Usefulness of laparo- or retroperitoneoscopic biopsy for retroperitoneal lymph node swelling of unknown origin.

Laparo- or retroperitoneoscopic biopsy was performed for five male patients with retroperitoneal lymph node swelling of unknown origin. Previous core needle biopsies were useless or inapplicable in all patients. A laparoscopic approach was used for two patients, with a retroperitoneoscopic approach for the remaining three patients. Sufficient specimens for not only pathological diagnosis but also further examinations such as immunophenotyping or DNA analysis were safely obtained. Convalescence was satisfactory and all patients were treated appropriately according to the resulting diagnosis. Th

Infarcted ectopic spleen presenting as retroperitoneal mass.

Wandering or ectopic spleen is an exceptionally rare condition. Because of the abnormally long pedicle, an ectopic spleen is prone to torsion and infarction. Patients can present with abdominal pain or an acute abdomen. The incidence is over a wide age range, but it occurs with a female predominance in the third to fifth decades. We present a case in a 26-year-old female of an infarcted ectopic spleen, initially thought to be a retroperitoneal mass on computed tomography scan, but found at surgery in the lower abdomen. The minimally invasive gynecologic surgeon needs to be aware of this entity

Endometriosis presenting like a psoas abscess.

Endometriosis is the presence of ectopic endometrial tissue that responds to hormonal stimulation. Although endometriosis occurs most often in the pelvis, several unusual sites have been reported. We present a case of endometriosis mimicking, a psoas abscess. A 39-year-old lady presented with pain around the right hip, of 5 years duration. Computed tomography CT scan showed a multi-loculated mass in the right iliopsoas muscle. On exploration, a hemorrhagic cystic mass was seen pressing on the femoral nerve. The lesion was excised and histological examination of the tissue showed features of en

Retroperitoneal bronchogenic cyst mimicking pancreatic cystic lesion.

Retroperitoneal bronchogenic cyst is detected extremely rarely and often masquerades as other diseases. Here, we report 2 cases of retroperitoneal bronchogenic cyst mimicking pancreatic mucinous tumor. Histologically, both cysts were composed of ciliated respiratory-like epithelium with abundant mucin content, smooth muscle bundles and mature cartilage, compatible with the diagnosis of retroperitoneal bronchogenic cyst. In addition to these 2 cases, another 42 retroperitoneal bronchogenic cysts reported in the English literature were collected for review and analysis. Twelve (28%) were located

Gastro-intestinal symptoms as clinical manifestation of peritoneal and retroperitoneal spread of an invasive lobular breast cancer: report of a case and review of the literature.

BACKGROUND: Distant spread from breast cancer is commonly found in bones, lungs, liver and central nervous system. Metastatic involvement of peritoneum and retroperitoneum is unusual and unexpected. CASE PRESENTATION: We report the case of a 67 year-old-woman who presented with gastrointestinal symptoms which revealed to be the clinical manifestations of peritoneal and retroperitoneal metastatic spread of an invasive lobular breast cancer diagnosed 15 years before. CONCLUSION: To the best of our knowledge, the case presented is the third one reported in literature showing a wide peritoneal a

Compression of kidneys in Erdheim-Chester disease of retroperitoneum: Open surgical approach.

Erdheim-Chester disease is a rare systemic non-Langerhans cell histiocytosis. We report the first case of surgical treatment of severe compression of renal parenchyma by retroperitoneal masses in a 61-year-old male patient with progressing renal failure. After 3 years of follow-up, we have concluded that the open surgical approach is an option in the management of renal complications in Erdheim-Chester disease.

[Retroperitoneal laparoscopic treatment of parapelvic renal cysts: report of 5 cases].

OBJECTIVES: Renal cysts of the sinuses or parapelvic cysts constitute a rare form of renal cyst, usually associated with low back pain. Symptomatic forms may be treated by percutaneous aspiration, but the treatment of these cysts has changed considerably over recent years due to the proximity of the renal hilum. Percutaneous treatment is now contraindicated due to its high complication rate, and open surgery is an invasive technique. Laparoscopy appears to be the most appropriate and most effective solution. PATIENTS AND METHODS: The authors report five cases of parapelvic renal cysts all pre

"Primary" benign retroperitoneal and intraspinal dumbbell-shaped cystic teratoma: case report.

