Sleep Disorders in Children with Rett Syndrome.

Rett syndrome (RTT) is an X-linked neurodevelopmental disorder marked by neurological regression, autonomic dysfunction, seizures, and significant sleep and breathing abnormalities. About 80% of affected individuals, especially young children, experience sleep disturbances such as insomnia, sleep-disordered breathing, nocturnal vocalizations, bruxism, and seizures. Breathing irregularities during sleep-like apnea, alternating hyperventilation, and hypoventilation-are common, with both obstructive and central sleep apnea identified through polysomnography. This review focuses on the prevalent s…

Timing of scratch and limb movements during sleep in children with atopic dermatitis.

In this retrospective analysis of children with atopic dermatitis (n = 6) who coincidentally had a video polysomnography, we found that most nocturnal limb movements in children with atopic dermatitis are non-scratch versus scratch, 109.0 ± 67.9 vs. 15.3 ± 5.4 (p = 0.01). Average scratch duration was 8.4 ± 2.7 s, which was not different by sleep stage. Scratch movements are distinct in timing, occurring most often during N2 sleep, in the first third of sleep, and peaking at 90 minutes after sleep onset, corresponding with completion of the first sleep cycle.

Research progress in rapid eye movement sleep behavior disorder.

Rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by abnormal dream acting behavior such as vocalization and twitching related to dream content during REM sleep. The diagnosis requires polysomnography demonstrating a loss of normal skeletal muscle atonia during REM sleep. Both idiopathic RBD and secondary RBD are highly related to synucleinopathy including Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy. Almost all idiopathic RBD patients will develop synucleinopathy after a few years. Therefore, RBD may be an early marker in the progression of…

Breathing abnormalities during sleep in Kleine-Levin Syndrome: fact or coincidence?

PURPOSE OF REVIEW: To highlight and critically discuss the possibility that breathing abnormalities during hypersomnolent episodes are an additional clinical feature of Kleine-Levin Syndrome (KLS). RECENT FINDINGS: A computerized literature search of breathing abnormalities in patients with KLS disclosed two old and a single recent publication describing a total of six patients with sleep apnea during hypersomnolent episodes. SUMMARY: Owing to scarce reports on complete polysomnography during and in-between hypersomnic episodes of KLS, the possibility that breathing abnormalities are an addi…

Obstructive sleep apnoea and arthrogryposis.

Arthrogryposis is a rare condition characterised by multiple congenital joint contractures. We present a case of a 10-year-old child with arthrogryposis and snoring. Polysomnography revealed significant obstructive sleep apnoea and hypoventilation that improved but did not completely resolve with adenotonsillectomy. With continuous positive airway pressure (CPAP) therapy, there was full resolution of all sleep disordered breathing. Initially, the patient admitted to difficulty tolerating nasal CPAP at home. However, she steadily improved adherence to therapy and admitted that with nasal CPAP u…

[Revision of obstructive sleep apnea syndrome in the child].

The obstructive sleep apnea syndrome (OSAS) is frequent in infancy and childhood. It is caused by a prolonged upper respiratory airway obstructioon during sleep, and adenotonsillar hypertrophy is the most important cause. OSAS may have an impact on physical and cognitive development, but syntoms in children are subtle and may pass unrecognised. Polysomnography is the gold standard technique for OSAS diagnosis and surgical approach with adenotonsillectomy is the most frequently treatment indicated. Early diagnosis and treatment and adequate follow up are important to prevent physical disturbanc…

[Obstructive sleep apnoea syndrome].

Sleep apnoea syndrome is a frequent disease, occurring in men aged more than 30 years and in postmenopaused women. Its prevalence is more than 1% in men and between 0,3%-0,5% in women. It is an important public health problem. Diagnosis is made by polysomnography, which is considered as a gold standard, or by polygraphy. Sleep apnoea syndrome must be looked for by clinicians in presence of symptoms such as sleepiness, snoring, or behavioural problems, because these patients have a high risk not only of driving crashes, but also of arterial hypertension, brain strokes, and myocardial infarction…

How to help patients with restless legs syndrome. Discerning the indescribable and relaxing the restless.

Restless legs syndrome is a common, potentially disabling condition that affects about 10% to 15% of the general population and yet is often unrecognized and misdiagnosed. It is mainly diagnosed clinically and only rarely requires polysomnography. The condition is usually primary and treatable. First, however, secondary causes should be sought, especially iron deficiency and peripheral neuropathy, because when the source is an accompanying factor or condition, the syndrome may be curable. The most effective drugs are dopaminergic agents, clonazepam, opioids, gabapentin, and clonidine. Addition…

Nocturnal tongue biting beyond epilepsy: sleep-related facio-mandibular myoclonus -a diagnostic and therapeutic challenge.

