Evaluation of Alt-RAMEC With Facemask in Patients With Unilateral Cleft lip and Palate (UCLP) Using Cone Beam Computed Tomography (CBCT) and Finite Element Modeling-A Clinical Prospective Study.

OBJECTIVE: Evaluate the effects of alternate rapid maxillary expansion and constriction (Alt-RAMEC) protocol on patients with unilateral cle … d at the anterior region with stresses radiating till the anterior nasal spine, zygomatic arch, and sphenoid bone. CONCLUSION: Alt- RAMEC protocol combined with facemask protraction can lead to significant maxillary advancement, with some transient decrease in buccal bone thickness noted thereby warranting long-term studies to further assess its effect on the …

Sphenoparietal sinus transposition technique: optimization of the surgical corridor with preservation of prominent bridging veins between the brain and the cranial base during aneurysm clipping via the pterional approach.

OBJECTIVE: The sylvian bridging veins between the brain and the dura on the inner surface of the sphenoid wing can restrict brain retraction for widening of the lateral retrocarotid space during clipping surgery for internal carotid artery (ICA)-posterior communicating artery (PCoA) and basilar apex (BX) aneurysms. In such cases, the authors perform extradural anterior clinoidectomy with peeling of the temporal dura propria from the periosteal dura and inner cavernous membrane around the superior orbital fissure, with the incision of the dura mater stretching from the base of the temporal side

IgG4-related disease involving distinctive areas of skull base: our experience.

BACKGROUND: Immunoglobulin G4-related disease (IgG4-RD) is a rare, systemic fibro-inflammatory condition characterized by lymphoplasmacytic infiltration with IgG4-positive plasma cells, storiform fibrosis, and elevated serum IgG4 levels. It can affect multiple organ systems, often mimicking malignancies or infections. Skull base involvement in IgG4-RD is uncommon and presents diagnostic and therapeutic challenges. We present three topographically distinct areas of skull base involvement patterns in IgG4-RD. CASE PRESENTATION: This case series describes three patients with IgG4-RD affecting di

Fungal Sinusitis Spreading to the Sellar Region Mimicking a Pituitary Tumor: Case Report and Literature Review.

Chronic fungal sinusitis (FS) can cause bone erosion and extend to the sellar region, often misdiagnosed as pituitary tumors or malignancies. We report a 56-year-old immunocompetent female with sphenoid FS presenting as a giant sellar mass compressing the optic chiasm, with normal pituitary function. The surgery successfully resolved her symptoms, and a histological examination confirmed the presence of a fungal hyphal mass. We conducted a literature review of 52 publications on FS cases with bone erosion and inflammatory extension to the sellar region, which included analyses of 67 patients (

Garcin syndrome resulting from a giant cell tumor of the skull base in a child.

Garcin syndrome is characterized by a progressive ipsilateral involvement of cranial nerves, culminating in paralysis of all or at least sev … ion. Computed tomographic scan indicated a destructive mass involving the right greater wing of the sphenoid bone and temporal petrous apex. Magnetic resonance imaging demonstrated a tumor arising from the temporosphenoidal region, infiltrating neither the brain nor the brainstem. No hydrocephalus was observed. Biopsy revealed giant cell tumor. Posterior treatm …

Association between type of skull base osteomyelitis (SBO), source, and infection spread in patients with proven SBO.

INTRODUCTION: Skull base osteomyelitis (SBO) is a challenging diagnosis with a variety of nonspecific clinical manifestations and imaging findings. This study aims to determine the association between the type of SBO and the source and infection spread in patients with histopathology/culture-proven SBO. MATERIALS AND METHODS: A retrospective analysis of 55 patients with suspected SBO was conducted. Twenty-two patients (40%) were excluded because either histology resulted in neoplasia without associated infection (2[9%]) or imaging findings were negative for SBO. An additional nine patients we

[Anterior cranio-facial resection for malignant tumors: report of 91 cases].

Between 1987 and 1994 we performed 103 anterior cranio-facial resections in patients affected by tumors involving the ethmoid, the nasal cavities and, sometimes, the orbit, the maxillary and sphenoid sinuses. The cibriform plate was always involved. The tumor invaded the frontal dura in 20 patients without intradural neoplastic vegetations. These were present in 6 cases. Ninety-one of these patients had a malignant tumor; from the histologic point of view we had 50 adenocarcinomas, 16 epidermoid carcinomas, 8 estesioneuroblastomas, 6 adenoid cystic carcinomas, 5 melanomas and 6 infrequent type

Persistent expression of Pax3 in the neural crest causes cleft palate and defective osteogenesis in mice.

