Joint analysis of panel count and interval-censored data using distribution-free frailty analysis.

We propose a joint analysis of recurrent and nonrecurrent event data subject to general types of interval censoring. The proposed analysis allows for general semiparametric models, including the Box-Cox transformation and inverse Box-Cox transformation models for the recurrent and nonrecurrent events, respectively. A frailty variable is used to account for the potential dependence between the recurrent and nonrecurrent event processes, while leaving the distribution of the frailty unspecified. We apply the pseudolikelihood for interval-censored recurrent event data, usually termed as panel cou

Inhomogeneity of capillaroscopic findings in systemic sclerosis.

BACKGROUND: Despite the great interest in capillaroscopy in systemic sclerosis (SSc), research on the possible combinations of different microvascular phenomena at different fingers in SSc patients have not been performed until now. OBJECTIVE: To assess the diversity of capillaroscopic findings in SSc. METHODS: The study includes analysis of the capillaroscopic findings in 40 SSc patients who were divided into the following categories: "scleroderma", type pattern - "early", "active", "late" phase, normal and/or nonspecific findings. The data were analyzed using descriptive statistics and t t

Differentiating morphea from lichen Sclerosus by using multiphoton microscopy combined with U-Net model for elastic fiber segmentation.

This paper describes a methodology to differentiate morphea from lichen sclerosus based on examination with multiphoton microscopy (MPM) composed of two-photon excited fluorescence (TPEF) and second harmonic generation (SHG). Subcellular-resolution images were acquired by MPM from unstained lesion tissues then process spectral analysis to quantify the TPEF and SHG signals. Moreover, U-Net was employed to segment elastic fiber in TPEF images to combine with collagen fiber in SHG images for precise fiber quantification. Predictions of segmentation showed excellent performance on several evaluati

Role of cardiopulmonary exercise test in early diagnosis of pulmonary hypertension in scleroderma patients.

INTRODUCTION: Pulmonary arterial hypertension (PAH) is a severe, high mortality and progressive disease. Early diagnosis and treatment improves the prognosis. Patients with scleroderma disease presents high risk of developing PAH. Established screening strategies - echocardiogram and DETECT algorithm - recognize the disease when it is already advanced. Cardiopulmonary exercise testing (CPET) detects pulmonary vascular injury in earlier stages. METHODS: Prospective study of 52 consecutive patients diagnosed of scleroderma in our health area, during 2 years (2018 and 2019). All of them undergo

Management Options for Linear Scleroderma ("En Coup de Sabre").

BACKGROUND: En coup de sabre (ECDS) is a form of linear-scleroderma, primarily affecting the face. Despite effectiveness of pharmacological interventions in the management of lesions, sequelae of cutaneous deformities are common. OBJECTIVE: Review the existing surgical tools for the correction of "en coup de sabre" ECDS lesions and facial deformities. MATERIALS AND METHODS: The authors conducted a literature search for reports on surgical interventions for ECDS deformations, in the electronic databases of PubMed, Scopus, and Cochrane Library Databases. RESULTS: Twenty-six publications repor

Dermoscopy, confocal microscopy and optical coherence tomography features of main inflammatory and autoimmune skin diseases: A systematic review.

BACKGROUND/OBJECTIVES: Non-invasive skin imaging features of main skin inflammatory and autoimmune diseases have been reported, although a comprehensive review of their correlation with histopathologic features is currently lacking. Therefore, the aim of this paper was to review the correlation of dermoscopic, reflectance confocal microscopy (RCM) and optical coherence tomography (OCT) criteria of main inflammatory and autoimmune skin diseases with their corresponding histopathologic criteria correlation. METHODS: Studies on human subjects affected by main inflammatory and autoimmune diseases

Digital ulcer debridement in systemic sclerosis: a systematic literature review.

Optimal wound care is an essential component in the management of systemic sclerosis (SSc) digital ulcers (DUs). DU debridement has been suggested to reduce ulcer-related pain and improve tissue healing. However, only a minority of rheumatologists perform DU debridement, and there is no standard of care/protocol. Our objectives were to (i) evaluate the current evidence for the use of debridement in DU management and (ii) assess whether there are any specific protocols. A systematic literature review was performed searching the PubMed database (between 01/01/1950-01/03/2019) in accordance with

The Scleroderma Hand: Manifestations of Disease and Approach to Management.

