FDG PET/CT in infection and inflammation--current and emerging clinical applications.

Integrated positron emission tomography/computed tomography (PET/CT) with the glucose analogue, 2-[(18)F]-fluoro-2-deoxy-d-glucose (FDG), is an evolving hybrid imaging technique in the evaluation of an important and diverse group of pathological conditions, which are characterised by infection and aseptic inflammation. With a rapidly expanding body of evidence, it is being increasingly recognised that, in addition to its established role in oncological imaging, FDG PET/CT also has clinical utility in suspected infection and inflammation. The technique can identify the source of infection or in

Optimizing Infection Prevention in Primary Shoulder Arthroplasty: Evidence-Based Strategies and Best Practices.

» Based on the available evidence, there are numerous strategies that may reduce infection risk in primary shoulder arthroplasty. Preoperatively, optimization of iron deficiency anemia and smoking cessation are associated with lower rates of periprosthetic joint infection and perioperative complications. In addition, risk of infection may be mitigated by deferring shoulder arthroplasty for at least three months following a corticosteroid injection. Management of biologic and targeted immunosuppressive therapies should be coordinated with medical specialists and tailored to the specific agent's

Challenges and strategies in the treatment of periprosthetic joint infection caused by multidrug-resistant Gram-negative bacteria: a narrative review.

BACKGROUND: Multidrug-resistant Gram-negative bacteria (MDR-GNB) are an increasing cause of periprosthetic joint infection (PJI), limit the available antibiotic options and affect patient outcomes. OBJECTIVES: We reviewed the therapeutic strategies for managing PJI caused by MDR-GNB, including surgical and antibiotic options. SOURCES: We performed a search regarding PJI caused by MDR-GNB without date restrictions, including experimental and clinical studies. CONTENT: Surgery plays a central role in the therapeutic approach to PJI caused by MDR-GNB. Although prosthesis removal provides highe

In Vitro Efficacy of Phage Therapy Against Common Biofilm-forming Pathogens in Orthopedics and Trauma Surgery.

Formation of biofilms by bacteria is a major challenge in a clinical setting. The importance of these biofilms increases in specialties where foreign bodies and prosthetic material are used. Orthopaedics is such a speciality and phage therapy could offer additional therapeutic options when dealing with biofilm infections. We conducted a systematic literature review using the PubMed database. We searched for phage activity against biofilms of the most common pathogens found in orthopaedics. The results of the systematic review were broken down into different categories and discussed accordingly.

Dual Surgical Setup May Improve Infection Control Rate of Debridement and Implant Retention Procedures for Periprosthetic Infections of the Hip and Knee.

BACKGROUND: Debridement, antibiotics, and implant retention (DAIR) is an appealing treatment option for periprosthetic joint infection (PJI) due to its low cost and low morbidity. There are many nonmodifiable risk factors for DAIR failure that have previously been established. A dual DAIR setup constitutes establishing a new, sterile field after the initial debridement. The purpose of this study is to determine whether the modifiable surgical technique of a dual setup improves the infection control rate following PJI. METHODS: A retrospective study was conducted from January 1, 2000 to Decemb

Antimicrobial characteristics of Berberine against prosthetic joint infection-related Staphylococcus aureus of different multi-locus sequence types.

BACKGROUND: Staphylococcal aureus (S. aureus) has become the leading causative pathogen of Prosthetic Joint Infection (PJI), which is the most devastating complication after arthroplasty surgeries. Due to the biofilm formation ability and emergence of multiple-drugs resistance strains of S. aureus, it has become an urgency to find new anti-staphylococcal agents to establish effective prophylaxis and treatment strategy for PJI. Extracted from a traditional Chinese herb, berberine is proved active in inhibiting S. aureus, while whether it exerts the same effect on PJI-related S. aureus remains u

Evaluation of Bacteriophage Anti-Biofilm Activity for Potential Control of Orthopedic Implant-Related Infections Caused by Staphylococcus aureus.

BACKGROUND: Despite significant advancements in surgical protocols and biomaterials for orthopedics, peri-prosthetic joint infection (PJI) remains a leading cause of implant failure. Staphylococcus aureus nasal colonization is an established risk factor for PJI, with methicillin-sensitive S. aureus a leading cause of orthopedic implant-related infections. The purpose of these in vitro studies was to investigate the antibacterial activity of a tailored bacteriophage cocktail against planktonic and biofilm-associated S. aureus. METHODS: The S. aureus strains (n = 30) were screened for their sus

Ceftriaxone susceptibility of oxacillin-susceptible Staphylococcus aureus from patients with prosthetic joint infection.

Ceftriaxone is used to treat oxacillin-susceptible S. aureus (OSSA) prosthetic joint infection (PJI). Susceptibility of ceftriaxone against OSSA has been questioned. Ceftriaxone susceptibility was determined against 100 PJI OSSA isolates. Ceftriaxone MIC90/MIC50 were 8/4 and 4/3μg/mL by broth microdilution and Etest, respectively. Ceftriaxone susceptibility is inferable by oxacillin susceptibility.

[Acute total knee replacement infection after a cat bite and scratch: a clinical case and review of the literature].