Transcription factors regulate tissue patterning and cell fate determination during development; however, expression of early regulators fre … of Pax3 in cranial neural crest cells resulted in cleft palate, ocular defects, malformation of the sphenoid bone, and perinatal lethality. Furthermore, we demonstrated that Pax3 directly regulates expression of Sostdc1, a soluble inhibitor of bone morphogenetic protein (BMP) signaling. Persistent Pax3 expression renders the cranial crest resistant to BMP-induc …

Defective neural tube closure and anteroposterior patterning in mice lacking the LIM protein LMO4 or its interacting partner Deaf-1.

LMO4 belongs to a family of transcriptional regulators that comprises two zinc-binding LIM domains. LIM-only (LMO) proteins appear to function as docking sites for other factors, leading to the assembly of multiprotein complexes. The transcription factor Deaf-1/NUDR has been identified as one partner protein of LMO4. We have disrupted the Lmo4 and Deaf-1 genes in mice to define their biological function in vivo. All Lmo4 mutants died shortly after birth and showed defects within the presphenoid bone, with 50% of mice also exhibiting exencephaly. Homeotic transformations were observed in Lmo4-n…

Orbital restoration surgery in the zygomaticotemporal and zygomaticofacial nerves and important anatomic landmarks.

A variety of etiologies may result in functional and aesthetic deficiencies requiring orbital reconstruction. Fractures of the zygomaticomaxillary complex in the acute stage are frequently accompanied by sensory disturbances of the zygomatic nerve (ZN). The purpose of the current study was to describe the anatomic and topographic landmarks of the ZN in 18 adult human cadavers regarding the localization and dimensions in the orbit. The zygomaticotemporal (ZTN) and zygomaticofacial nerves (ZFN) along the lateral wall of the orbit passed through the zygomaticotemporal and zygomaticofacial foramen

Clinical signs of orbital wall fractures as a function of anatomic location.

OBJECTIVE: The objective of this study was to see whether clinical signs of medial orbital wall fractures distinguished these fractures from fractures of the lateral orbital wall and the orbital floor. STUDY DESIGN: The orbital fractures of 424 patients were analyzed. The patients were divided into 2 groups: (1) patients with orbital fractures with a medial orbital wall component and (2) patients with orbital fractures without a medial orbital wall component. RESULTS: Orbital fractures with involvement of the medial orbital wall showed a significantly higher incidence (P =.001) of diplopia a

[Extradural intracavernous epidermoid cyst. Apropos of 2 cases].

The authors report two cases of intracranial, extradural, intracavernous epidermoid cyst. In one case the cyst was ruptured into the subarachnoid space and in the substance of the temporal lobe. The two cysts had to be subtotally resected because of dense adherences with the internal carotid artery. In both cases the post-operative periods were uneventful.

Pubertal spurts in cranial base and mandible. Comparisons within individuals.

Serial data from cephalometric radiographs were analyzed for 34 boys and 33 girls who had cephalometric radiographs annually near each birthday from at least age 7 through 18 years. Spurts were defined for this study as increases between successive cranial base increments that exceeded 0.75mm/year in boys or 0.5mm/year in girls. The corresponding criterion for the mandible was 1.0mm/year in either sex. Pubescence was defined as the 4- year period spanning 2 years before and after peak height velocity. Spurts during pubescence were common, tending to occur about 1.6yr earlier in girls than boys

[Nervous system lesions in juvenile angiofibromas of the base of the skull with intracranial spread].

The authors examined the nervous system in 12 patients with intracranial dissemination of an adolescent angiofibroma of the brain basis. It was possible to depict signs of a lesion of some cranial nerves and the brain stem as well as x-ray confirmation of a bone destruction in the brain basis. The found changes were connected with the direct influence of the angiofibroma on the above mentioned structures.

Excavating Meckel's cave: Cavum-trigeminale-cephaloceles (CTCs).

BACKGROUND AND PURPOSE: Cavum-trigeminale-cephaloceles (CTCs) are rare lesions of Meckel's cave and the petrous apex. Despite distinctive imaging features, they are frequently mistaken for other petrous apex lesions. In contrast to many of these entities, CTCs do--when asymptomatic--not require any invasive work-up or even surgical excision. Since correct diagnosis has profound impact on clinical decision-making, we report on a series of CTCs with distinct imaging features and their important differential diagnoses. MATERIAL AND METHODS: We report a retrospective series of 5 patients with CTC

Versatility of the pedicled nasoseptal flap in the complicated basal skull fractures.

With the advancement of transnasal endoscopic technique, endoscopic repair of basal skull fractures has considerably substituted former external approaches. The endoscopically feasible pedicled flap, named nasoseptal flap has been extending its range of application, since it was introduced for the reconstruction of the defect after resection of skull base tumors. We introduce two patients with complicated basal skull fractures at different sites who were successfully treated by the transnasal endoscopic approach using nasoseptal flap.