Scleroderma is a rare autoimmune connective tissue disorder that often affects the hands. Manifestations in the hands include calcium deposits within the soft tissues that cause pain and may ulcerate through the skin, digital ischemia resulting in chronic wounds and digital gangrene, and joint contracture. Because of the underlying disease, patients with scleroderma have poorly vascularized tissue and a deficient soft tissue envelope, which make surgery particularly challenging. However, when undertaken with care, surgical intervention is often the best option for addressing the disabling hand

Imatinib: a novel treatment approach for generalized morphea.

BACKGROUND: Morphea is a rare chronic inflammatory disease that involves skin and subcutaneous tissues, causing skin sclerosis. Generalized morphea is characterized by extensive cutaneous involvement and poor response to therapy. Although the pathophysiology for morphea is not completely understood, abnormal signaling through the platelet-derived growth factor and transforming growth factor beta axes seems to play a major role in the inflammatory response and sclerotic process. Imatinib, a tyrosine kinase inhibitor, interferes with both transforming growth factor beta and platelet-derived grow

Primary atrophic profound linear scleroderma. Report of three cases.

We present 3 unusual cases of deep linear, primary atrophic scleroderma, not preceded by inflammatory reaction and sclerosis, involving the subcutis and deeper tissues. These cases differ in the course and prognosis from typical profound scleroderma since they do not lead to disfiguration and crippling deformities. In contrast to the atrophies left after regression of morphea or linear scleroderma, they do not involve the dermis, which does not show discoloration or changes in texture. Infiltrates in the endomysium, involvement of deeper tissues and the progressive character of the disease arg

[What information can digital plethysmography provide in acrosyndromes?].

Digital plethysmography records digital pulsation and measures blood flow by occlusion and perfusion pressure. The pulsatile curve is modified in vasomotor disorders of the extremities, and is delayed and flattened in obstructive arteriopathy. In Raynaud's phenomenon, the basal flow (at 22 degrees C) decreases with clinical severity, and suppression of post-ischemic blood flow by cold seems to be an attribute of Raynaud's disease. Digital perfusion pressure is usually greater than or equal to 70% of the humeral systolic pressure. When it decreases under hot conditions, organic obstructive arte

[The viewpoint of the dermatologist toward white atrophy].

Milian's white atrophy (1929), linked by English authors with livedoid vasculitis and segmentary hyaline vasculitis, is "the microvascular expression of diseases of variable aetiologies". It can be isolated, without vasculitis (idiopathic or secondary to an inflammatory livedo); the variety with vasculitis, most often observed, stems from loco-regional causes (above all from post-phlebitic venous hypertension) but also from general causes (collagenoses, infections, carcinamatoses). The need for an attempt at a better approach to white atrophy is clear.

Fabrication of a flexible prosthesis for the edentulous scleroderma patient with microstomia.

The exercise prosthesis serves as an interim device to achieve an increased opening of the jaws. Once the patient is able to insert the existing denture or when an impression tray can be placed in the patient's mouth without extreme discomfort, its purpose will have been achieved; the dentures should be relined or remade. Some patients need only achieve slight increases in the maximal vertical oral opening to make prosthetic treatment possible (Fig. 6). While this technique is useful for the edentulous scleroderma patient, it may have application for individuals with limited mouth opening seco

[Differential diagnostic studies using the dermatoscope in selected diseases].

The Zeis dermatoscope can be used beyond its known applications for precise and detailed skin surface examinations of different types of dermatosis. Differential diagnostic problems are solved by comparative observation of efflorescence and efflorescence fields of similar appearance. The examination series presented, the results of which are discussed in detail, is concerned mainly with surface examinations of parapsoriasis en plaques, seborrhea en plaques and the clinically similar early forms of mycosis fungoides. Also mentioned are examinations of lichen sclerosus et atrophicus and circumsc
