Admiral nursing and the family care of a parent with dementia: using autobiographical narrative as grounding for negotiated clinical practice and decision-making.

BACKGROUND: Providing family care at home for a person with a long-established diagnosis of dementia has been recognized for some time as a multifaceted and complex phenomenon where care roles alter during the course of the condition. Presently, the nursing response to meeting such need is currently poorly defined and documented in the literature and depends more upon descriptive illustrations of practice efficacy rather than any evaluated system of knowledge and skills transfer or framework utilization. AIM: The aim of the study was to capture an autobiographical account of a family carer's

The construction of identity during group therapy among adults with traumatic brain injury.

This investigation examined how the interpretive practices of a speech-language pathologist (SLP) contributed to the construction of identity among adults with traumatic brain injury (TBI) during group therapy in a hospital setting. Six group therapy sessions were video-recorded and transcribed (yielding a total of 8,056 utterances). Attention was paid to patient expressions of identity and ability, the communicative functions of these expressions, and the manner in which these expressions were evaluated by the SLP. The SLP was also interviewed regarding the nature of therapy practice. Analysi

Meleis's theory of nursing transitions and relatives' experiences of nursing home entry.

AIM: This paper explores the extent to which Meleis's mid-range theory of nursing transitions is supported by the findings of a study exploring relatives' experiences of the move to a nursing home. BACKGROUND: Mid-range nursing theories are useful tools in helping to understand the scope of nursing practice in a range of contexts and situations. However, as yet, many formal mid-range theories have not been adequately tested. METHODS: Findings from a constructivist study of relatives' experiences of nursing home entry were re-analysed in relation to the extent to which they reflected the doma

[The right of the client in geriatric psychiatry to a personal, individual world view. How can we, as caregivers, learn to share this?].

"Validation" after Naomi Feil is the name of a method which is used and recognized the world over as an appreciative way of dealing with the highly aged. Validation is based on universally valid humanist principles. A person is viewed as an individual being having a good reason for his current behavior. This is true for "still oriented" as well as for "non-oriented" persons. It is necessary to recognize and categorize the different behavioral patterns in the various phases of disorientation. Here we speak of the physical and psychological characteristics of the four phases of disorientation. T

Qualitative study of young people's and parents' beliefs about childhood asthma.

BACKGROUND: Asthma continues to be a common childhood chronic illness managed principally in primary care. Self-management requires co-ordinated efforts of young people, carers and health professionals. Non-compliance occurs even when parents are supervising care, suggesting that decisions are made on the basis of beliefs that contrast with professional advice. Health professionals therefore need to understand the views of parents (or other carers) and patients to promote good self-management. Little attention has been given to carers' and young people's perspectives on asthma. AIM: To gain i

Assisting medical students to conduct empathic conversations with patients from a sexual medicine clinic.

OBJECTIVE: To describe an exercise, the structure and content of which assists medical students to conduct potentially embarrassing conversations concerning sexual health which require expression of empathy, to integrate previous learning, to identify their further learning needs, and to develop and test strategies to meet these needs. METHOD: Students' role play, sequentially, speakers (patients) and listeners (clinical students) in a "carousel," in which all students are engaged at all times in a sequence of pairings which change at rapid intervals. Half way through the exercise, students r

Psychological versus pharmacological treatments of bulimia nervosa: predictors and processes of change.

This article extends the acute outcome findings from a study comparing psychological and pharmacological interventions for bulimia nervosa (B.T. Walsh et al., 1997) by examining 3 additional domains: predictive factors, therapeutic alliance, and time course of change. One hundred twenty women were randomized to cognitive-behavioral therapy (CBT), supportive psychotherapy (SPT) plus antidepressant medication or a placebo, or a medication-alone condition. Results indicate that high baseline frequencies of binge eating and vomiting, as well as a positive history of substance abuse or dependence,

The influence of postoperative telephone calls on pain perception: a study of 118 periodontal surgical procedures.

This age-matched and sex-matched study examined the influence of postoperative telephone calls on pain perception and on the number of analgesics used for pain relief. Adult periodontitis subjects (n = 118) received periodontal surgery after examination and sanative therapy (scaling, root planing, and removal of local irritants). All subjects received similar care, postoperative instructions, and medication, except 59 subjects were phoned 24 hours postoperative (PC group), and 59 were not (NC group). Callers covered 10 points and were reassuring and positive about surgical outcomes. One week p

The role of pharmacists in HIV/STD prevention: evaluation of an STD syndromic management intervention in Nepal.

OBJECTIVE: To evaluate the effects of a pilot sexually transmitted disease (STD) syndromic case management training for pharmacists in Nepal on STD drug dispensing behavior, HIV/STD prevention communication, and condom promotion and sales. METHODS: Pre- and post-intervention interviews with samples of 160 pharmacists were conducted utilizing the simulated client method to collect data on pharmacists' response to men reporting urethral discharge. In order to assess the long-term effects, a sub-sample of 38 pharmacists trained 7-9 months before the follow-up survey was analysed separately. RES

On what basis hope? Modern progress and postmodern possibilities.

