Strategies to recruit plateletpheresis donors from a registry of HLA-typed, unrelated, bone marrow donors.

Rising demand for single-donor platelet components--from random donors, to maintain platelet inventories, or from HLA-compatible donors, to support alloimmune platelet-refractory patients--necessitated increasing the size of a community plateletpheresis donor registry. This study compares two strategies for recruiting whole-blood donors into a plateletpheresis program. The whole-blood donors who were asked to participate in this study had recently joined an unrelated bone marrow donor registry and had been HLA-typed as part of that process. An in-person recruitment strategy, which was time-int

Interest of presbyopes in contact lens correction and their success with monovision.

The presbyopic population is seen as a large potential source of contact lens wearers. The aims of this study were: (1) to estimate the percentage of presbyopes interested in contact lenses, (2) to ascertain the success of interested presbyopes with monovision correction, and (3) to determine the percentage willing to continue wearing monovision lenses after 1 month's trial. Seven practitioners in Sydney surveyed consecutive presbyopes attending their practices about their interest in contact lenses. Of the 1133 presbyopes surveyed, 314 (28%) were interested in trying monovision lenses. A tota

Predictors of non-participation in a cardiac telerehabilitation programme: a prospective analysis.

AIMS: Current cardiac telerehabilitation (CTR) interventions are insufficiently tailored to the preferences and competences of individual patients, which raises the question whether their implementation will increase overall participation and adherence to cardiac rehabilitation (CR). However, research on patient-specific factors that influence participation and adoption of CTR interventions is scarce. The aim of this study was to evaluate which patient-related characteristics influence participation in a novel CTR intervention in patients with coronary artery disease. METHODS AND RESULTS: Thi

Operationalizing patient and stakeholder engagement in high-acuity trials: Lessons from IMPROVE-AD.

Patient and stakeholder engagement is increasingly emphasized in clinical research, yet practical guidance on how to operationalize engagement in high-acuity, multicenter surgical trials remains limited. We describe the design and early implementation of a structured engagement strategy within the IMPROVE-AD (Improving Outcomes in Vascular Disease-Aortic Dissection) trial. IMPROVE-AD is a pragmatic, multicenter randomized trial comparing optimal medical therapy alone with optimal medical therapy plus thoracic endovascular aortic repair for uncomplicated type B aortic dissection. A formal patie

Improving patient engagement in clinical trials: Launching the BOAT trial with key stakeholders.

Non-cystic fibrosis bronchiectasis is a chronic inflammatory disease associated with recurrent exacerbations and reduced quality of life. The Bronchiectasis: Optimising Azithromycin prevention Treatment (BOAT) trial is a double-blind, randomised, non-inferiority trial to compare the clinical effectiveness of complete discontinuation of azithromycin prophylaxis vs continuous azithromycin prophylaxis as a standard of care. A Study Within A Trial (SWAT) was developed for the BOAT trial to assess the use of video animations as a recruitment tool, guided by active Patient and Public Involvement and

Predictors of two forms of attrition in a longitudinal health study involving ageing participants: an analysis based on the Whitehall II study.

BACKGROUND: Longitudinal studies are crucial providers of information about the needs of an ageing population, but their external validity is affected if partipants drop out. Previous research has identified older age, impaired cognitive function, lower educational level, living alone, fewer social activities, and lower socio-economic status as predictors of attrition. METHODS: This project examined attrition in participants of the Whitehall II study aged between 51-71 years, using data from questionnaires participants have completed biennially since 1985 when the study began. We examine the

Factors associated with clinical research participation among Medicare beneficiaries with prostate cancer in the United States.

INTRODUCTION: Clinical research participation among patients with prostate cancer remains limited, and demographic, clinical, and contextual factors may influence opportunities for participation. Although disparities in prostate cancer diagnosis and treatment are well documented, less is known about the factors associated with clinical research participation in this population. This study examined patient-level and contextual factors associated with clinical research participation among older Medicare beneficiaries with prostate cancer. METHODS: A retrospective claims-based analysis was condu

Effects of a Patient Decision Aid for Nonmetastatic Prostate Cancer Established in Routine Care: The Randomized Controlled EvEnt-PCA Trial.

BACKGROUND AND OBJECTIVE: Since 2016, >21 000 patients with prostate cancer (PC) used our personalized online decision aid in routine care in Germany. We analyzed the effects of this online decision aid for men with nonmetastatic PC in a randomized controlled trial. METHODS: In the randomized controlled EvEnt-PCA trial, 116 centers performed 1:1 allocation of 1115 patients with nonmetastatic PC to use an online decision aid (intervention = I) or a printed brochure (control = C). The primary outcome was treatment decision after 14 mo. The secondary endpoints were knowledge, acceptance, decisio

Shared decision-making in central centrifugal cicatricial alopecia.

Central centrifugal cicatricial alopecia (CCCA) is a primary lymphocytic scarring alopecia, which predominantly occurs in middle-aged Black women. The benefits and use of shared decision-making (SDM) models in patients with CCCA have yet to be studied. We conducted a cross-sectional survey to investigate the role of SDM in the management of CCCA. SDM processes were evaluated using the nine-item Shared Decision-Making Questionnaire (SDM-Q-9), the Decisional Regret Scale (DRS) and the Control Preferences Scale (CPS). Between June and August 2023, members of the Scarring Alopecia Foundation were

Explorative observational study of Dutch patient-clinician interactions: operationalisation of personal perspective elicitation as part of shared decision-making in real-life audio-recorded consultations.

