Sex-Based Differences in Association Between Coverage Disruptions and Preventive Care.

Adults with part-year or year-round uninsurance are less likely to receive recommended preventive services. Sex differences in preventive care use have been documented, but it remains unclear whether sex moderates the association between insurance coverage disruptions and preventive care use. The current study analyzed data from 37,659 adults aged 18-64 in the 2019 and 2023 National Health Interview Survey. Insurance coverage over the past 12 months was categorized as continuous private, continuous public, coverage gap, or continuously uninsured. Outcomes included blood pressure and cholestero

Community Paramedics Address Health Inequities Within EMS Through Mobile Integrated Healthcare (MIH) Program Referrals from a "Medical Home".

To advance local health equity, a large urban fire department's Mobile Integrated Healthcare (MIH) Unit worked with a Chicago-based health care organization to refer patients in "medical homes" to a MIH program. Medical home coordinators referred individuals to Community Paramedics (CPs) in the MIH Unit for program enrollment with a 4-visit model in 4 clinical pathways (hypertension, diabetes, asthma/Chronic Obstructive Pulmonary Disease [COPD], and heart failure). A total of 911 utilization of ambulance transportation, emergency department (ED) visits, and inpatient hospitalizations before an

Assessing the Impact of Housing on 30-Day ED Readmission: A Retrospective Analysis of Hospital Discharges in Maryland.

Despite being an important social determinant of health (SDOH), independent association of housing with emergency department (ED) utilization is not well characterized. This study evaluated whether a previously validated ZIP code-level multidimensional housing index is associated with 30-day ED readmission in Maryland. We conducted a retrospective analysis of Maryland ED visits from 2019 to 2023, linked to the American Community Survey housing estimates. The primary outcome was 30-day ED readmission. The variable of interest was a validated ZIP code-level housing index reflecting housing value

Pediatric Care Coordination and Oral Health Outcomes in the United States: A Cross-Sectional Analysis of 2022-23 National Survey of Children's Health.

The association between pediatric care coordination (PCC) and the prevalence of oral health problems (OHP) among children and youths in the United States was investigated. Using data from the 2022 to 23 National Survey of Children's Health for participants ≤17 years, multivariable Poisson regression models were employed to estimate the association between effective PCC and OHP (the presence of ≥1 parent-reported case of dental caries, gum bleeding, or dental pain). The study estimated the population attributable risk (PAR) to quantify the proportion of potentially avoidable OHP with effective

A New Approach to Identifying Avoidable Emergency Department Visits Among High-Need High-Cost Medicaid Patients.

Patients with high need and high costs (HNHCs) are frequent users of emergency departments (EDs), and some of their visits may be avoidable with better access to primary care. A better understanding of the reasons for ED use of HNHC patients can inform cost containment and quality improvement efforts. Our goal was to identify reasons for ED visits based on categories of services received by HNHC patients during the visit and to examine the likely role of different categories of primary care providers (PCPs) in reducing potentially preventable visits. We used the 2018 California Medicaid enroll

Medical Respite as a Transitional Care Intervention to Improve Health and Housing Outcomes for Women Experiencing Homelessness.

People experiencing homelessness have high medical complexity and are at increased risk for poor post-acute outcomes after hospital discharge, yet evidence on medical respite programs for women is limited. We conducted a retrospective evaluation of a 10-bed medical respite program serving women experiencing homelessness in Northeast Ohio. Records of all women referred between March 2022 and May 2024 were reviewed to assess program implementation and resident outcomes, including health care utilization, health and health management improvement, and housing transitions at discharge. Among 226 re

Single and Dual Sensory Loss and Cardiovascular Disease in US adults: Findings from the 2021 Behavioral Risk Factor Surveillance System.

