Covert Tracking to Visual Stimuli in Comatose Patients With Traumatic Brain Injury.

OBJECTIVES: This study investigated video eye tracking (VET) in comatose patients with traumatic brain injury (TBI). METHODS: We recruited healthy participants and unresponsive patients with TBI. We surveyed the patients' clinicians on whether the patient was tracking and performed the Coma Recovery Scale-Revised (CRS-R). We recorded eye movements in response to motion of a finger, a face, a mirror, and an optokinetic stimulus using VET glasses. Patients were classified as covert tracking (tracking on VET alone) and overt tracking (VET and clinical examination). The ability to obey commands w

Internal medicine intern performance on the gastrointestinal physical exam.

OBJECTIVES: The gastrointestinal (GI) physical exam provides critical information about underlying disease states. However, since assessment of physical examination skills is rarely conducted as part of internal medicine residency training, little is known about resident performance on the GI physical exam. METHODS: During a clinical skills assessment that took place between November 2019 and February 2020, internal medicine interns examined the same patient with chronic liver disease while being observed by faculty preceptors. We compared the exam maneuvers performed with those expected by t…

Look into my eyes: What can eye-based measures tell us about the relationship between physical activity and cognitive performance?

BACKGROUND: There is a growing interest to understand the neurobiological mechanisms that drive the positive associations of physical activity and fitness with measures of cognitive performance. To better understand those mechanisms, several studies have employed eye-based measures (e.g., eye movement measures such as saccades, pupillary measures such as pupil dilation, and vascular measures such as retinal vessel diameter) deemed to be proxies for specific neurobiological mechanisms. However, there is currently no systematic review providing a comprehensive overview of these studies in the fi

Surgical treatment of endo-periodontal lesion - а case report.

The interrelationship between periodontium and pulp makes the treatment of combined endo-periodontal lesions (EPL) more challenging. It involves successful elimination of both periodontal and endodontic lesion. The current case report represents the efficacy of regenerative potential of enamel matrix derivates (Emdogain®) in endo-periodontal lesion after successful endodontic treatment. A 39-year-old woman presented with EPL on a left first mandibular molar. After the initial 3 months of healing, the clinical examination showed that the furcation involvement still prevailed. A decision was mad

The role of Meibomian glands on the development of dry eye disease in patients with unilateral pseudoexfoliation.

PURPOSE: To evaluate the Meibomian glands and presence of dry eye in patients with unilateral pseudoexfoliation. METHODS: The study included 32 patients with clinical pseudoexfoliation (PEX) in one eye and 30 controls. Schirmer and tear film break-up time (TBUT) tests were performed, along with a detailed ophthalmological examination, meibography and evaluation of areas of meibomian gland loss. Subjective symptoms were evaluated using the ocular surface disease index (OSDI). RESULTS: Schirmer test results for eyes with or without PEX were similar (10.438±4.47mm and 11.563±4.35mm, respectivel

The utility of augmented teledermatology to improve dermatologist diagnosis of cellulitis: a cross-sectional study.

Dermatology consultation for cases of presumed cellulitis improves diagnostic accuracy and management. However, access to in-person consultation remains limited, a gap that could be filled with teledermatology. Augmented teledermatology may improve outcomes. In this cross-sectional study, 20 dermatologists (60% of whom reported conducting inpatient consults > 1 month per year) reviewed 10 real-life cases representing either cellulitis or pseudocellulitis as diagnosed by in-person dermatology consultation. For each case, respondents recorded their diagnosis, confidence, and management decisions

Differential disease diagnoses of epistaxis based on dynamic uncertain causality graph.

PURPOSE: Epistaxis is a common symptom and can be caused by various diseases, including nasal diseases, systemic diseases, etc. Many misdiagnosis and missed diagnosis of epistaxis are caused by lack of clinical knowledge and experience, especially some interns and the clinicans in primary hospitals. To help inexperienced clinicans improve their diagnostic accuracies of epistaxis, a computer-aided diagnostic system based on Dynamic Uncertain Causality Graph (DUCG) was designed in this study. METHODS: We build a visual epistaxis knowledge base based on medical experts' knowledge and experience.

Clinically Significant Distress and Physical Problems Detected on a Distress Thermometer are Associated With Survival Among Lung Cancer Patients.

BACKGROUND: The distress thermometer is a well-established screening tool to detect clinically significant distress in cancer patients. It is often administered in combination with the problem list, differentiating further between various (e.g., physical and emotional) sources of distress. OBJECTIVE: The present study aimed to extend previous research on the association between distress and overall survival. A further exploratory analysis aimed to evaluate the predictive value of the problem list for overall survival. METHODS: Patients (n = 323) with newly diagnosed lung cancer were recruite

Assessing a child or adolescent with low back pain is different to assessing an adult with low back pain.

In contrast to an assessment of an adult presenting with low back pain (LBP), clinicians should utilise different approaches when assessing … to actively listen to a child's narrative (and that of their guardian) and to conduct an effective physical examination. This viewpoint also summarises the relationship between potential clinical diagnoses and key elements of a physical examination. Deciding on the type and timing of paediatric-specific physical examination tests requires unique child-centred conside …

Bilateral Epibulbar Dermolipomas in a Patient With Goldenhar Syndrome.

A 19-year-old woman consulted for bilateral superotemporal conjunctival lesions that had been present since birth. Physical examination also revealed hemifacial microsomia and bilateral auricular appendages. Medical history included heart, hearing, and neurologic problems. Excision biopsy of these lesions showed nodular proliferations of adipose tissue, which corresponded to the diagnosis of dermolipomas in the setting of Goldenhar syndrome. Complete examination for other first and second branchial arch anomalies is warranted in patients presenting with such lesions.

Deep Gluteal Pain in Orthopaedics: A Challenging Diagnosis.

Identifying the specific source of gluteal pain can elude the most seasoned orthopaedic diagnosticians. Patients will often present with a protracted course of symptoms, and failure to successfully identify and treat the underlying etiology leads to frustration for both patient and clinician. Pain deep in the buttocks can arise from compression, inflammation, or injury of one or more of the structures in this anatomically dense area. Although sacroiliitis, hip arthritis, and trochanteric bursitis may also masquerade as gluteal pain, sciatic nerve irritation in its various presentations causes

Prevention and Management of the Failed Flatfoot.

The acquired adult flatfoot deformity is a challenging condition to manage and even more so in a revision setting. To "get it right" the first time, it is important to completely understand the deformity by performing a complete history, careful physical examination, and deliberate evaluation of radiographs. There are several techniques to achieve the complete correction of complex flatfoot deformities. There are also strategies for revising failed flatfoot reconstruction in the setting of overcorrection and undercorrection and malunion.
