The epidemiology of infant shaft fractures of femur or humerus by incidence, birth, accidents, and other causes.

BACKGROUND: The purpose of this population-based registry study was to analyze both birth-related femur and humerus shaft fractures and diagnosed later in infancy, as regards incidence, perinatal characteristics, other diagnoses, and reported accidents. METHODS: Children born in 1997-2014, diagnosed with a femur or humerus shaft fracture before age 1 year, were identified in the Swedish Health Registries. Rate of birth fractures were estimated by combining femur and humerus shaft fractures coded as birth-related with femur and humerus shaft fractures diagnosed during day 1-7 without registere

[Prosthesis replacement in periprosthetic humeral fractures].

OBJECTIVE: Stabilization of the humerus with preservation or restoration of the shoulder function. INDICATIONS: Always in the presence of a loose prosthesis. It may become necessary in conditions of poor bone stock and if osteosynthesis is not possible. CONTRAINDICATIONS: Noncompliant patients due to alcohol or drugs. Local infections. SURGICAL TECHNIQUE: The loose implant is removed using an extended anterior deltopectoral approach. After exploration of the fracture and extended soft tissue release, the glenoidal components are implanted with visualization and protection of the axillary ne

Fracture of the superior border of the scapula.

Twenty-four patients who sustained a fracture of the superior border of the scapula are reviewed. All were associated with a fracture of the coracoid process and other shoulder injuries. There were 19 long fracture lines and 5 short. The former and 3 of the latter were extensions of a coracoid fracture; in 2, there appeared to be an avulsion fracture by traction through the superior transverse scapular ligament resulting from the upward displacement of the coracoid.

Acute-on-Chronic Axillary Artery Thrombus After Reverse Total Shoulder Arthroplasty for Failed Proximal Humerus Open Reduction and Internal Fixation: A Case Report.

CASE: A 71-year-old woman presented with post-traumatic arthritis 11 months after open reduction and internal fixation for a left proximal humerus fracture (PHF) dislocation. After revision to reverse total shoulder arthroplasty (rTSA), the patient's left upper extremity was found to be avascular. An emergent thrombectomy was performed with restoration of arterial flow after removal of an acute-on-chronic axillary artery thrombus. CONCLUSION: Although rare, as rTSA becomes more common for management of PHF, incidence of associated vascular injuries is likely to rise. Screening methods and cli

Improved outcomes for proximal humerus fracture open reduction internal fixation augmented with a fibular allograft in elderly patients: a systematic review and meta-analysis.

BACKGROUND: To date, no meta-analysis has been performed on the efficacy of fibular allograft augmentation for the management of proximal humerus fractures. The purpose of this study was to evaluate the radiographic and clinical outcomes of proximal humerus fractures treated with a locking compression plate (LCP) with or without fibular allograft augmentation. METHODS: The Cochrane Database of Systematic Reviews, the Cochrane Register of Controlled Trials, PubMed, MEDLINE, Web of Science, and SCOPUS were queried in June of 2021 for literature comparing the radiographic and clinical outcomes f

Subtraction osteotomy of the humeral head in posterior shoulder dislocation: a case report.

Locked posterior dislocations of the shoulder with an impacted fracture of the humeral head and an articular surface defect greater than 35-40% are generally treated with a femoral head bone graft or prosthesis. We present a case in which a subtraction osteotomy with osteoclasia on the impacted zone was performed to try to make the articular surface of the humeral head congruent and continuous. With a 42-month follow-up, the clinical outcome, in terms of mobility and pain, was very good; X-rays show there was no avascular necrosis of the humeral head nor signs of articular arthrosis. The aim o

Neglected acromioclavicular joint injury in coracoid process fractures: a retrospective cohort study.

INTRODUCTION: Coracoid process fractures are frequently associated with injuries to the superior suspensory shoulder complex, including the acromioclavicular (AC) joint. However, concomitant AC joint injuries may be overlooked in the acute phase, potentially limiting treatment options. This study aimed to investigate the association between coracoid process fractures and SSSC injuries and to identify imaging-based predictors of concomitant AC joint injury. METHODS: A retrospective single-center cohort study including 60 patients with coracoid process fractures was conducted between February 2

Does Simultaneous Fixation of Both Distal Radius and Distal Ulnar Fractures Improve Outcomes? A Retrospective Cohort Study.

Introduction: Distal metaphyseal ulnar fractures are often found in conjunction with distal radius fractures. However, there is no consensus on optimal management. The purpose of this study was to determine whether simultaneous fixation of both distal radius and distal ulnar fractures would improve outcomes. Materials and Methods: Patients treated for distal radial fractures over a 4-year period at our trauma center were identified, and their medical records were analyzed. Twenty-three patients met the inclusion criteria for this study. All radius fractures were fixed using a volar locking pla

A 2-year experience, management and outcome of 200 clavicle fractures.