In the last 15 years only few cases of Pasteurella multocida (P. multocida) total knee arthroplasty infection have been published, mostly related to cat or dog bites or scratches. We report a case of P. multocida total knee arthroplasty infection in a 64-year- old patient, 10 days after being scratched and bitten by his cat. The patient was successfully treated with debridement and tibial interspacer exchange and antibiotic treatment for 6 weeks. Antimicrobial prophylaxis should be considered in cat or dog bites or scratches victims with prosthetic joints.

Hematogenous infection of total hip arthroplasty with Actinomyces following a noninvasive dental procedure.

This article describes a case of an infected total hip arthroplasty following a dental procedure. A 59-year-old man underwent total hip arthroplasty for osteoarthritis and had a routine recovery. Approximately 9 months postoperatively, he underwent a dental cleaning without antibiotic prophylaxis. One month later, he reported gradually worsening right hip pain and a purulent discharge. After several unsuccessful interventions, the patient was referred to the authors' facility. The patient's history, draining sinus tract, and radiographic changes were considered diagnostic of a late chronic inf

Late bacterial endocarditis of an amplatzer atrial septal defect occluder device.

A 66-year-old man with a large secundum atrial septal defect (ASD) that had been repaired percutaneously 30 months previously with an Amplatzer ASD occluder (ASO) presented with fevers and Staphylococcus aureus bacteremia. Transesophageal echocardiography revealed a 1.5 x 1.5 cm mobile mass on the left atrial side of his ASO consistent with a vegetation. When the ASO was explanted, the left atrial side of the device was poorly endothelialized. In conclusion, the present report is the first description of late infective endocarditis in an adult with an ASO.

Contemporary management of cardiovascular implantable electronic device infections.

Cardiovascular implantable electronic device (CIED) implantation rate has substantially risen in the foregoing decades. Unfortunately, this upsurge in CIED implantation rate has been accompanied by a disproportionate rise in the rate of CIED infections. Device infection is a major complication of CIED implantation, necessitating removal of an infected device followed by systemic antimicrobial therapy and reimplantation of a new system. In this article, we review the current epidemiology, risk factors, diagnostic strategy and contemporary management of CIED infection. In addition, we address th

Contact lens considerations for patients infected with HIV.

BACKGROUND: As an eye care practitioner, one must be aware of the many sequelae of HIV disease and its impact on contact lens patients. With the progressing development of anti-HIV medications, patients infected with HIV are living longer and more productive lives. Thus a greater number of patients infected with HIV may be candidates for contact lens wear. PURPOSE: The eye care practitioner will play an important role in caring for patients infected with HIV who wear contact lenses. Theoretically, contact lens wear in patients infected with HIV could potentially increase their risk of corneal

Current osteomyelitis mouse models, a systematic review.

Osteomyelitis is an inflammatory bone disease caused by an infecting microorganism leading to a gradual bone loss. Due to the difficulty in studying osteomyelitis directly in patients, animal models allow researchers to investigate the pathogenesis of the infection and the development of novel prophylactic, anti-inflammatory and antimicrobial treatment strategies. This review is specifically focused on the in vivo mouse osteomyelitis studies available in literature. Thus, a systematic search on Web of Science and PubMed was conducted using the query "(infection) AND (mice OR mouse OR murine) A

Precision of a phage susceptibility spot assay assessed with 154 clinical Staphylococcus aureus isolates.

Lytic phages are a promising consideration to combat recalcitrant bacterial infections. To advance the possibility of using phage therapy in the clinical routine, standardized phage susceptibility testing methods are needed. Here, staphylococcal phage K was tested against 154 periprosthetic joint infection-associated Staphylococcus aureus isolates using a double overlay spot assay in technical and biological triplicates. Individual plaques were assessed using a morphology scoring system ranging from 0 (no plaques) to 4 (large, clear plaques), with scores ≥2 considered indicative of infectivity

Antimicrobial Pocket Flush and Cardiac Implantable Electronic Device Infection: Meta-Analysis of Real-World Studies.

INTRODUCTION: Clinical guidelines do not recommend the routine rinsing of cardiac implantable electronic devices (CIED) with antimicrobials before pocket closure("antimicrobial pocket flush.") However, despite current recommendations and a lack of evidence regarding the utility of this practice for reducing CIED infections, the antimicrobial pocket flush is still commonly employed. Clarifying the true value of this technique is important for supporting evidence-based practice and avoiding unnecessary antimicrobial use. Therefore, we performed meta-analysis of observational studies to determine

From Failure to Success: Lithotripsy Transforms Lead Extraction in a Same-Patient Case.

INTRODUCTION: Cardiac implantable electronic device infections often necessitate extraction, which can be complicated by calcified adhesions, leading to increased procedural risk and reduced efficacy. Given these challenges, intravascular lithotripsy (IVL) has been proposed as a tool for lead extraction to aid in overcoming calcification. This report presents the first reported case where shockwave IVL was successfully employed to extract a severely calcified lead system after an initial extraction attempt failed due to extensive calcification. CLINICAL BACKGROUND: A 58-year-old woman with a