Prior work has shown that persons with disabilities have higher rates of poor cardiovascular outcomes than persons without disabilities. Little research has evaluated this relationship in persons with single and dual sensory loss. This study aimed to examine the association between single and dual sensory loss and cardiovascular disease (CVD). Data were obtained from 2021 Behavioral Risk Factor Surveillance System, a representative household survey of 234,394 noninstitutionalized civilian adults. The independent variable was sensory loss (neither, vision loss only, hearing loss only, both visi

Optimizing Care Management Targeting: A Machine Learning Comparison of Regression, Classification, and Ranking Approaches for Identifying Future High-Cost Patients.

Effective care management programs depend on accurately identifying patients who are likely to account for a disproportionate share of future health care expenditures. Unlike traditional risk-adjustment models, which emphasize population-level predictive accuracy, care management targeting is fundamentally a top-k decision problem focused on maximizing expenditure capture. This study used 2000-2023 MEPS data to compare regression-, classification-, and learning-to-rank machine learning approaches for predicting next-year health care expenditures. Primary outcomes included precision-at-k, cost

Development of Digital Algorithms for the Early Identification of Rare Clinical Outcomes Using Electronic Medical Records: A Case Study of Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a painful, progressive, and disfiguring rare inflammatory skin condition with significant diagnostic delays due to limited awareness. This study aims to develop and validate digital algorithms using integrated electronic medical records (ieMR) to identify undiagnosed HS patients. A test cohort of 121 HS cases (patients who visited a dermatologist-led HS clinic and received treatment) and 187,106 controls (emergency department patients) were identified from patients attending a Queensland tertiary hospital from 2018 to 2022. Using demographics, structured ieMR d

Budget Impact Analysis of Ferric Derisomaltose for Heart Failure in a Rapidly Aging Regional Population.

Heart failure with reduced ejection fraction (HFrEF) poses a significant clinical and economic burden in Malaysia, particularly in Perak. Iron deficiency (ID) in HFrEF is associated with increased hospitalization risk. While international guidelines recommend intravenous ferric derisomaltose (FDI), its economic viability within Perak's public hospitals remains undefined. This study estimated the 3-year budget impact of introducing FDI therapy for HFrEF patients with ID in Perak public hospitals from the Malaysian Ministry of Health perspective. An Excel-based budget impact model compared a "Wo

Knowledge, Symptom Awareness, and Referral Practices of Primary Care Providers in Early-Onset Colorectal Cancer.

Colorectal cancer (CRC) incidence and death rates are increasing among people under age 50 in the United States. In addition to promoting on-time screening for average-risk adults starting at age 45, CRC signs and symptoms may need timely assessment by a health care provider to rule out other conditions and identify a diagnosis. The objective of this study was to examine primary care providers' (PCPs') awareness of CRC-related symptoms and their diagnostic approaches for patients under age 50 using the 2023 DocStyles panel survey. This survey included 1490 PCPs practicing in the United States.

Cervical Cancer and Sexually Transmitted Infections Screening Among Individuals with a Substance Use Disorder: A Retrospective Cohort Study.

Substance use disorders (SUDs) have been associated with barriers to preventive medicine screening, but little is known about the association of SUDs and cervical cancer and sexually transmitted infections (STIs) screening rates. This retrospective cohort study used de-identified medical records to select women aged 31-59 as of January 1, 2019. Logistic regression models were computed to estimate the association between any SUDs, including alcohol, compared to no SUD diagnoses, and odds of cervical cancer and STIs screening during the observation period (2019-2024) before and after controlling

Cost-Effectiveness of Screening and Early Diagnosis Strategies for Cancer in the General Adult Population in Low- and Middle-Income Countries: A Systematic Review.

Cancer is one of the leading causes of morbidity and mortality worldwide, disproportionately affecting low- and middle-income countries (LMICs) due to barriers to obtaining access to health care services and screening programs. The current study aimed to synthesize evidence on cost-effective strategies for cancer screening and early diagnosis in adults (≥18 years) in LMICs. The literature search was conducted in PubMed, the Cochrane Library, Embase, EconLit, CINAHL, LILACS, Global Health, and the Web of Science Core Collection databases. Patient Engagement with General Bulk Outreach: Impact of Primary Care Provider vs. Care Team Signature in General Bulk Patient Outreach.

