A Preliminary Investigation of the Prevalence of Motor Speech Impairment Within a Pediatric Outpatient Clinical Setting.

BACKGROUND: Childhood apraxia of speech (CAS) is a motor speech disorder often co-occurring with language impairment and complex neurodevelopmental disorders. An association between autism spectrum disorder (ASD) and CAS has been suggested but not definitely quantified. The reported prevalence of CAS has been variable. The current study sought to quantify prevalence of apraxia as well as comorbid ASD presentation in a pediatric population in a large outpatient hospital setting. METHOD: An electronic medical record (EMR) was used to identify all pediatric clients between the ages of 24 and 48

Geometry of orofacial neuromuscular signals: speech articulation decoding using surface electromyography.

Objective. In this article, we present data and methods for decoding speech articulations using surface electromyogram (EMG) signals. EMG-based speech neuroprostheses offer a promising approach for restoring audible speech in individuals who have lost the ability to speak intelligibly due to laryngectomy, neuromuscular diseases, stroke, or trauma-induced damage (e.g. from radiotherapy) to the speech articulators.Approach.To achieve this, we collect EMG signals from the face, jaw, and neck as subjects articulate speech, and we perform EMG-to-speech translation.Main results. Our findings reveal th

Who to Refer for Speech Therapy at 4 Years of Age Versus Who to "Watch and Wait"?

OBJECTIVE: To examine predictors of speech disorder resolution versus persistence at age 7 years in children with speech errors at age 4 years. STUDY DESIGN: Participants were drawn from a longitudinal, community cohort. Assessment at age 4 years (N?=?1494) identified children with speech errors. Reassessment at age 7 years allowed categorization into resolved or persistent categories. Logistic regression examined predictors of speech outcome, including family history, sex, socioeconomic status, nonverbal intelligence, and speech error type (delay vs disorder). RESULTS: At age 7 years, persi

Imitating dysphonic voice: a suitable technique to create speech stimuli for spoken language processing tasks?

PURPOSE: The aim of this study was to assess the suitability of imitated dysphonic voice samples for their application in listening tasks investigating the impact of speakers' voice quality on spoken language processing. METHODS: A female voice expert recorded speech samples (sustained vowels and connected speech) in her normal voice and while imitating a dysphonic voice. Voice characteristics, authenticity, and consistency of the two voice qualities were evaluated by means of acoustic measurements (Acoustic Voice Quality Index [AVQI], jitter, shimmer, harmonics-to-noise ratio [HNR]) and perc

A treatment for dysprosody in childhood apraxia of speech.

PURPOSE: Dysprosody is considered a core feature of childhood apraxia of speech (CAS), especially impaired production of lexical stress. Few studies have tested the effects of intervention for dysprosody. This Phase II study with 3 children investigated the efficacy of a treatment targeting improved control of relative syllable durations in 3-syllable nonwords representing strong-weak (SW) and weak-strong (WS) stress patterns (e.g., BAtigu or baTIgu). Treatment sessions were structured along the principles of motor learning (PML) approach. METHOD: Three children, age 7 to 10 years, with mild

Dysarthria and aphagia: a case study of neuromuscular treatment.

This case study involves a 44-year-old man with flaccid dysarthria, right hemiparesis, left facial paresis, left vocal fold paralysis, and aphagia following multiple gunshot wounds. He was treated surgically with a tracheostomy, feeding gastrostomy, left temporalis muscle sling, left cricopharyngeal myotomy, hypoglossal nerve anastomosis, and left true vocal fold Teflon injection. After seven years, the patient was referred for speech therapy. The areas of glottic closure, velopharyngeal closure, and tongue strength and mobility were stressed initially in speech therapy. Since these mechanisms

Speech characteristics associated with the Furlow palatoplasty as compared with other surgical techniques.

Reported here are the results of a retrospective study of the speech outcome for 63 cleft subjects who had Furlow repairs compared with 20 subjects who had other procedures. The two groups of children were similar in cleft type, sex, and race. The same two surgeons repaired the palates in both groups, and the same two speech pathologists with high reliability examined the children at least 5 years postoperatively using the Pittsburgh Weighted Values for Speech Symptoms Associated with VPI (velopharyngeal incompetence). Subjects who had had Furlow repairs were superior on measures of hypernasal

Contextual and linguistic correlates of intelligibility in children with developmental phonological disorders.

Listeners' glosses of children's intended words provided data for two studies of the potential influence of selected contextual and linguistic variables on word intelligibility. Several regularities associated with the occurrence of unintelligible words were identified. In Study I, intelligibility outcomes were associated with utterance length and fluency, word position, intelligibility of adjacent words, phonological complexity, and grammatical form. In Study II, intelligibility outcomes were associated with phonological complexity, syllabic structure, and grammatical form. Discussion conside

Functional outcomes following palatal reconstruction with a folded radial forearm free flap.

Defects of the soft palate often occur after extirpative procedures are performed to treat oropharyngeal cancers. These defects usually result in velopharyngeal insufficiency and an alteration in speech and deglutition. Palatal prostheses have been used to circumvent this problem in the past. Recently, however, folded radial forearmfreeflaps have been introduced for reconstruction of the soft palate to eliminate velopharyngeal insufficiency and the need for a prosthesis. We conducted a study to evaluate pharyngeal and palatal functions following reconstruction of soft-palate defects with radia

Automatic Speech Recognition from Neural Signals: A Focused Review.

Speech interfaces have become widely accepted and are nowadays integrated in various real-life applications and devices. They have become a part of our daily life. However, speech interfaces presume the ability to produce intelligible speech, which might be impossible due to either loud environments, bothering bystanders or incapabilities to produce speech (i.e., patients suffering from locked-in syndrome). For these reasons it would be highly desirable to not speak but to simply envision oneself to say words or sentences. Interfaces based on imagined speech would enable fast and natural commu

A comprehensive description of functioning and disability in children with velopharyngeal insufficiency.

UNLABELLED: Children with velopharyngeal insufficiency (VPI) experience functional impairments in a variety of areas that extend beyond the primary physical impairment associated with this disorder. At present, the physical deficits associated with VPI have been studied extensively; however, a comprehensive description of social and communicative participation in this population is needed. Therefore, a biopsychosocial framework such as the International Classification of Functioning, Disability and Health for Children and Youth (ICF-CY), may offer an enhanced understanding of the daily experie

Criteria for SLI: the Stark and Tallal legacy and beyond.

Since it first appeared, the Stark and Tallal (1981 ) criteria for the selection of children with specific language impairment (SLI) has had a profound influence on research with this population. A review of the recent literature indicates that these criteria continue to be used, in part or in whole, in current research. However, the recent literature also provides illustrations of the use and interpretations of norm-referenced tests that can serve to update current best practices in subject selection. The original criteria for IQ and language test scores, along with their more recent adaptati

The effects of speech and language therapy for a case of dysarthria associated with Parkinson's disease.

We report the results of a single-subject multiple baseline across behaviours experiment of a 74-year-old woman presenting with Parkinson's disease. Her speech was typical of a hypokinetic dysarthria. The main features of her dysarthric speech were: a restriction in the modulation of fundamental frequency, an inappropriate pitch level and a rate disturbance. Three measures of prosody were operationally defined as follows: (1) linguistic modulation of fundamental frequency, (2) mean fundamental frequency and (3) rate. Treatment focused on ameliorating these aspects employing a multiple baseline
