Mucoepidermoid carcinoma (MEC), an extremely rare tumor, arises from the epithelial component of preexisting parotid Warthin tumors (WT). Among the 309 cases of surgically resected WTs in Chinese PLA General Hospital and Beijing Shijitan Hospital of Capital Medical University, 5 cases (1.6%) fulfilled the criteria for MECs transformed from WTs. Clinicopathological characteristics of MECs was demonstrated in order to avoid misdiagnosis of this rare type of tumor. All the 5 patients, 3 males and 2 females, presented painless masses in the parotid gland. MECs were located inside or at the edge of

Parotidectomy for benign tumors is undergoing constant evolution. The potential for recurrence and malignant transformation of pleomorphic adenomas creates complexities that have forced head and neck surgeons to undertake more comprehensive parotid surgery with facial nerve dissection. This approach carries inherent morbidities, including facial nerve injury, Frey syndrome, and facial asymmetry, that have to be addressed. Extracapsular dissection is compared with conventional superficial parotidectomy; surgical histologic findings are discussed as well as outcome data. More novel approaches ar

BACKGROUND: To determine if Extracapsular Dissection (ECD) versus Partial Superficial Parotidectomy (PSP) differ in terms of tumor recurrence and transient and permanent facial nerve dysfunction. ECD does not dissect the facial nerve. PSP dissects the facial nerve and provides a 1-2 cm margin around the tumor.
METHODS: A 38-year Ovid Medline search (1970-2008) and an historical review with statistical analysis of published manuscripts were performed.
RESULTS: Tumor recurrence occurred in 36/1183 (3.0%) ECD cases and 1/340 (0.3%) PSP cases; (p < 0.05). Permanent facial nerve dysfunction occur

Desmoplastic malignant melanoma often arises in sun damaged skin of the head and neck and shows frequent neurotropism. Although metastatic melanoma frequently involve the parotid, direct spread to the parotid has been rarely reported. We evaluated five cases of desmoplastic malignant melanoma involving the parotid gland with clinical and pathological evidence of precursor cutaneous lesions in four of the five cases. The parotid involvement in four cases was tumoural, and three of these were not clinically suspected to be melanoma. The histological appearance in all five cases was that of a sar

Sarcomas arising in a parotid gland is an uncommon occurrence and a sarcomatous transformation in a recurring pleomorphic adenoma is even rare. Diagnosis of salivary gland tumors has proven to be challenging. Pleomorphic adenomas (PA) are the most commonly diagnosed tumors of the salivary gland and a malignant transformation in preexisting or recurring ones is even thinner on the ground. Malignant transformations in a pleomorphic adenoma can occur in carcinomas, carcinosarcomas, or sarcomas. We are sharing an interesting case of a 68-year-old male with a parotid region swelling with a history

The objective of this study is to verify the role of digital modified parotid tumor zoning method in modified parotid incision. The data of patients with parotid benign tumors from November 2021 to December 2023 were collected. Through the use of digital technology for soft tissue reconstruction, the parotid tumor was divided into four areas according to the digital image marker points. We designed the surgical incision according to the parotid gland division, found that it was feasible to guide the incision selection by division, and summarized the common incision and division corresponding,

Phenotypic plasticity refers to the ability of tumour cells to undergo molecular and phenotypic changes in response to microenvironmental stresses, genetic and epigenetic alterations, or treatment-induced selective pressures, thereby promoting tumour progression. This case report discusses a man in his 70s with pleomorphic adenoma of the parotid, where phenotypic plasticity led to the development of pleomorphic liposarcoma in the parotid through transdifferentiation. Sarcomas can arise in the parotid from pre-existing pleomorphic adenomas or de novo, with only eight reported cases of primary p

It is important to define the accuracy of fine-needle aspiration cytology (FNAC) in the diagnosis of Warthin tumor (WT). This systematic review and meta-analysis evaluated the accuracy of FNAC in the diagnosis of WT in the parotid gland and WT growth rate. For determination of FNAC accuracy, 17 studies, encompassing 1710 cases, were included. Pulled random model estimates of sensitivity, specificity, PPV, and NPV were 93.7% (95%CI: 92.1, 95.3), 97.9% (95%CI: 97, 98.9), 93.3% (95%CI: 91.5, 95.1), and 97.4% (95%CI: 96.4, 98.4), respectively. FNAC is highly reliable for the diagnosis of WT of the

BACKGROUND: High-grade transformation Adenoid cystic carcinoma (HGT-AdCC) of the parotid gland is a rare transformation noted in slow growing low grade AdCC. Perineural invasion and spread is an important feature of this tumor. Temporal bone involvement is rare. A total of only 10 cases of HGT-AdCC in parotid gland has been reported in literature so far predominantly in the elderly with peak incidence in 5th-6th decade.
CASE PRESENTATION: We present a young lady of HGT-AdCC of right parotid gland with temporal bone involvement in the form of isolated perineural invasion (PNI) of facial nerve

OBJECTIVES: The aim of this study was to assess the relationship between the stimulation amplitude and the distance to the facial nerve.
METHODS: This study was designed as a prospective clinical study. A total of 20 patients (12 males, 8 females) were included. Partial superficial parotidectomy was performed in all patients with intraoperative facial monitoring. Measurements were made on the main trunk and major branches. Stimulation was started at 1 mA and incrementally increased to 2 and 3 mA's. The shortest distance creating a robust response (>100 mV) was recorded.
RESULTS: At 1 mA, 2 m

Handling of the facial nerve during surgery for parotid cancer is relevant for the patient's long-term quality of life. In about two thirds of cases, the facial nerve is not affected by the tumor. In these cases, in addition to complete tumor resection, identification and preservation of the facial nerve are important components of a successful operation. If the nerve is infiltrated by the tumor, the affected part must be resected during radical parotidectomy. When possible, primary nerve reconstruction leads to the best long-term cosmetic and functional outcomes. Individual selection of the o
