Narrating kidney disease: the significance of sensation and time in the emplotment of patient experience.

Drawing on research conducted among patients in Ireland, this article examines the narrative constructions of chronic kidney failure and explores the ways in which patient narratives cross-cut and subvert modernist medical constructions of transplantation as a therapeutic outcome, an endgame, a "gift of life." In experience, patients dismantle this construction structure by emplotting their stories around the painful lack of an ending, ardently brought to bear by the lived realities of immunosuppressant drug therapy, the silent fears of graft rejection, and the isolation of recipiency. They ar

Responses of guinea pig pups during isolation in a novel environment may represent stress-induced sickness behaviors.

When guinea pig pups are isolated in a novel environment, they show an initial active phase of behavioral responsiveness characterized by vocalizations and locomotor activity. One earlier study found that after about an hour, pups began to exhibit a second, passive stage of responsiveness marked by a crouched stance, eye-closing, and extensive piloerection. The present experiments tested the hypothesis that the responses during the second, passive stage result from the isolation experience activating pathways underlying the acute phase response, i.e., that behaviors of the second stage represe

Efficacy and safety of fluoxetine in the treatment of patients with major depression after first myocardial infarction: findings from a double-blind, placebo-controlled trial.

OBJECTIVE: Depression and hostility are significant risk factors for mortality and morbidity after myocardial infarction (MI). Much research is still needed to identify effective ways to reduce emotional distress in patients with cardiovascular disease. This double-blind, placebo-controlled study investigated the efficacy and safety of the antidepressant fluoxetine in patients with depression after their first MI. METHODS: Fifty-four patients with major depression after MI were randomly assigned to receive a flexible-dose regimen of fluoxetine or placebo for the first 9 weeks of a double-blin

["Filtered normality": psychological causes and sequelae of specific retinal diseases].

Until now there is a lack of investigation in psychological aspects of retinopathy in patients of the middle ages. The few studies have been found, focused on an old-fashioned psychopathology. Medical treatment, f.e. laser therapy is only partially available, constant visual loss to blindness is possible. Life events, coping strategies, social support, and symptomatic distress of patients with presumed ocular histoplasmosis (group 1) and patients with retinopathy centralis serosa (group 2) were compared with another and with a comparison group. Group 1 scored higher on the Schedule of Recent E

Psychological morbidity in newly referred patients with cancer.

Over the last 10 yr, research has shown that although the majority of patients are able to cope with the stresses of cancer, a substantial number have difficulties which require some form of psychological help. This study reports on the levels of psychological distress in a heterogeneous group of 117 newly referred out-patients with cancer over a 6-month period. Each patient reported their levels of distress by completing two self-administered questionnaires (the GHQ-30 and HADS) on three separate occasions. A descriptive examination of the socio-demographic characteristics of the sample was a

Spatial learning impairment in mice infected with Legionella pneumophila or administered exogenous interleukin-1-beta.

The effect of interleukin-1 beta (IL 1 beta) on spatial learning was examined. In one experiment, C57BL/6 mice were given daily injections (100 ng/mouse) of recombinant murine IL1 beta prior to training on the Morris water maze. In another experiment, mice were infected with a sublethal dose of a gram-negative bacterium (Legionella pneumophila; Lp). Mice rendered ill by the infection were given either anti-IL1 beta antibodies (100 micrograms/mouse) or saline and then trained on the water maze. Results indicated that (1) exogenous IL1 beta blocked acquisition of spatial learning, (2) Lp infecti

Predictors of burden in spouse caregivers of individuals with Alzheimer's disease.

This study used the following model of distress: Distress = [Exposure to Stress + Vulnerability]/[Psychological and Social Resources]. The constructs in the model were operationalized as (a) distress in response to caregiver experiences (burden); (b) exposure to stress (care recipient functional impairment in activities of daily living [ADLs]); (c) vulnerability (caregiver health problems, anger, and anxiety); and (d) resources (coping, outlook on life, and social supports). Long-term burden (15-18 months after entry) was predicted by several baseline variables: burden, care recipient ADLs, vu

Psychosocial effects on carers of living with persons with dementia.

A survey of members of the Alzheimer's Disease and Related Disorders Society confirmed high rates of psychological morbidity, though not of depression, and social isolation in family carers of persons with dementia. Psychological morbidity in carers was associated with having an affected person at home, the carer being a spouse, demanding problem behaviours, poor physical health in the carer, social isolation, dissatisfaction with social supports, greater use of psychotropic medication, and a deteriorated marital relationship. Carers and patients had high rates of consultations with doctors an

Engel's "Psychogenic Pain and the Pain-Prone Patient:" a retrospective, controlled clinical study.

