Clinical and radiographic parameters associated with best versus worst clinical outcomes in minimally invasive spinal deformity surgery.

OBJECTIVE Minimally invasive surgery (MIS) techniques are increasingly used to treat adult spinal deformity. However, standard minimally invasive spinal deformity techniques have a more limited ability to restore sagittal balance and match the pelvic incidence-lumbar lordosis (PI-LL) than traditional open surgery. This study sought to compare "best" versus "worst" outcomes of MIS to identify variables that may predispose patients to postoperative success. METHODS A retrospective review of minimally invasive spinal deformity surgery cases was performed to identify parameters in the 20% of patie

Anteroposterior curvatures of the spine in adolescent athletes.

OBJECTIVE: To assess the shape of anteroposterior vertebral curvatures in adolescents who practice team sports. MATERIAL AND METHODS: 57 females and 104 males aged 14-17 years, playing volleyball, basketball or handball, and 63 females and 99 males as a control group. A Rippstein plurimeter was used to measure the angles of thoracic kyphosis and lumbar lordosis. RESULTS: The study has revealed significant differences in lumbar lordosis in male athletes compared to the control (p=0.01). Male volleyball players had greater thoracic kyphosis (p=0.002) than basketball players. Female athletes ha

Deformity of lumbar spine after selective dorsal rhizotomy for spastic cerebral palsy.

To observe the effect of selective dorsal rhizotomy (SDR) on the deformity of the lumbar spine, a retrospective review was conducted from October 1992 to December 2002 on children who had undergone SDR in our department, and in whom preoperative and postoperative spine radiographs had been obtained. The angles of hyperlordolysis were measured. The study group comprised 219 patients, in which 61 cases were followed up and examined between May 2004 and February 2005. The mean age at surgery was 6.9 years (range 3-20 years), with a mean time to recent follow-up radiographs of 6.3 years (5-9 years

Does age-adjusted pelvic incidence minus lumbar lordosis overcorrection truly increase the risk of proximal junctional failure? A confounder-adjusted multivariate analysis in adult spinal deformity.

BACKGROUND CONTEXT: Correction beyond age-adjusted pelvic incidence minus lumbar lordosis (PI-LL) targets (age-adjusted PI-LL overcorrection) is believed to increase the risk of proximal junctional failure (PJF) in adult spinal deformity (ASD) surgery. However, this association has not been analyzed after adjusting for confounding variables. PURPOSE: To investigate whether age-adjusted PI-LL overcorrection independently increases the risk of PJF after accounting for potential confounders. STUDY DESIGN: Retrospective cohort study using prospectively collected data from a single tertiary spine

Continuous lengthening potential after four years of magnetically controlled spinal deformity correction in children with spinal muscular atrophy.

Magnetically controlled growing rods (MCGR) are commonly implanted for the treatment of early-onset scoliosis. While most authors report favorable short-term results, little is known about long-term deformity correction. This prospective cohort study assesses spinal deformity control in a homogeneous spinal muscular atrophy (SMA) patient group treated with MCGR implants, a standardized lengthening protocol and a minimum follow-up of four years. 17 SMA patients with progressive scoliosis were treated with MCGR implanted parallel to the spine with rib-to-pelvis fixation. Radiologic measurements

Minimally Invasive Posterior Spinal Nonfusion Surgery in Patients With Adolescent Idiopathic Scoliosis Using a Bipolar One-Way Self-Expanding Rod System: Protocol for a Single-Center Clinical Cohort Study.

BACKGROUND: The current surgical treatment for patients diagnosed with progressive and severe adolescent idiopathic scoliosis (AIS) consists of the correction of the spinal curvature, followed by posterior spinal fusion (PSF). However, research has uncovered short- and long-term complications of posterior spinal fusion in patients with AIS. Minimally invasive growing rod techniques have successfully been used to treat patients with early-onset scoliosis and neuromuscular scoliosis. It may be questioned if minimally invasive posterior spinal nonfusion (PSnF) surgery with bipolar instrumentation

Differences in the geometrical spinal shape in the sagittal plane according to age and magnitude of pelvic incidence in healthy elderly individuals.

BACKGROUND: Several studies indicated the influence of age and sex on spinal alignment using spino-pelvic radiographic parameters. However, information regarding the geometrical assessment of the sagittal spinal plane in the elderly population remains limited. This study aimed to determine the apices of lumbar lordosis and thoracic kyphosis, and spinal inflection point in elderly individuals and clarify the effect of age, sex, and pelvic incidence (PI) on sagittal geometry. METHODS: In total, 440 volunteers (193 men; 247 women) were enrolled. The spino-pelvic radiographic parameters were meas

The effect of spinal orthoses on pain, kyphosis angle, balance, fall risk, and quality of life in older adults with hyperkyphosis: A systematic review.

Hyperkyphosis, defined as excessive forward thoracic curvature, is associated with pain, impaired balance, increased fall risk, and reduced quality of life in older adults. Spinal orthoses have been proposed as a therapeutic option. This systematic review evaluated their effects on pain, kyphosis angle, balance, fall risk, and quality of life. The protocol was registered in PROSPERO (CRD42024607584) and followed PRISMA 2020 guidelines. Comprehensive searches of PubMed, Web of Science, Scopus, CINAHL, and Cochrane Library up to November 2024 identified 16 eligible studies involving 757 particip

Physiotherapy as an adjuvant to the surgical treatment of anterior chest wall deformities: a necessity? A prospective descriptive study in 21 patients.

