Mechanisms of caries induced by sugars: a narratives review from microbial metabolism to oral ecological imbalance and public health strategies for caries prevention.

Dental caries is defined as a chronic, multifactorial disease characterized by the demineralization of dental hard tissues resulting from the acid production by oral microbial communities metabolizing dietary sugars. The ingestion of sugars is a pivotal ecological factor in the progression of caries, with mechanisms that extend beyond merely providing substrates for cariogenic bacteria. This review explores the influence of sugars on the metabolism, adhesion, biofilm formation, and interspecies interactions of oral microorganisms, with a particular focus on species such as Streptococcus mutans…

Complicated subacute bacterial endocarditis in a patient with ventricular septal defect.

Infective endocarditis (IE) is an uncommon but life-threatening infection. Despite advances in management, it still causes high morbidity and mortality. We report the case of an 8-year-old girl who presented with a prolonged fever of 2.5 months duration and a history of a small perimembranous ventricular septal defect. She was diagnosed with subacute bacterial endocarditis secondary to Streptococcus mutans. The patient developed a septic pulmonary embolism; however, with the use of appropriate antimicrobial therapy, she made an uneventful recovery. Clinicians should have a high index of suspic…

Tea tree oil in inhibiting oral cariogenic bacterial growth an in vivo study for managing dental caries.

Dental caries is considered a major health burden, and preventive strategies are needed to improve oral health. It is suggested that natural essential oils possess anti-plaque formation properties and exhibit strong antimicrobial activity; however, in vivo studies to support these concepts are scarce. We evaluated the effects of tea tree oil (TTO) on caries initiation and progression in vivo to generate supportive data for clinical studies in patients at high risk of caries. We first assessed TTO in vitro against Streptococcus mutans and Streptococcus sobrinus, two of the most common oral bact…

Cytotoxicity, antibacterial efficacy, and fluoride content of commercial herbal toothpastes.

OBJECTIVE: This study evaluated fluoride concentration, antibacterial activity against Streptococcus mutans, and cytotoxicity of commercially available herbal toothpastes to assess their safety and clinical potential. MATERIALS AND METHODS: Fifteen herbal toothpastes were tested. Fluoride concentration was measured using a fluoride ion-selective electrode. Antibacterial activity against S. mutans was assessed by agar diffusion, and cytotoxicity was evaluated using the MTT assay in L929 fibroblasts and human gingival fibroblasts. Antibacterial and cytotoxicity data were analyzed using one-way …

Specific host translation perturbations promote Tn Smu 1 loss independently of the canonical ImmA regulator.

Integrative and conjugative elements (ICEs) are major drivers of horizontal gene transfer and bacterial genome evolution. Although ICE-encoded regulatory circuits have been extensively characterized, the impact of host physiology on the stability of integrated ICEs remains poorly understood. Here, we identify a host-dependent pathway that links specific host translation perturbations to loss of the ICE Tn Smu1 in Streptococcus mutans . Analysis of host-gene deletion mutants revealed that disruption of fmt , rnjA , or rnjB -three translation-associated host genes-reproducibly promoted Tn Smu1 l…

Clinical and microbiological investigations of anorexia nervosa.

Several studies have examined the relationship between anorexia nervosa and oral status. However, none has used control subjects. Furthermore, none has examined bacteriological data in these subjects to determine whether dietary restrictions, particularly of carbohydrates, change the levels of Streptococcus mutans and Lactobacillus species. Fifteen female subjects recently hospitalized for anorexia were studied along with fifteen age- and sex-matched control subjects. The parameters examined included salivary pH and flow rate, levels of S. mutans and Lactobacillus spp., DMFT scores, facial and…

Genetic characterization of glyoxalase pathway in oral streptococci and its contribution to interbacterial competition.

OBJECTIVES: To analyze contributions to microbial ecology of Reactive Electrophile Species (RES), including methylglyoxal, generated during glycolysis. METHODS: Genetic analyses were performed on the glyoxalase pathway in Streptococcus mutans (SM) and Streptococcus sanguinis (SS), followed by phenotypic assays and transcription analysis. RESULTS: Deleting glyoxalase I (lguL) reduced RES tolerance to a far greater extent in SM than in SS, decreasing the competitiveness of SM against SS. Although SM displays a greater RES tolerance than SS, lguL-null mutants of either species showed similar to…

Local antibiotic delivery in periodontitis: drug release and its effect on supragingival biofilms.

The effect of a drug-delivery system containing antibacterial metronidazole (MDZ) prescribed for periodontitis on supragingival biofilm was evaluated, and possible interference by this biofilm in the drug release profile was investigated. Streptococcus mutans biofilms were grown and exposed to a controlled-release formulation of MDZ or the same formulation without MDZ (vehicle control). Untreated biofilms were used as a negative control (NC). Biofilms and culture medium (containing detached cells) were collected 24, 48, 72, and 96 h after first exposure to treatments. The biomass of the MDZ gr…

In vitro bacterial adherence onto pellicle-coated aesthetic restorative materials.

Adhesion of oral bacteria to teeth and restorative materials plays an important role in the pathogenesis of oral diseases. This study investigated the initial adhesion of Streptococcus mutans to enamel and restorative materials. Three types of composites and two types of glass ionomer cements were used. The specimens were coated with freshly collected human parotid saliva. The salivary coated samples were incubated with cell-free glucosyltransferase, and further incubated with sucrose solution. Finally, the specimens were incubated with 3H-thymidine labelled bacteria. Adhesion of the bacteria …

Effects of 275 nm Ultraviolet Light-Emitting Diode Irradiation on Oral Bacteria In Vitro and Toothbrush Sanitization.

The oral cavity harbors a complex microbial community where pathogens implicated in dental caries and periodontitis can heavily colonize toothbrushes, transforming them into persistent sources of contamination that threaten both oral and systemic health. Consequently, this study evaluated the bactericidal efficacy of 275 nm ultraviolet light-emitting diode (UV-LED) irradiation against common oral bacteria in vitro and its practical utility for extraoral toothbrush sanitization. Suspensions of Streptococcus mutans, Streptococcus sanguinis, Porphyromonas gingivalis, and Fusobacterium nucleatum w…

In Vitro Evaluation of Biofilm Formation by Oral Microorganisms on Clear Aligner Materials: Influence of Mouthwash Exposure.

Clear aligners have gained popularity in orthodontics due to their aesthetics, comfort, and removability; however, their prolonged intraoral wear and frequent removal-reinsertion cycles create favorable conditions for microbial colonization. This in vitro study evaluated the efficacy of seven commercially available mouthwash formulations in inhibiting biofilms of Streptococcus mutans, Streptococcus oralis, and Candida albicans formed on four different clear aligner materials. Standardized aligner fragments were incubated for 24 h with microbial suspensions to allow biofilm formation, treated f…
