Response of juvenile circumscribed pityriasis rubra pilaris to topical tazarotene treatment.

We report juvenile circumscribed pityriasis rubra pilaris (type IV) in a 12-year-old Afghani girl in whom local therapy with tazarotene led to rapid, sustained remission. Tazarotene might be considered a useful treatment for circumscribed pityriasis rubra pilaris.

Acute juvenile pityriasis rubra pilaris: a case report after mononucleosis infection.

A 24-year-old male boy presented dermatosis which first appeared acutely after an infection at age 17. Clinical and histopathologic examinations were consistent with a diagnosis of juvenile pityriasis rubra pilaris type III. Treatment with UVB narrow-band led to complete resolution of the dermatitis within 1 year. Pityriasis rubra pilaris is a papulosquamous disorder of unknown etiology, which can be treated with retinoids, methotrexate, cyclosporine, and narrow-band phototherapy.

Septic, CD-30 positive febrile ulceronecrotic pityriasis lichenoides et varioliformis acuta.

We report life-threatening febrile ulceronecrotic pityriasis lichenoides et varioliformis acuta in an 8-year-old girl. Hemorrhagic-crusted papules and plaques covered over 90% of the patient's body, leaving her susceptible to Pseudomonas aeruginosa and Staphylococcus epidermidis bacteremia as well as Candida parapsilosis fungemia. Sepsis delayed definitive treatment of the underlying cutaneous disease for 2 weeks. Combined therapy with methotrexate and cyclosporin caused remission of the process. Although immunohistochemistry revealed CD-30 positive cells, suggesting the diagnosis of lymphomat…

Comparative study of ketoconazole versus selenium sulphide shampoo in pityriasis versicolor.

Forty patients suffering from pityriasis versicolor were treated with either 2% ketoconazole shampoo (20 patients) or 2.5% selenium sulphide shampoo (20 patients), once a week for three weeks. On global assessment after one month of start of therapy, 19 (95%) out of 20 patients treated with ketoconazole shampoo were cured while one case had mild residual disease. In selenium sulphide shampoo group, 17 (85%) out of 20 patients were cured, one had mild residual disease and two had considerable residual disease. No significant difference was observed in the response rates in the two groups. Relap…

Ultra-short topical treatment of pityriasis versicolor with 2.5% bifonazole cream.

The therapeutic efficacy of a preparation containing 2.5% bifonazole was investigated by comparing three different treatment modalities--A, B, and C. Group A used bifonazole only on Days 1, 2 and 3, and the Group C on Days 1, 3 and 5. Of the patients in Group A 56% had a negative mycological examination at the end of the study. The results obtained in Groups B and C were not significantly different: 92% of the patients had a negative mycological examination at the end of the study. Electron microscope (EM) studies showed morphological alterations such as loss of cytoplasmic organization with s

[Pityriasis-lichenoides-et-varioliformis-acuta-like drug exanthema caused by astemizole].

We report on a 40-year-old male patient who developed an unusual generalized drug eruption taking the form of a histologically confirmed pityriasis lichenoides et varioliformis acuta (PLEVA) after oral intake of the H1-antagonist astemizole. On two occasions, independently repeated medication with astemizole exacerbated the typical rash again. Oral exposure and the specific lymphocyte transformation test confirmed the suspected causal connection between astemizole and PLEVA.

Comparative clinicopathological study on pityriasis lichenoides chronica and small plaque parapsoriasis.

The term parapsoriasis refers to a group of chronic asymptomatic scaly dermatoses of unknown etiology about which there is still controversy over the nosology and nomenclature of the different conditions that comprise the group, particularly pityriasis lichenoides chronica (PLC) and small plaque parapsoriasis (SPP). In an attempt to establish the distinctive clinicopathologic features of these two dermatosis, we prospectively studied 44 patients who presented with the typical clinical and histologic picture of either of these two diseases. SPP was clinically characterized by scaly oval plaques…

Ultrastructure of pityriasis rubra pilaris with observations during retinoid (etretinate) treatment.

The light and electron microscopic structure of pityriasis rubra pilaris (PRP) is described in five patients. Hyperkeratosis, hypergranulosis, keratotic plugs in the follicular openings, acanthosis and focal parakeratosis were observed. A moderate perivascular infiltrate was seen in the upper dermis. Electron microscopy revealed moderately activated keratinocytes, a decreased number of tonofilaments and desmosomes, enlarged intercellular spaces, parakeratosis with lipid-like vacuoles and a large number of keratinosomes. Lymphoid cells were present in the epidermis in moderate numbers. At the d…

Complete Resolution of Pityriasis Rubra Pilaris With Targeted Treatment: A Case Report.

Pityriasis rubra pilaris (PRP) is a rare inflammatory dermatosis characterized by erythematous plaques with follicular hyperkeratotic papules, presenting challenges in diagnosis and management. Here, we present a case report of a 70-year-old male with a known medical history of diabetes mellitus and hypertension who presented with a two-year history of pruritic, purple papules, plaques, and scaly lesions distributed symmetrically on the hands, chest, and legs. Histological examination confirmed PRP, revealing classic features of PRP, including broad epidermal ridges with thick suprapapillary p…

Treatment of refractory pityriasis rubra pilaris with biologic therapy: a case series.

Pityriasis rubra pilaris (PRP) is a rare inflammatory dermatosis. There are limited data on its treatment with biologic therapies. We present a case series to evaluate the efficacy, time to primary response (TTPR) and tolerability of biologics in PRP management. A retrospective analysis of 11 patients with PRP treated with biologics at a tertiary Australian dermatology department was conducted. Patient demographics, prior therapies and treatment outcomes were reviewed. Biologics demonstrated variable efficacy: significant responses were seen with secukinumab (three of four, 75%), tildrakizumab…

Apremilast in the treatment of pityriasis lichenoides et varioliformis acuta: a case report.

OBJECTIVES: To evaluate the efficacy and safety of apremilast, a phosphodiesterase-4 (PDE-4) inhibitor, in treating pityriasis lichenoides et varioliformis acuta (PLEVA) and explore its potential mechanisms. METHODS: We present a 16-year-old male with refractory PLEVA who failed conventional therapies (systemic corticosteroids, acitretin). Treatment with oral apremilast (30 mg twice daily) was initiated. RESULTS: Significant improvement was observed within 1 week, with near-complete clearance by 8 weeks. No relapse or adverse events occurred during 12-month follow-up. CONCLUSIONS: Apremilas…

When Pityriasis Versicolor wears a vitiligo mask: a puzzle in an immunocompromised patient.

A 47-year-old male with a history of colonic adenocarcinoma and recent chemotherapy presented with persistent hyperpigmentation of the trunk, initially thought to be vitiligo due to the striking hyperpigmented lesions and well-demarcated hypopigmented skin. Histological examination of the hyperpigmented lesions, originally thought to represent normal skin, revealed findings consistent with lichen planus pigmentosus. Due to the clinical-histological discordance, a Scotch test was performed, which identified fungal elements consistent with pityriasis versicolor (PV). Treatment with oral fluconaz…