We present a case of sleep-related facio-mandibular myoclonus (FMM) in a man in the third decade of life with recurrent tongue-biting episodes during sleep onset. Video-polysomnography and video-electroencephalogram revealed myoclonic jaw jerks without epileptiform activity. The episodes were associated with stress and improved with clonazepam and non-pharmacological interventions. This case highlights sleep-related FMM as a rare, under-recognised cause of sleep-related tongue biting, distinct from epilepsy and bruxism. Early diagnosis through neurophysiological studies is crucial to prevent m…

Airway obstruction in two children with congenital central hypoventilation syndrome and review of the literature.

Congenital central hypoventilation syndrome (CCHS) is an autosomal dominant disease that is caused by heterozygous mutations in the paired-like homeobox 2B gene (PHOX2B). Madani et al. described an abnormally high degree of not only central apnea but also obstructive and mixed apnea in Phox2b27Ala/+newborn mice. Newborns with CCHS must undergo polysomnography for obstructive respiratory events in order to guide the optimal ventilation strategy if oxygen desaturation, bradycardia, and malaise persist under noninvasive ventilation. Newborns and infants with CCHS must be systematically tested for…

Functional (Psychogenic) Movement Disorders Presenting During Sleep.

BACKGROUND: Functional (psychogenic) movement disorders are involuntary movements that seems to originate from activation of voluntary motor pathways in the brain. The movements typically present during the waking hours with variable frequency. CASE PRESENTATION: We present the case of a 24-year-old woman with FMDs during the waking state, but also during stages 1 and 2 of non-REM sleep and REM sleep, recorded with polysomnography. Such movements caused arousal leading to excessive daytime sleepiness and fatigue. CONCLUSIONS: FMDs may disrupt sleep causing day time somnolence, adding morbidi…

Combined Orthodontic and Surgical Treatment in a 8-Years-Old Patient Affected By Severe Obstructive Sleep Apnea: A Case-Report.

An eight-years-old girl showed a restless sleep with snoring and severe apnea episodes, a mandibular retrognathia, mouth breathing, maxillary transverse discrepancy, mandibular transverse discrepancy, moderate crowding and anterior open-bite. The CBCT showed an anterior collapse of the epiglottis. The treatment consisted in a rapid palatal expansion, an epiglottoplasty and a reduction of the tongue base. Polysomnography revealed that apnea-hypopnea index improved from 21,8 episodes/hr at the baseline to 0,6 episodes/hr, average oxygen saturation from 96,5% to 98,1%, oxygen desaturation events …

Polysomnographic characterization of pergolide-induced sleep attacks in idiopathic PD.

Both dopamine agonists and levodopa may induce episodes termed "sleep attacks" in patients with PD. These episodes are well detailed behaviorally, but little is known about their neurophysiologic characterization. The authors performed a 24-hour polysomnography (PSG) in a PD patient taking pergolide in combination with levodopa, in which four of these diurnal sleep episodes occurred. PSG findings were followed up after pergolide withdrawal. Sleep episodes shared with narcolepsy both behavioral and EEG findings. However, pergolide partly restored a more physiologic sleep architecture, which was…

Brainstem evoked potentials in adult sleep apnea.

Brainstem evoked potentials (BSEP) were recorded in 23 patients with adult sleep apnea (ASA). Three patients were studied with all-night polysomnography prior to our testing. They were categorized as having obstructive, central, or mixed sleep apnea depending on the predominant sleep findings. All patients with central sleep apnea had abnormal BSEP with prolongation of wave V. A majority of the remaining patients with obstructive sleep apnea and mixed sleep apnea had abnormal BSEP, but without specific configurations. These findings substantiate our hypothesis that brainstem dysfunction may pl…

Diagnostic potential of biomarkers in the assessment of pediatric obstructive sleep apnea-A systematic review and meta- analysis.

UNLABELLED: PURPOSE  : Periodic hypopnea or apnea during sleep, along with episodes of hypoxia and sleep fragmentation caused by frequent awakenings to restore the upper airway are the characteristics of Pediatric obstructive sleep apnea (P-OSA). While extensive literature evaluates biomarkers in the diagnosis of OSA in adults, no systematic review summaries their diagnostic accuracy in P-OSA. This systematic review evaluates the diagnostic accuracy of biomarkers in assessing the P-OSA compared to Polysomnography in children. METHOD: The research question was established in PECO format. The s…

Assessment of sleep quality in patients with orofacial pain and headache complaints: A polysomnographic study.

BACKGROUND: Sleep is a physiological function essential for survival, recovery, tissue repair, memory consolidation, and brain function. Pain is also an indispensable aspect of human life. The coexistence of sleep disorders and pain is often described in the literature, yet it is critical to define sleep not only subjectively but also using objective instrumental methods, such as polysomnography, that provide data on sleep quality. OBJECTIVES: The aim of the study was to determine the relationship between orofacial pain (OFP), headache (HA) and sleep quality using subjective and objective sle…