OBJECTIVES: Patients' preferences, values and contexts are important elements of the shared decision-making (SDM) process. We captured those elements into the concept of 'personal perspective elicitation' (PPE), which reflects the need to elicit patients' preferences, values and contexts in patient-clinician conversations. We defined PPE as: 'the disclosure (either elicited by the clinician or spontaneously expressed by the patient) of information related to the patient's personal preferences, values and/or contexts potentially relevant to decision-making'. Our goal was to operationalise the c

Assistive technology and people: a position paper from the first global research, innovation and education on assistive technology (GREAT) summit.

Assistive technology (AT) is a powerful enabler of participation. The World Health Organization's Global Collaboration on Assistive Technology (GATE) programme is actively working towards access to assistive technology for all. Developed through collaborative work as a part of the Global Research, Innovation and Education on Assistive Technology (GREAT) Summit, this position paper provides a "state of the science" view of AT users, conceptualized as "People" within the set of GATE strategic "P"s. People are at the core of policy, products, personnel and provision. AT is an interface between th

Treat-to-target (T2T) recommendations for gout.

OBJECTIVES: The treat-to-target (T2T) concept has been applied successfully in several inflammatory rheumatic diseases. Gout is a chronic disease with a high burden of pain and inflammation. Because the pathogenesis of gout is strongly related to serum urate levels, gout may be an ideal disease in which to apply a T2T approach. Our aim was to develop international T2T recommendations for patients with gout. METHODS: A committee of experts with experience in gout agreed upon potential targets and outcomes, which was the basis for the systematic literature search. Eleven rheumatologists, one ca

Applying a deliberation model to the analysis of consultations in haemophilia: Implications for doctor-patient communication.

OBJECTIVE: Literature highlights the importance of communication in order to achieve patient's adherence. However, the specific dialogical components likely to favor patient adherence are not clear. In this study, the deliberation dialogue model was applied as an ideal model of optimal deliberation to real physician-patient consultations in the field of hemophilia in order to identify misalignments with the model and possible improvements in physician-patient communication. METHODS: By applying the deliberation model, we analyzed a corpus of 30 check-up consultations in hemophilia. RESULTS:

Client-centered assessment.

When occupational therapists assess function or occupational performance, they must consider each person's unique needs and abilities, as well as the environmental and social factors that may be affecting the clients' performance. Therefore, occupational therapists must use evaluation tools that are individualized and sensitive to the clients' varying needs and situations. One approach to individualized evaluation is the use of a client-centered process of setting goals and assessing change. This paper describes the development of a methodology for a client-centered assessment currently being

Direct family participation in care in adult intensive care: A Walker and Avant concept analysis.

BACKGROUND: Family participation is increasingly recognised in adult intensive care units (ICUs). However, family participation, involvement and engagement are often used as umbrella terms encompassing presence, emotional support, communication, shared decision-making and direct care. Consequently, the meaning and professional boundaries of direct family participation in care (DFPC) remain unclear. AIM: To clarify the defining attributes, antecedents, consequences, empirical referents and operational definition of DFPC in adult intensive care. METHODS: Walker and Avant's method guided the an

Comparing the ability of OPTION(12) and OPTION(5) to assess shared decision-making in genetic counselling.

OBJECTIVES: OPTION(12) is the most widely used tool to measure shared decision-making (SDM) in health care. A newer scale, OPTION(5), has been proposed as a more parsimonious measure that better addresses core concepts of SDM. This study compares OPTION(5) to OPTION(12) in prenatal genetic counselling. METHODS: Two raters independently used OPTION(12) and OPTION(5) to score 27 clinical encounters between genetic counsellors (GC) and women with pregnancies at increased risk for genetic conditions. Global and item scores on the two instruments were compared to test concurrent validity and to id

Patient involvement in diabetes-related foot ulcers: What do patients need and prefer when undergoing treatment at multidisciplinary outpatient foot centres? A phenomenological hermeneutic study.

We adopted a qualitative descriptive design to gain a broad understanding of the experiences, needs and preferences. Semi-structured interviews were conducted with 21 patients with diabetes-related foot ulcers using an interview guide. Patients were recruited from the four multidisciplinary foot centres in the Eastern Danish Region of Zealand. The methodology utilised a phenomenological hermeneutical perspective, allowing participants to share their experiences. The data analysis was inspired by Paul Ricoeur's philosophy of text interpretation. Three themes were identified: (1) Becoming depend

Experiences of Patients After Withdrawal From Cancer Clinical Trials.

IMPORTANCE: Cancer clinical trials (CCTs) provide patients an opportunity to receive experimental drugs, tests, and/or procedures that can lead to remission. For some, a CCT may seem like their only option. Little is known about experiences of patient-participants who withdraw or are withdrawn from CCTs. OBJECTIVE: To examine patient-participants' experiences during withdrawal from CCTs. DESIGN, SETTING, AND PARTICIPANTS: This qualitative, descriptive study used a semistructured interview designed specifically for it, with open-ended and probing questions. The study took place at a National