STUDY DESIGN: A case of unusual dumbbell-shaped cystic teratoma is presented. OBJECTIVES: To investigate and describe an unusual case of retroperitoneal cystic teratoma with spinal invasion in an adult. The image findings, minimally invasive surgical procedures, and clinical outcome were discussed, and the literature was reviewed. SUMMARY OF BACKGROUND DATA: Primary benign cystic teratomas of the retroperitoneum are rare. Extension of these lesions into the spinal canal is more rare. In this report, by presenting a case of cystic teratoma with such extension, the origin of the tumor, and the

Evolution of retroperitoneal lymphadenectomy (RPLND) in the management of non-seminomatous testicular cancer (NSGCT).

The metastatic lymphatic drainage of testis cancer to the retroperitoneum was noted clinically about a century ago. Beginning with extraperitoneal approaches, RPLND was attempted. The first cure after RPLND of node positive disease was in 1905 by Cuneo in Paris. Transperitoneal approaches failed due to infection until post World War II experience at Walter Reed Army Hospital. Thoracoabdominal approaches became popular several decades later. But Improved exposure and vascular management strategies led to increased usage of the transabdominal approach once again. The advent of platinum based com

[Retroperitoneal hematoma in children].

INTRODUCTION: Retroperitoneal hematoma is frequently due lo blunt trauma and is a challenging problem. Incidence of this complication in adults is high, but in childhood is uncommon. MATERIALS AND METHODS: Analysis of 30 cases is presented. PTS was found between 10 and 12 points, zone II, 12, zone III, 17, and zone I, only one. We observed 33 associated injuries: 17 were pelvic fractures and nine, ureteral rupture. Three patients underwent hepatic or splenic laceration. Seventy-three percent of patients had exploratory laparotomy, but did not have exploration of retroperitoneum. CONCLUSIONS:

Retroperitoneoscopic renal pedicle lymphatic disconnection for chyluria.

OBJECTIVE: To report our experiences in retroperitoneoscopic renal pedicle lymphatic stripping for chyluria. METHODS: Six cases of filarial chyluria were admitted to our hospital from November 2001 to June 2002. Of these cases, 4 were men and 2 women, with age ranging from 34 to 52 years (mean, 42 years). Diagnosis was made by using urine test for the presence of chyle and fat globule, cystoscopy, excretory urogram and retrograde pyelography. Chyluria was found on the left renal unit in 2 cases and on the right side in 4 cases. The technique of retroperitoneoscopic management of chyluria cons

[Uterus in a male: persistent Mullerian duct syndrome. Report of a case with embryological considerations].

We describe a case of persistent mullerian duct syndrome incidentally found in a 79-year-old man. It was characterized by the presence of a uterus-like retroperitoneal mass composed of an ovoidal corpus and rudimental portio with a central lumen. Histologically the mass was leiomyomatous in nature, closely resembling myometrium, while the central lumen was lined by a monolayer of ciliated cuboidal to columnar epithelium similar to embryo-fetal celomatic epithelium. Although this is a dysembryogenetic syndrome due to the lack of regression of the mullerian duct, it may be incidentally discovere

Successful surgical treatment for spontaneous retroperitoneal hematoma in polycythemia vera: report of a case.

Bleeding is a common complication of such myeloproliferative disorders as polycythemia vera, and it usually occurs in either the skin or mucosa. We experienced a case of a massive retroperitoneal hematoma with no history of trauma in association with polycythemia vera. A 68-year-old man was referred to our hospital as a case of acute abdomen, and was diagnosed as having a retroperitoneal hematoma and polycythemia vera based on radiological and hematological examinations. Because the mass effect of the hematoma caused a progressive deterioration of his respiratory condition, surgical drainage w

Isolated retroperitoneal intradiaphragmatic bronchogenic cyst. A case report.

Isolated retroperitoneal bronchogenic cysts are extremely rare. We report a case which was intradiaphragmatic intimately associated with the musculature of the left crus diaphragmatic and unconnected with any other structures. Ultrasound and computed tomography findings were consistent with a pancreatic or an adrenal mass. Pathology confirmed a bronchogenic cyst. The outcome is favourable and the overall prognosis is good.

[Nonoperative management of major blunt renal lacerations with urinary extravasation: report of two cases].

Two cases of major blunt renal trauma with urinary extravasation managed by conservative treatment are presented herein. The first case: A 13-year-old girl fell from a bicycle. A computed tomographic (CT) scan showed right renal laceration with retroperitoneal hematoma. Renal arteriography showed evidence of fresh arterial bleeding without visualization of the lower pole. She was treated conservatively with superselective arterial embolization of the lower pole. She developed no urinoma with normal renal function. Renal renin and blood pressures stayed within the normal range in follow up stud
