Retinal artery and vein thrombotic occlusion during pregnancy: markers for familial thrombophilia and adverse pregnancy outcomes.

BACKGROUND: Ocular vascular occlusion (OVO), first diagnosed during or immediately after giving birth, often reflects superposition of the physiologic thrombophilia of pregnancy on previously undiagnosed underlying familial or acquired thrombophilia associated with spontaneous abortion, eclampsia, or maternal thrombosis. SPECIFIC AIM: We describe OVO, first diagnosed during pregnancy or immediately postpartum, in three young females (ages 32, 35, 40) associated with previously undiagnosed familial thrombophilia. RESULTS: Branch retinal artery occlusion (BRAO) occurred at 9 and 13 weeks gesta…

Central retinal and posterior ciliary artery occlusion after intralesional injection of sclerosant to glabellar subcutaneous hemangioma.

The aim of this study is to describe vision loss caused by central retinal artery and posterior ciliary artery occlusion as a consequence of sclerotherapy with a polidocanol injection to a glabellar hemangioma. An 18-year-old man underwent direct injection with a 23-gauge needle of 1 mL of a polidocanol-carbon dioxide emulsion into the glabellar subcutaneous hemangioma under ultrasound visualization of the needle tip by radiologists. He developed lid swelling the next day, and 3 days later at referral, the visual acuity in the left eye was no light perception. Funduscopy revealed central retin…

Primary antiphospholipid syndrome presenting with a branch retinal artery occlusion in a 15-year-old boy.

Acute vascular events are rare in the pediatric population, but there is an association with the presence of antiphospholipid antibodies. When there is no other underlying medical disorder, this is referred to as primary antiphospholipid syndrome. We present a case of a 15-year-old boy who developed an acute superior branch retinal artery occlusion. Complete evaluation revealed significant elevations in antiphospholipid antibodies. To our knowledge, there are no cases in children of primary antiphospholipid syndrome presenting with this clinical manifestation.

A fluorescein angiographic study of branch retinal artery occlusion (BRAO) - the retrograde filling of occluded vessels.

BACKGROUND: For assessing the prognosis of a branch retinal artery occlusion (BRAO), examination of the arterial blood flow by fluorescein angiography is necessary. PATIENTS AND METHODS: In seven patients (mean age: 68.1, youngest 61, oldest 76 years old), with BRAO of varying involvement and extent, the disturbed retinal blood flow was demonstrated by this method. All the patients were subjected to Doppler sonography of the carotid arteries and all had a general medical examination. RESULTS: The most impressive sign was the retrograde filling of the retinal arterial and/or venous branches f…

Therapeutic decisions involving disparate clinical outcomes: patient preference survey for treatment of central retinal artery occlusion.

BACKGROUND: Major therapeutic decisions are made by patients with information and guidance provided by their physicians. The values patients place on different outcomes and the risks they are willing to accept are important factors in making these decisions. New beneficial therapies associated with potentially serious complications are now available for some blinding diseases. The authors aim to determine the maximum amount of risk of stroke and death persons would accept to recover vision. METHODS: Standardized survey of adults with normal vision. RESULTS: Thirty-nine percent and 37% of sur

Central retinal artery occlusion after scoliosis surgery with a horseshoe headrest. Case report and literature review.

Central retinal artery occlusion is a well-documented entity that occurs after trauma and embolic, thrombotic, or spasmodic episodes in both children and adults. Its occurrence as a complication of elective surgery is very rare but quite tragic. This report describes a case of central retinal artery occlusion occurring in a child after scoliosis surgery in which a horseshoe headrest was used. Recommendations are given on how to avoid this serious complication.

Retinal artery tortuosity in ex-premature infants. 18-year follow-up on eyes of premature infants.

Ophthalmic examinations have been performed on 52 expremature infants who had reached their late teens. These patients all weighed 1500 g or less at birth, and were born during a period of transition from liberal to restricted oxygen therapy for premature infants. Fourteen patients had some degree of retrolental fibrosis. In 4 cases there was complete, bilateral, destructive retrolental fibroplasia (RLF) with microphthalmos; and in 3 cases in which RLF was complete unilaterally, the other eye showed retinal scarring with traction of the disc in 2 patients and extreme tortuosity of the retinal

Dramatic recovery of vision in patient with acute central retinal artery occlusion treated with local intra-arterial tissue plasminogen activator.

Central retinal artery occlusion (CRAO) is a devastating disease resulting in severe visual loss in most cases. It has no proven treatment, and a variety of management options used clinically all have dismal rate of success. We report outcome of treatment with local intra-arterial thrombolysis administered 2.75 hours after devastating visual loss from incomplete CRAO.

The Efficacy and Safety of Intravenous Prostaglandin E1 in the Management of Retinal Artery Occlusion: A Systematic Review.

PURPOSE: To evaluate the efficacy and safety of prostaglandin E1 (PGE1) for the management of central/branch retinal artery occlusion (CRAO/BRAO). METHODS: Our protocol was registered prospectively on PROSPERO (CRD42024522961). We searched 4 electronic databases [PubMed, Scopus, Web of Science, and Google Scholar] to retrieve all studies reported using PGE1 for patients with CRAO/BRAO. We conducted a Wilcoxon signed-rank test to assess the effect of PGE1 on the visual acuity (VA) of included subjects. VA change was utilized as a measure to assess the degree of VA improvement. We assessed the …

Nationwide Incidence and Trends in Central Retinal Arterial Occlusion Management: A 5000-Patient Analysis.

Central retinal artery occlusion (CRAO) is a rare and visually debilitating vascular condition characterized by sudden and severe vision loss. CRAO is a compelling target for intravenous alteplase (tPA) and endovascular mechanical thrombectomy (MT) due to pathophysiological similarities with acute ischemic stroke; however, the utility of these interventions in CRAO remains dubious due to limited sample sizes and potential risks. To assess usage and outcomes of tPA and MT in CRAO, we queried the National Inpatient Sample database using International Classification of Disease, Ninth and Tenth ed…

Central retinal vein and artery occlusion associated with sildenafil: a case report and review of the literature.

BACKGROUND: Sildenafil is a selective phosphodiesterase type 5 inhibitor used for the treatment of erectile dysfunction and pulmonary hypertension. It is available over the counter in many countries. While there have been a few reports of retinal vascular occlusion following sildenafil consumption, most cases have other comorbidities as risk factors for the disease, and the exact causal role of this drug in these conditions remains unclear. CASE PRESENTATION: We present the case of a healthy 32-year-old Iranian man who developed combined central retinal vein occlusion and retinal artery occlu…