Engel's hypothesis of pain-prone patients having a distinct pattern of developmental psychosocial experiences was tested in a controlled design including four groups of 20 patients each: A) psychogenic pain, B) organic pain, C) psychogenic bodily symptoms, and D) organic disease. On admission an open-ended interview, including childhood experiences, was tape-recorded. Measures were taken to minimize observer bias. Patients in group A had, compared to the other groups, significantly increased prevalence of: "Parents, verbally and/or physically abusive of each other," "parents, abusive of the ch

[The structure and function of the semicircular canals].

This review discusses present-day concepts concerning the structure and function of semicircular canals of the vestibular apparatus. It is indicated that the data available today about the function of semicircular canals suggest that they should be considered as a system with distributed parameters and that particular attention should be given not only to canal-canal interaction but also to canal-otolith interaction. A special section is allowed for the discussion of an agreement between mathematical description of the behavior of semicircular canals and experimental data. This work is of theo

Biological mechanism of antidepressant effect of omega-3 fatty acids: how does fish oil act as a 'mind-body interface'?

The unsatisfactory results of monoamine-based antidepressant therapy and the high occurrence of somatic symptoms and physical illness in patients with depression imply that the serotonin hypothesis is insufficient to approach the aetiology of depression. Depressive disorders with somatic presentation are the most common form of depression. Somatization, the bodily symptoms without organic explanation, is similar to cytokine-induced sickness behaviour. Based on recent evidence, omega-3 polyunsaturated fatty acids (n-3 PUFAs, or n-3 fatty acids) are enlightening a promising path to discover the

Transmissibility of Trypanosoma brucei during its development in cattle.

Recent outbreaks of Trypanosoma brucei rhodesiense sleeping sickness in Soroti District of eastern Uganda have demonstrated the important role cattle can play as reservoirs of this parasite. To clarify the epidemiological importance of the cattle reservoir, experiments were conducted to determine the ease with which T. brucei is transmitted during the course of its development in Friesian cattle. The development of T. brucei in cattle is characterized by an acute phase with high levels of parasitaemia and a decline in PCV. The acute phase is followed by a chronic phase during which the PCV rem

Trypanosoma brucei: comparison of circulating strains in an endemic and an epidemic area of a sleeping sickness focus.

Human sleeping sickness in East Africa is characterized by periods of long-term endemicity interspersed with short-term epidemics. The factors generating these huge changes are largely uncharacterized but probably reflect complex interactions among socioeconomic factors, ecological factors, and the movement and diversity of trypanosome strains. To investigate the role of trypanosome strains in the generation of these epidemics, we addressed two important questions. (1) Are the trypanosome strains circulating within a focus the same during times of endemicity and during an epidemic? (2) How sta

Cytokines in central nervous system trypanosomiasis: cause, effect or both?

The late, or encephalitic, stage of human African trypanosomiasis (HAT), or sleeping sickness, is typified by a diffuse meningoencephalitis characterised neuropathologically by perivascular infiltration of inflammatory cells. While the cause of this neuroinflammatory reaction is not understood, there is evidence for the roles of pro-inflammatory cytokines such as IFN-gamma and TNF-alpha and counter-inflammatory cytokines such as IL-10, with the balance of these influencing disease outcome. Because of the practical difficulties of obtaining serial measurements in patients, it has proved difficu

[Neuroendocrine factors in pathogenesis of motion sickness and the protective effect of drugs].

The paper presents a generalization of many-year complex investigations performed at various departments of the Institute of Medico-Biological Problems (Moscow) and various laboratories and institutes of the Ministry of Public Health and the Russian Academy of Medical Sciences, devoted to the pathogenesis of motion disease (sea sickness, vestibulo vegetative syndrome, various forms of kinetosis). The main attention is given to interpretation of the mechanisms of therapeutic and prophylactic action of drugs used for the treatment of such disorders. In particular, the role of a hormonal componen

Lipocalin-2 is dispensable in inflammation-induced sickness and depression-like behavior.

RATIONALE: While the relationship between inflammation and depression is well-established, the molecular mechanisms mediating this relationship remain unclear. RNA sequencing analysis comparing brains of vehicle- and lipopolysaccharide-treated mice revealed LCN2 among the most dysregulated genes. As LCN2 is known to be an important regulator of the immune response to bacterial infection, we investigated its role in the behavioral response to lipopolysaccharide. OBJECTIVE: To explore the role of LCN2 in modulating behavior following lipopolysaccharide administration using wild type (WT) and lc