INTRODUCTION: Clavicle fractures can cause pain and functional impairment if not managed appropriately. This article evaluates the prevalence of clavicular fractures, estimates the number of cases requiring operative treatment, evaluates whether removal of implant is a frequent necessity and compares the final functional outcome of the operative and non-operative groups. PATIENTS AND METHODS: Between November 2005 and November 2007, patients with clavicular fractures were eligible for participation. Patients below 18 years of age and those with pathological fractures were excluded. Demographi

Acromioclavicular joint injuries.

Acromioclavicular joint injuries, including sprains and dislocations, are common shoulder problems treated by orthopaedic surgeons. The classification of acromioclavicular joint injuries comprises six grades, according to the degree of ligamentous disruption and displacement. Grades I and II are generally treated nonoperatively, whereas types IV, V, and VI are treated surgically. Treatment for grade III injuries remains controversial. Fractures of the distal aspect of the clavicle have been divided into three types. Type I fractures are stable and require brief immobilization. Type II fracture

Anterior interosseous nerve palsy as a result of prolonged shoulder immobilisation.

This is a rare case of an anterior interosseous nerve (AIN) palsy in a patient as a result of a prolonged period of shoulder immobilisation. The patient had an open reduction internal fixation of a midshaft clavicle fracture. They subsequently underwent removal of metal due to symptomatic prominence of the metal work. The patient was in a shoulder immobiliser for a period of 5 months in total. They developed progressive AIN palsy as a result of a positional compression due to prolonged wearing of a shoulder immobiliser. This resolved with conservative management and careful observation.

Arthroscopic Assisted Percutaneous Screw Fixation of a Postage Stamp Anterior Glenoid Fracture.

Bio-absorbable anchors are widely used for Bankart repair in shoulder instability surgery. Suture anchor placement for labral repair may give rise to osteolysis and/or create stress risers in the glenoid rim, which may be the underlying reasons for glenoid rim fracture with a fracture line passing through previous anchor placement sites, the so-called "postage stamp fracture". Intraarticular fractures of the glenoid have been treated via open reduction and internal fixation through an arthrotomy, which may lead to potential postoperative complications like infection, neurovascular injuries, jo

The pattern of the fracture and displacement of the fragments predict the outcome in proximal humeral fractures.

Our aim was to determine the effect of the initial pattern of fracture and the displacement of fragments on the outcome of proximal humeral fractures treated conservatively. We followed 93 consecutive patients prospectively for one year. Final movement and strength were compared with those of the contralateral side. The final American Shoulder and Elbow Society score and the Disabilities of Arm, Shoulder and Hand and Short-Form 36 questionnaires were compared with those provided by the patient on the day of the injury. Radiographs and CT scans with three-dimensional reconstruction were obtaine

Dynamic compression plating versus locked intramedullary nailing for humeral shaft fractures in adults.

BACKGROUND: Surgical fixation of fractures of the shaft of the humerus generally involves plating or nailing. It is unclear whether one method is more effective than the other. OBJECTIVES: To compare compression plating and locked intramedullary nailing for primary surgical fixation (surgical fixation of an acute fracture or early fixation following failure of conservative treatment) of humeral shaft fractures in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (February 2011), The Cochrane Library 2011, Issue 1, MEDLINE and EMBASE (b

Problems, complications, and reinterventions in 4893 onlay humeral lateralized reverse shoulder arthroplasties: a systematic review (part I-complications).

BACKGROUND: Several modifications to the original Grammont reverse shoulder arthroplasty (RSA) design have been proposed to prevent distinctive issues, such as both glenoid and humeral lateralization. The aim of this systematic review was to determine rates of problems, complications, reoperations, and revisions after onlay lateralized humeral stem RSA, hypothesizing that these are design related. METHODS: This systematic review was performed in accordance with the PRISMA statement guidelines. A literature search was conducted (01.01.2000-14.04.2020) using PubMed, Cochrane Reviews, Scopus, an

Functional and radiographic medium-term outcome evaluation of the Humerus Block, a minimally invasive operative technique for proximal humeral fractures.

BACKGROUND: Conservative treatment of severe displacement of proximal humeral fracture fragments yields bad functional results, but open operative techniques have a high risk of avascular necrosis of the humeral head. We performed a medium-term outcome evaluation of the Humerus Block (Synthes, Oberdorf, Switzerland), a minimally invasive technique used in selected patients with proximal humeral fractures, to investigate the functional and radiographic outcome. MATERIALS AND METHODS: Of 47 patients operated on with the Humerus Block, 34 with a minimum follow-up of 30 months and a mean follow-u

Delayed operative management of fractures of the scapula.

Between 1998 and 2007, 22 patients with fractures of the scapula had operative treatment more than three weeks after injury. The indications for operation included displaced intra-articular fractures, medialisation of the glenohumeral joint, angular deformity, or displaced double lesions of the superior shoulder suspensory complex. Radiological and functional outcomes were obtained for 16 of 22 patients. Disabilities of the Arm, Shoulder, Hand (DASH) and Short form-36 scores were collected for 14 patients who were operated on after March 2002. The mean delay from injury to surgery was 30 days
