESIGN: Retrospective study. SETTING: Tertiary referral hospital. PATIENTS: Five patients with impending complications, whose disease progressed or whose con…

Chronic osteomyelitis: a continuing orthopaedic challenge in developing countries.

Nine patients with chronic osteomyelitis, three with problems due to diagnosis, three with dilemmas regarding treatment and three with other complications are presented. It is suggested that although the diagnosis of osteomyelitis in most cases is straightforward, presentation might sometimes be similar to other conditions, which can lead to a dilemma in the diagnosis. Because of the formidable complications, which may be difficult to manage and the difficulty in guaranteeing permanent cure, the best approach is prevention by judicious treatment of acute haematogenous osteomyelitis and of open…

Acute haematogenous osteomyelitis: microbial conversion and unusual age presentation.

From 1983 to 1989, 110 cases of haematogenous osteomyelitis were studied retrospectively. The most commonly affected were children under 1 year. No adult cases were reported. Staphylococcus aureus was isolated from 72.7% of cases. During 1992-1997, 80 cases were studied prospectively. The most commonly affected were children aged 9 years. This group included 19 adults. S. aureus was isolated from 43.7% of the cases. There was a clear difference in the incidence of S. aureus and age presentation in the cases before and after the Gulf conflict. Working children and malnutrition might have caused…

Osteomyelitis in sickle cell haemoglobinopathy with elevated fetal haemoglobin.

The prevalence rate and characteristics of osteomyelitis in children below 15 years of age were studied in a population characterized by a high prevalence of sickle cell disease (SCD) with elevated fetal haemoglobin (HbF). All children born in our institution routinely undergo a haemoglobinopathy screening test. Osteomyelitis developed in 25 children (16 boys and nine girls) with a mean age of 5.5 years. Fourteen had sickle cell disease (SCD) and 11 had no haemoglobinopathy. The prevalence of osteomyelitis in children with SCD was 69 times higher than in those without (283 vs 4/10(5)). All 14 …

Subchondral acute inflammation in severe arthritis: a sterile osteomyelitis?

Although arthritis is often associated with synovial inflammation, the osseous changes in inflammatory and degenerative arthritis are principally reactive, and typically lack an acute inflammatory component. We have recently encountered several osteoarticular specimens removed at the time of large joint arthroplasty that have shown a distinctive pattern of subchondral acute inflammation (SCAI) resembling acute bacterial osteomyelitis. These microscopic findings heretofore have not been recognized as a component of the histopathology of arthritis. To determine the frequency of SCAI, we examined…
