Case Report: Innovative anesthetic approaches for whole lung lavage in an infant with pulmonary alveolar proteinosis.

INTRODUCTION: Pulmonary alveolar proteinosis (PAP) is a rare disease in infancy characterized by the accumulation of lipoprotein material within the alveoli, leading to impaired gas exchange, ventilation-perfusion mismatch, and, in severe cases, respiratory failure that may result in death. Treatment options include medical therapy and whole lung lavage (WLL), typically requiring lung isolation techniques or extracorporeal membrane oxygenation. Previous studies have reported the application of several lung isolation techniques in pediatric WLL. However, the use of a bronchial blocker (BB) in i

[Bilateral recurrent laryngeal nerve paralysis in a child following a neurosurgical operation].

We experienced a case of 7-year-old boy who developed bilateral recurrent laryngeal nerve paralysis following an elective neurosurgical operation under oxygen-nitrous oxide-isoflurane anesthesia. He underwent a removal of brain tumor in the supratentorial region on supine position. After the removal of the endotracheal tube in the intensive care unit, he developed marked respiratory effort and inspiratory stridor. A diagnosis of bilateral recurrent laryngeal nerve paralysis was made by a direct diagnostic laryngoscopy. The patient needed continuous care for his airway patency with tracheotomy

Robert Abbe and Wilms' Tumor.

Robert Abbe (1851-1928), a true polymath of American surgery who made seminal contributions to Wilms' tumor (WT), is largely unknown to pediatric surgeons. The long list of his achievements is formidable. Although Abbe was principally a general surgeon, he was a pioneer of neurological, transplant, vascular, colorectal and plastic surgeries, besides being an innovative radiotherapist. In an era when antibiotics, safe anesthesia, blood transfusion, anticancer drugs, radiotherapy and intensive care units were non-existent, Abbe successfully performed pediatric nephron-sparing surgery (partial ne

Ultrasound-Guided Stellate Ganglion and Interscalene Blocks for Refractory Pediatric Parsonage-Turner Syndrome: A Case Report and Clinical Implications.

BACKGROUND: Parsonage-Turner syndrome (PTS) is an uncommon inflammatory brachial plexopathy; pediatric and bilateral cases are rare, and evidence for managing refractory pediatric pain is limited. CASE REPORT: A previously healthy 6-year-old girl developed sudden severe left upper-limb pain with progressive wrist-drop in August 2023; by October, the right arm was affected. Nerve conduction studies/electromyography confirmed brachial plexopathy and magnetic resonance imaging showed diffuse thickening of left C5-C7. Laboratory tests were unremarkable, except for an isolated anti-glycosphingolip

Transesophageal Bronchoscopic Ultrasound-guided Fine-needle Aspiration (EUS-B-FNA) in a 3-Year-Old Child.

Evaluation of mediastinal lymphadenopathy in children is challenging and surgical procedures (mediastinoscopy/thoracotomy) are usually performed wherever tissue sampling is required. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a widely utilized and minimally invasive modality for evaluation of mediastinum (lymphadenopathy, masses, and nodal staging in patients with lung cancer) in adults. Smaller size of pediatric trachea potentially limits the use of EBUS. The EBUS bronchoscope can also be introduced into the esophagus for mediastinal evaluation and samplin

Contemporary management of refractory pediatric glaucoma.

PURPOSE OF REVIEW: The management of pediatric glaucoma poses a unique challenge in terms of maintaining lifelong vision and combating an aggressive scarring response from surgery. Contemporary literature regarding the surgical management of children with pediatric glaucoma who fail, or are at a high risk of failure, from conventional surgery is limited. The aim of this review is to highlight recent developments in relation to the current opinion regarding the management of children with refractory pediatric glaucoma. RECENT FINDINGS: Some studies have reported impressive success rates with c

Use of a magnetic double J stent in pediatric patients: A case-control study at two Canadian pediatric centers.

BACKGROUND: Ureteral stents with magnetic tips (Blackstar©) were recently approved for use in Canada. To our knowledge this is the first published evidence of their use in pediatric patients. Traditionally, pediatric stent insertion and removal are performed under general anesthetic. Magnetic stents have three main benefits in pediatric patients; cost savings, decreased OR time and reduced general anesthetic exposure. METHODS: This study was a proof of concept pilot, ran from May 2017-May 2018 to demonstrate the safety and efficacy of magnetic stents in pediatric patients. Patients undergoing

Anesthesia for the adult patient with an unrepaired congenital cyanotic heart defect: a case report.

Adult congenital heart disease, previously considered a rare comorbidity, is increasingly becoming a reality for today's anesthesia providers. Improvements in prenatal diagnosis, sophisticated surgical techniques and equipment, advances in pediatric critical care, enhanced efficacy of cardiovascular pharmacologic agents, and an overall increase in postrepair survival rates have resulted in an estimated population of approximately 800,000 adults with congenital heart disease. Despite successful surgical repair or palliation, these individuals present the anesthesia provider with a multitude of

Treating pediatric port-wine stains in aesthetics.

A port-wine stain (PWS) is a vascular birthmark present in 0.3% to 0.5% of newborns. If untreated, this erythematous patch will grow proportionally with the child to thicken and darken with age. PWSs have implications for the child's quality of life for many years, with cosmetic, medical, and psychosocial disability. Controversy exists in many aspects surrounding laser treatment of these birthmarks in the pediatric population. We have reviewed the clinical features as well as the historic and current laser treatment of PWS. We have also examined the current hot topics of debate surrounding the
