Q fever outbreak in the terraced vineyards of Lavaux, Switzerland.

Coxiella burnetii infection (Q fever) is a widespread zoonosis with low endemicity in Switzerland, therefore no mandatory public report was required. A cluster of initially ten human cases of acute Q fever infections characterized by prolonged fever, asthenia and mild hepatitis occurred in 2012 in the terraced vineyard of Lavaux. Epidemiological investigations based on patients' interviews and veterinary investigations included environmental sampling as well as Coxiella-specific serological assay and molecular examinations (real-time PCR in vaginal secretions) of suspected sheep. These investi…

The first reported cases of Q fever endocarditis in Thailand.

We describe the first two reported cases of Q fever endocarditis in Thailand. Both patients were male, had pre-existing heart valve damage and had contact with cattle. Heightened awareness of Q fever could improve diagnosis and case management and stimulate efforts to identify risk factors and preventive measures.

Role of innate and adaptive immunity in the control of Q fever.

Acute Q fever is commonly resolved without an antibiotic regimen, but a primary infection may develop into a chronic infection in a minority of cases. Coxiella burnetii, the causative agent of Q fever, is known to infect macrophages both in vitro and in vivo. It has been observed that the intracellular survival of C. burnetii requires the subversion of the microbicidal properties of macrophages. Adaptive immunity is also essential to cure C. burnetii infection, as demonstrated by clinical studies and animal models. Indeed, the control of infection in patients with primary Q fever involves a sy…

Q Fever: an old but still a poorly understood disease.

Q fever is a bacterial infection affecting mainly the lungs, liver, and heart. It is found around the world and is caused by the bacteria Coxiella burnetii. The bacteria affects sheep, goats, cattle, dogs, cats, birds, rodents, and ticks. Infected animals shed this bacteria in birth products, feces, milk, and urine. Humans usually get Q fever by breathing in contaminated droplets released by infected animals and drinking raw milk. People at highest risk for this infection are farmers, laboratory workers, sheep and dairy workers, and veterinarians. Chronic Q fever develops in people who have be…

The first serologic study of Q fever in sheep in Iran.

Coxiella burnetii is an obligate intracellular microorganism that causes Q fever in humans and animals. In ewes, C. burnetii infections are generally asymptomatic, but they can lead to abortions, stillbirths, and delivery of weak and unviable lambs. Serological assays are suitable for screening herds. Enzyme-linked immunosorbent assays (ELISA) technique has a high sensitivity and a good specificity. The aim of this study was to investigate the presence of anti-C. burnetii antibodies among sheep in southeast Iran. A total of 85 serum samples were collected from ten sheep flocks from April to Se…

[Three years of Q fever in the Netherlands: faster diagnosis].

OBJECTIVE: To assess if more rapid diagnosis and treatment is possible and to assess if this could be improved, since the first outbreak of Q fever in 2007. DESIGN: Retrospective study of secondary data. METHODS: Analysis of surveillance data regarding Q fever over the period 2007 to 2009 and additional information on some patients from 2007 and 2008 obtained from general practitioners. RESULTS: Diagnostic delay fell sharply between 2007 and 2009 and to a lesser extent, so did therapeutic delay from 2007 to 2008. In high incidence areas, diagnosis and treatment was faster with a lower propo…

Markers of infection in inpatients and outpatients with acute Q-fever.

BACKGROUND: Query-fever (Q-fever) is a zoonotic infection caused by the intracellular Gram-negative coccobacillus Coxiella burnetii. A large ongoing outbreak of Q-fever has been reported in the Netherlands. We studied various markers of infection in inpatients (hospitalised) and outpatients (treated by a general physician) with acute Q-fever in relation to disease severity. METHODS: Leukocyte counts, C-reactive protein (CRP) and procalcitonin (PCT) concentrations were measured in 25 inpatients and 40 outpatients upon presentation with acute Q-fever. Chest X-rays, if available, were analysed a

[An outbreak of Q fever in The Netherlands--possible link to goats].

In 2007, 73 cases of Q fever were identified through reports and retrospective analyses; the affected region extended from Tilburg in the southwest to Arnhem in the northeast. The infections arose in late spring, particularly in May and June. Several spontaneous abortions due to Q fever occurred on 4 dairy goat farms in the same region. The national incidence of spontaneous abortion due to Q fever was 6 cases in 2006 and 7 in 2007. Climatically, this southern region was extraordinarily dry during April 2007. All pregnant women from a small region with the highest incidence in northeast North B…

Q fever endocarditis: a case report and review of the literature.

The case of a 31-year-old man from Alberta diagnosed with Q fever endocarditis is presented. To the authors' knowledge, this is the first case of Q fever endocarditis diagnosed in the province of Alberta. The patient had undergone open valvulotomy for congenital aortic stenosis as an infant. He presented with congestive heart failure secondary to severe aortic regurgitation and underwent mechanical aortic valve replacement. Early failure of the mechanical prosthesis and numerous laboratory abnormalities prompted an investigation for endocarditis, which was initially negative. Markedly positive…

Zoonoses are animal and human diseases. Q fever is primarily a zoonosis-an animal disease that can be transmitted to humans under certain conditions. Recent epidemiological studies suggest that Q fever should be considered as a public health problem in many countries where it is present, but unrecognizable due to inadequate disease controls. Through specific…

[Acalculous cholecystitis: an uncommon form of presentation of Q fever].

Acute acalculous cholecystitis is a very rare clinical presentation of Q fever. We report the case of a 38-year-old man who presented with fever associated with elevation of liver enzyme levels and thickening of the gallbladder wall on abdominal ultrasonography and who was initially diagnosed with acute acalculous cholecystitis. Due to the persistence of fever and transaminase elevation despite antibiotic treatment, a liver biopsy was performed. Characteristic "doughnut" epithelioid granulomas were observed, suggesting a diagnosis of granulomatous hepatitis caused by Q fever, which was confirm…

Probabilistic evaluation of aspirating doses of a Q fever pathogen.

The process of biological aerosol penetration into respiratory organs is connected with the estimation of the amount of an aspiration dose of microorganisms (D) a human gets in the center of infection. Here we submit a probabilistic model describing the process of hits of a Q fever pathogen in a human respiratory tract. This approach makes it possible to get qualitatively different probabilistic estimations of doses received and corresponding values of critical time intervals of exposure at which the amount accumulated in the respiratory tract turns into an infecting dose. This permits one to …

Q-Fever nowadays presents the most diffuse disease in the world, caused by the microorganisms from the family Risckettiacae. This disease is Coxiellae burnetii. The laboratory diagnosis of the Q-Fever can be stated either by the isolation of the causer from the patient material, either by the proving of the specifically antibodies. The serologic diagnostics presents the choile method in Q-Fever. The aim of this work is to illustrate the results of the detection of the serum in patients with the clinical symptoms at the infeçtion Coxiellom burnetii. [Q Fever in pregnancy: a case report and review of the literature].

Endemic occurrence of Q fever among persons in close contact with domestic animals is well known in some rural regions of Germany. The prevalence of antibodies indicating acute Q fever in pregnancy reported in the literature varies between 0.2 % and 4.7 % of the screened population. Q fever in pregnancy initially manifests as placentitis and often leads to premature birth (30 %), growth restriction (46 %), spontaneous abortion (22 %) or fetal death in utero (7 %). Some impairment of pregnancy is observed in over 70 % of cases with seroconversion during pregnancy. Thus Q fever serology should b…