Modern and postmodern versions of hope as they apply to services for persons labeled as having mental retardation were examined. Proponents of modernism construct hope as relying on an ever-improving science to accurately comprehend mental retardation and other disabilities and the effectiveness of professional interventions. This myth of scientific progress is traced in various forms through American intellectual history to the development of special education as interventionist social science. Advocates of postmodernism cast doubt upon the grand narrative of modernism and critique modern soc

Staffing and staff performance in services for people with severe or profound learning disability and serious challenging behaviour.

Eighteen people with severe or profound learning disability and very serious challenging behaviour were tracked for four and a half years. Thirteen subjects moved to staffed houses in the community. A multiple time-series design and direct observation were used to evaluate whether there was any change in staffing and staff contact. The houses had higher staff ratios and proportionately even higher levels of assistance and other contact with clients, who showed significant improvement in engagement in meaningful activity without overall increase in major problem behaviour.

The Therapist Evaluation Inventory: evidence for an unidimensional scale.

The internal structure of the Therapist Evaluation Inventory (TEI) was investigated with data gathered from therapy sessions conducted by 16 individual psychotherapists with 292 psychiatrically disturbed adolescent inpatients. A principal components analysis (Varimax rotation) indicated a single dimension that accounted for 70.39% of scale variance. The findings did not corroborate a factor structure consistent with the a priori scales of the TEI.

Interpersonal behavior and therapeutic progress: therapists and clients rate themselves and each other.

To measure the extent to which progress in psychotherapy can be predicted from the interpersonal styles of therapists and clients, 42 outpatients and their 11 therapists rated themselves and each other on Benjamin's Structural Analysis of Social Behavior (SASB) scales. In addition, therapists rated amount of client change and therapy success-failure, based on the outcome section of the Rogers and Dymond (1954) scale, and clients used the Strupp, Fox, and Lessler (1969) questionnaire to evaluate progress-to-date. Clients also completed the Hopkins Symptom Checklist (HSCL) at the start of therap

The end phase of analysis: indicators, functions, and tasks of termination.

This paper considers the end phase of analysis. Beginning with a brief review of the literature on termination, specifically the indicators for initiating the termination process, we identify the structural attainments necessary for the patient to successfully complete the analysis and to maintain smooth post-analytic functioning. We stress in this regard the significance of self-analytic functions and the relative immutability of the transference neurosis. These points are illustrated with clinical examples. Our paper concludes with a discussion of the tasks and contributions of the analyst d

Analysability in relation to early psychopathology.

Analysability is a highly important subject, both in the treatment of patients and in selecting candidates for analytical training. The assessment of analysability can be studied from two complementary points of view: the dynamic (with the data obtained in the preliminary interviews) and the genetic (with data obtained from early childhood by means of psychoanalytical treatment). The purpose of this study will be to contribute to the study of analysability from the genetic point of view. When one goes deeper into the analysis of a patient there may be doubts as to the limits and possibilities

Empowering Collaboration With Patients: Healthcare Staff's Perspectives on Conditions for Shared Decision-Making in Swedish Psychiatric Care.

RATIONALE: Shared decision-making (SDM) is advocated in psychiatric care to facilitate patients' engagement in treatment discussions about their own care. However, applying SDM in practice involves more than simply referring to an advocated method. It is therefore interesting to study healthcare professionals' perceptions and experiences of the conditions necessary for patients to truly participate in SDM. AIM: To explore healthcare staff's experiences of patient involvement in psychiatric care, paying particular attention to perceived prerequisites, barriers and strategies relevant to shared

Usability and feasibility of ADappt: a digital toolkit to support communication on diagnosis and prognosis in memory clinics.

BACKGROUND: ADappt is a digital toolkit for both memory clinic professionals and patients to support communication on diagnosis and prognosis in memory clinics. We aimed to evaluate ADappt's usability and feasibility in clinical practice. METHODS: In this mixed-methods study, we first assessed usability via think-aloud sessions with ten memory clinic professionals from eight memory clinics, six patients, and one care partner. Think-aloud comments were deductively categorized into: content, navigation, and design. Second, we conducted a feasibility study in four memory clinics. Eight memory cl

The relationship between trust in primary healthcare providers among patients with diabetes and levels of depression and anxiety.

BACKGROUND: Trust in healthcare providers has been shown to improve several clinical and patient-reported outcomes. However, its relationship with depression and anxiety has not been investigated among patients with chronic health conditions, such as diabetes. Therefore, the aim of this study was to examine whether trust in primary care physicians among patients with diabetes is associated with their levels of depression and/or anxiety. METHOD: Adult patients (≥18 years) with a diagnosis of diabetes, confirmed through their electronic health records, were recruited and interviewed from the pr

Negotiating Control of HIV: A Qualitative Analysis of HIV-Positive Latino MSM in North Carolina.

The majority of HIV disease management happens outside of clinical encounters. As such, understanding how HIV fits into the lives of patients is key to improving HIV outcomes. The concept of control is a vital part of the social context within which health is managed. This qualitative study explores how a sample of Latino men who have sex with men living with HIV in North Carolina (n = 16) conceptualize and negotiate control in the context of living with HIV. We found that participants use a range of strategies to exert control over their lives including: understanding their infection, engagin
