A method is described for the fabrication of a closed hollow bulb obturator prosthesis using a hard thermoforming splint material and heat-cured acrylic resin. The technique allowed the thickness of the thermoformed bulb to be optimized for weight reduction, while the autopolymerized seal area was covered in heat-cured acrylic resin, thus eliminating potential leakage and discoloration. This technique permits the obturator prosthesis to be processed to completion from the wax trial denture without additional laboratory investing, flasking, and processing.

We present a unique surgical complication in a 19-year-old patient who underwent a "supported" Ross procedure for congenital aortic stenosis. In the present case, herniation of pulmonary autograft material through coronary fenestrations in the Dacron supporting material can be appreciated. This case suggests a possible need to modify surgical technique to ensure that all autograft tissue remains contained within the Dacron bolster.

CASE REPORT: A 55-year-old man presented with a retrograde acute type A aortic dissection with an entry tear 30 mm below the left subclavian artery. From 1995 through 1998, we encountered eight patients with infected Dacron patches after previous carotid endarterectomy. A single-center prospective study is presented on 35 patients who underwent anterior cruciate ligament reconstruction using a Dacron ligament prosthesis. Thirteen cases of diaphragmatic rupture following blunt trauma or gunshot wounds are presented. In 10 cases the diagnosis of diaphragmatic rupture was made immediately following the injury, and the defect was closed by primary diaphragmatic suture. Aortoenteric fistula is a dramatic and rare complication, which may occur following abdominal aortic surgery. In this report, we present images of a 65-year-old patient who presented with acute peritonitis two months after aortobifemoral bypass grafting operation with a bifurcated dacron graft.