PURPOSE: The authors postulated that physiotherapy as an adjuvant to the surgical treatment of anterior chest wall deformities is only indicated if specific abnormalities can be found that could be corrected by physiotherapy. The purpose of this study is to investigate whether such abnormalities can be found and to evaluate their course during a postoperative period of 18 months. METHODS: Twenty-one patients, 16 with pectus excavatum and 5 with pectus carinatum, were evaluated 6 weeks before and 6 weeks, 6 months, and 18 months after surgical correction. Postural impairments, spinal mobility

Kyphotic Posture Reduces Respiratory Efficiency During Walking and Running.

BACKGROUND: Altered spinal alignment can affect ventilatory mechanics, yet the acute impact of kyphotic posture during dynamic exercise remains unclear. This study aimed to examine the acute effects of a simulated kyphotic posture on ventilatory efficiency during treadmill standing, walking, and running in healthy young adults. We hypothesized that kyphotic posture would increase ventilatory burden, particularly at higher exercise intensities. METHODS: Ten healthy young men completed two postural conditions (kyphotic and neutral) while standing, walking (4 km/h), and running (10 km/h) on a re

Current Status of Vertebral Body Tethering for Adolescent Idiopathic Scoliosis: An Umbrella Review.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a prevalent spinal deformity among teenagers worldwide. Vertebral body tethering (VBT) is an innovative, minimally invasive technique developed to address spinal curvature by modulating vertebral growth. However, the existing body of evidence regarding the effectiveness and safety of VBT in treating AIS is fragmented and requires thorough consolidation and critical assessment. METHODS: Six databases were thoroughly examined, yielding 11 relevant systematic reviews and meta-analyses. The methodological quality of the included studies was eva

Ependymal polarity defects coupled with disorganized ciliary beating drive abnormal cerebrospinal fluid flow and spine curvature in zebrafish.

Idiopathic scoliosis (IS) is the most common spinal deformity diagnosed in childhood or early adolescence, while the underlying pathogenesis of this serious condition remains largely unknown. Here, we report zebrafish ccdc57 mutants exhibiting scoliosis during late development, similar to that observed in human adolescent idiopathic scoliosis (AIS). Zebrafish ccdc57 mutants developed hydrocephalus due to cerebrospinal fluid (CSF) flow defects caused by uncoordinated cilia beating in ependymal cells. Mechanistically, Ccdc57 localizes to ciliary basal bodies and controls the planar polarity of e

Convolutional Neural Network to Segment Laminae on 3D Ultrasound Spinal Images to Assist Cobb Angle Measurement.

A recent innovation in scoliosis monitoring is the use of ultrasonography, which provides true 3D information in one scan and does not emit ionizing radiation. Measuring the severity of scoliosis on ultrasonographs requires identifying lamina pairs on the most tilted vertebrae, which is difficult and time-consuming. To expedite and automate measurement steps, this paper detailed an automatic convolutional neural network-based algorithm for identifying the laminae on 3D ultrasonographs. The predicted laminae were manually paired to measure the lateral spinal curvature on the coronal view, calle

Congenital abnormalities of the urogenital tract in association with congenital vertebral malformations.

There is a close link between the embryological development of the musculoskeletal system and all other main organ systems. We report a prospective series of 202 patients with congenital vertebral abnormalities and document the associated abnormalities in other systems. There were 100 boys and 102 girls. In 153 there were 460 associated abnormalities, a mean of 2.27 abnormalities for each patient. Intravenous pyelography was carried out on 173 patients (85.6%) and ultrasonography on the remaining 29 (14.4%). Patients with genitourinary anomalies were more likely to have musculoskeletal (p = 0.

Mobile arm supports: criteria for successful use in muscle disease patients.

Twenty-nine subjects who had been fitted with mobile arm supports 6 months to 11 years prior to the initiation of this study were evaluated to determine criteria for successful use of such supports. All subjects were diagnosed as having Duchenne muscular dystrophy or spinal muscular atrophy. Three methods were used to collect data: 1) review of medical charts, 2) interview with patient and family member(s), and 3) observation of the patient in his wheelchair for sitting stability, adjustment and control of the mobile arm support, and speed and efficiency in self-feeding with and without the su

Circumspinal decompression and fusion through a posterior midline incision to treat central calcified thoracolumbar disc herniation: a minimal 2-year follow-up study with reconstruction CT.

PURPOSE: There have been several surgical approaches used in the treatment of thoracolumbar disc herniation (TLDH) from T10/11 to L1/2. However, central calcified TLDH cases are still challenging to spine surgeons. The anterior transthoracic approaches and lateral/posterolateral approaches are all essentially performed from one side; thus, the compressive lesion and the dura matter on the other side of the spinal canal are not clearly visualized, predisposing the procedure to incomplete decompression or inadvertent cord manipulation. Moreover, a number of these approaches are technically deman
