The effect of steroid therapy was studied on twenty-eight cases of Bell's palsy. Complete and partial recovery was obtained in twenty-five cases. Delay in starting the therapy with steroids seems to be a major factor responsible for the failures.

The history, anatomy, and pathology of trigger finger, carpal tunnel syndrome, and de Quervain's disease are discussed. A review of 171 such cases treated with steroid injections indicates that injection for trigger finger and de Quervain's disease can be quite successful. The results in cases of carpal tunnel syndrome are less predictable.

Antibiotic usage has rendered neurosyphilis uncommon, and cerebral gummas are rare. Reduced awareness of cerebral gummas and abolition of serologic screening can delay diagnosis of this treatable disease. Diagnostic confusion between syphilitic and nonsyphilitic cerebral mass lesions can be increased by apparent resolution of a gumma during steroid therapy. Such an occurrence in a young woman emphasizes the need for serologic testing for syphilis in diagnosing cerebral mass lesions. Seventy-six ichthyotic male patients with a biochemically confirmed diagnosis of steroid sulphatase deficiency are reported. Ascertainment was based on either a previous diagnosis of placental steroid sulphatase deficiency (21 probands and 15 secondary cases), or ichthyosis with steroid sulphatase deficiency (29 probands and 11 secondary cases). Cytosolic estrogen (ER), progesterone (PR), androgen (AR), and glucocorticoid (GR) receptors were evaluated in 6 meningiomas and 12 gliomas (6 astrocytomas and 6 glioblastomas) using a dextran-coated charcoal method. A patient undergoing chronic corticosteroid therapy presented with mesenteric lipomatosis that simulated malignant masses. Computerized tomography was instrumental in identifying these apparent tumors as benign fatty deposits. The spectrum of lipomatous abdominal lesions amenable to diagnosis by computerized tomography is reviewed.

Low maternal plasma and urinary oestrogen concentrations in pregnancy are usually indicative of fetal problems, either placental insufficiency or fetal adrenal hypoplasia. Paediatricians, however, should follow obstetricians in becoming increasingly aware that deficiency of placental steroid sulphatase activity, a condition related to X linked ichthyosis, may produce the same abnormalities.

Between 1963 and 1980 cortisone-induced osteonecrosis has been seen in 122 patients at the Royal Victoria Hospital, Montreal. Of these, 68 had had renal homotransplants; the others suffered from a variety of conditions. The femoral head was most commonly affected; the humeral head, distal femur, proximal tibia, talus and capitellum were also involved. There were two patterns of involvement, similar in all locations. Serum concentrations of pregnenolone sulphate, dehydroepiandrosterone sulphate and 5-androstene-3 beta,17 beta-diol sulphate were measured by radioimmunoassays in twelve patients with various types of ichthyosis. In X-linked ichthyosis (n = 5), ichthyosis vulgaris (n = 5) and lamellar ichthyosis (n = 1), steroid sulphates were not significantly higher than in the control subjects. Several etiologic agents have been associated with this complication, although the pathogenesis in each instance may prove to be more uniform. Clinical observations showed that the natural history of sarcomatous lesions may be different among males and females, and it may be influenced by hormonal factors.