Bulk messaging is an important population health tool used to engage patients in preventive care and chronic disease management, yet little is known about optimal formatting of the communication. One factor to consider is whether the signatory of the patient-facing communication is the patient's own Primary Care Provider (PCP) or a generic care team signature. In this quasi-randomized, non-blinded study we compared identical generic bulk outreach messages directed toward patients with an upcoming appointment and invited them to self-schedule a Medicare Annual wellness visit prior to their sche

Disparities in Recombinant Zoster Vaccine Coverage in the United States.

Recombinant zoster vaccine (RZV) was recommended in 2020 in the US for adults aged ≥50 years. This retrospective study used cross-sectional data from 2020 National Health Interview Survey respondents aged ≥50 years. Sample weights were applied to allow results to be representative of the non-institutionalized US population. Associations between respondent characteristics and coverage were explored using logistic regression. An estimated 14.1% of US adults aged ≥50 years had received ≥1 dose of RZV by 2020. Coverage varied by race and ethnicity (6.3% Hispanic, 6.9% Black/African American, 13.7%

Identifying Patients with Heart Failure Eligible for Guideline-Directed Medical Therapy.

A majority of patients with heart failure (HF) do not receive adequate medical therapy as recommended by clinical guidelines. One major obstacle encountered by population health management (PHM) programs to improve medication usage is the substantial burden placed on clinical staff who must manually sift through electronic health records (EHRs) to ascertain patients' eligibility for the guidelines. As a potential solution, the study team developed a rule-based system (RBS) that automatically parses the EHR for identifying patients with HF who may be eligible for guideline-directed therapy. The…

The Social Risk ACTIONS Framework: Characterizing Responses to Social Risks by Health Care Delivery Organizations.

Social risks refer to individuals' social and economic conditions shaped by underlying social determinants of health. Health care delivery organizations increasingly screen patients for social risks given their potential impact on health outcomes. However, it can be challenging to meaningfully address patients' needs. Existing frameworks do not comprehensively describe and classify ways in which health care delivery organizations can address social risks after screening. Addressing this gap, the authors developed the Social Risk ACTIONS framework (Actionability Characteristics To Inform Organi

Comprehensive Approach to Opioid Management in a Primary Care Network.

In response to the opioid epidemic, the Centers for Disease Control and Prevention released best practice recommendations for prescribing, yet adoption of these guidelines has been fragmented and frequently met with uncertainty by both patients and providers. This study aims to describe the development and implementation of a comprehensive approach to improving opioid stewardship in a large network of primary care providers. The authors developed a 3-tier approach to opioid management: (1) establishment and implementation of best practices for prescribing opioids, (2) development of a weaning

Trends and Patterns of Social History Data Collection Within an Electronic Health Record.

There is increased acceptance that social and behavioral determinants of health (SBDH) impact health outcomes, but electronic health records (EHRs) are not always set up to capture the full range of SBDH variables in a systematic manner. The purpose of this study was to explore rates and trends of social history (SH) data collection-1 element of SBDH-in a structured portion of an EHR within a large academic integrated delivery system. EHR data for individuals with at least 1 visit in 2017 were included in this study. Completeness rates were calculated for how often SBDH variable was assessed a

Addressing Maternal Health Disparities: Building a Novel Two-Generation Approach to Comprehensive Postpartum Care.

The United States is facing a maternal health crisis with increasing rates of severe maternal morbidity and mortality. To improve maternal health and promote health equity, the authors developed a novel 2-generation model of postpartum and pediatric care. This article describes the Two-Generation Clinic (Two-Gen) and model of care. The model combines a dyadic strategy for simultaneous maternal and pediatric care with the collaborative care model in which seamless primary and behavioral health care are delivered to address the physical health, behavioral health, and social service needs of fami
