INTRODUCTION: The growing number of adolescents and young adults (AYAs) living with chronic conditions highlights the need for effective healthcare transition programmes (TPs) to support the transfer from paediatric to adult care. Despite existing recommendations, many AYAs experience inadequate preparation, resulting in disrupted care and adverse health outcomes. The COCCOS project developed a co-designed TP to address these challenges. This study presents the protocol for an evaluation of this programme.
METHODS AND ANALYSIS: A multicentric quasi-experimental pretest-posttest study will be

AIMS: Immigrants in western countries face an increased risk of developing diabetes and have been shown to receive lower quality of diabetes care. However, it is uncertain whether this disparity in care persists when comparing immigrants and non-immigrants with similar levels of marginalization with respect to the social determinants of health.
METHODS: Using population-based healthcare administrative data linked to immigration and neighbourhood census data, we conducted a retrospective cohort study of all people aged ≥ 40 years with diabetes in Ontario, Canada on 1 April 2019. Process measur

Background: Quality indicators (QIs) are used to measure and evaluate quality of care. However, QIs to evaluate care for HF patients in general practice in Belgium are lacking. Therefore, this study aimed to determine which QIs, rooted in the electronic health record (EHR), are useful to monitor quality of care for patients with HF in general practice. Methods: The RAND/UCLA appropriateness method (a modified Delphi method) was used to define these assigned QIs. First, a literature review was performed to generate a list of possible QIs for HF. Second, by applying the SMART principle, 25 QIs w

With the increases in complexity of care for breast health concerns, there is a growing need for efficient and effective clinical evaluation, especially for vulnerable populations at risk for poor outcomes. The Breast Health Center at Boston Medical Center is a multidisciplinary program, with internists providing care alongside breast surgeons, radiologists, and patient navigators. Using a triage system previously shown to have high provider and patient satisfaction, and the ability to provide timely care, patients are assigned to either a breast surgeon or internist. From 2007 to 2009, intern

OBJECTIVES: High-quality care for intensive care unit patients and families includes palliative care. To promote performance improvement, the Agency for Healthcare Research and Quality's National Quality Measures Clearinghouse identified nine evidence-based processes of intensive care unit palliative care (Care and Communication Bundle) that are measured through review of medical record documentation. We conducted this study to examine how frequently the Care and Communication Bundle processes were performed in diverse intensive care units and to understand patient factors that are associated

BACKGROUND: Remote methods may help older adults improve their physical function, but understanding the perspectives of experts who deliver this care is critical. This modified Delphi process aimed to develop expert consensus on effectiveness, feasibility and implementation of remote assessment and management of physical function in older adults.
METHODS: This online Delphi process included experts involved in supporting older adults to maintain or improve their health. The Round 1 survey presented 33 Delphi statements assessed using an 11-point Likert scale (0-10; ≥7 indicated agreement). St

The WHO concept of Healthy Ageing (ie, the process of developing and maintaining the functional ability that enables well-being in older age) has initiated a global discussion about the need for shifting paradigms to reorient health and social services towards person-centred and coordinated models of care. In particular, the integration of health and social care services is critical to provide the basis for comprehensive information sharing and service delivery to support the evolution of the older person over time. The capability to monitor and respond to an older person's changing health and

BACKGROUND: We previously demonstrated improvement in bronchopulmonary dysplasia and nosocomial infection among preterm infants at 12 neonatal units using the Evidence-based Practice for Improving Quality (EPIQ). In the current study, we assessed the association of Canada-wide implementation of EPIQ with mortality and morbidity among preterm infants less than 29 weeks gestational age.
METHODS: This prospective cohort study included 6026 infants admitted to 25 Canadian units between 2008 and 2012 (baseline year, n = 1422; year 1, n = 1611; year 2, n = 1508; year 3, n = 1485). Following a 1-yea

In 2006, the national Select Panel on Preconception Care published a set of 10 recommendations on how to improve preconception health and healthcare in the United States. Since then, CDC has been engaged in efforts to ensure that those recommendations are implemented. To help translate the national recommendations into action at the local level, CDC funded CityMatCH, a national maternal and child health organization representing urban health departments, to coordinate a practice collaborative. Beginning in October 2006, multidisciplinary teams from Hartford, Connecticut, Nashville, Tennessee,

Approximately one third of hospitalized older adults develop a new disability by discharge, which places them at risk for readmission, institutionalization, and death. Various risk factors, both modifiable and nonmodifiable, coalesce in the acute care setting. As frontline health care providers, nurses are crucial to the process of altering modifiable risk factors by assessing patients’ risk for functional decline during hospitalization using a standardized instrument and treating risk with evidence-based interventions. Barriers to meeting this goal must be overcome, such as a lack of evidence

Children who join families through the process of adoption often have multiple health care needs. After placement in an adoptive home, it is essential that these children have a timely comprehensive health evaluation. This evaluation should include a review of all available medical records and a complete physical examination. Evaluation should also include diagnostic testing based on the findings from the history and physical examination as well as the risks presented by the child's previous living conditions. Age-appropriate screens should be performed, including, for example, newborn screeni

Heart failure (HF) is the major contributor to cardiovascular morbidity and mortality. Given its rising prevalence, the costs of HF care can be expected to increase. Multidisciplinary management of HF can improve quality of care and survival. However, specialized HF programmes are not widely available in most European countries. These circumstances underlie the suggestion of the Heart Failure Association (HFA) of the European Society of Cardiology (ESC) for the development of quality of care centres (QCCs). These are defined as health care institutions that provide multidisciplinary HF managem

CONTEXT: The integration of peer support by accompanying peers (APs) into oncology care settings enhances the patient's experience through the contribution of their experiential knowledge. APs play a key role in providing emotional support, active listening, information, and shared lived experiences. However, little is known about how different clinical settings-such as a genetic risk assessment and oncological treatment-influence how these contributions are expressed and perceived.
OBJECTIVE: This study assessed whether the perceived contributions of accompanying peers (APs) varied across th

BACKGROUND: We assessed the impact of an evidence-based neuroprotection care bundle on the risk of brain injury in extremely preterm infants.
METHODS: We implemented a neuroprotection care bundle consisting of a combination of neuroprotection interventions such as minimal handling, midline head position, deferred cord clamping, and protocolization of hemodynamic and respiratory managements. These interventions targeted risk factors for acute brain injury in extremely preterm infants (born at gestational age less than 29 weeks) during the first three days of birth. Implementation occurred in a
