A Gas Chromatography-Mass Spectrometry Method to Determine Tramadol Abuse Using Urine Samples.

Background The synthetic opioid tramadol is widely used as a pain reliever. Unlike other opioids, it is used freely worldwide, unaffected by international controls resulting in abuse and accidental intoxication. Analytical methods are necessary to prove tramadol abuse because 30% of the drug is excreted unchanged. Methodology This study describes a sensitive, precise, and accurate gas chromatograph-mass spectrometry (GC-MS) method for tramadol quantification in biological samples using solid-phase extraction (SPE) for sample preparation. Results A total of 747 samples were analyzed for suspect

Illicit psychotropic substances in the air: The state-of-art.

The occurrence of psychotropic substances (PSs) in the air is known since long time. Recently, attention was paid to illicit PS, with most investigations undertaken in Italy and Spain. In general, collection of illicit substances was performed through aspirating airborne particulates onto filters; afterwards, gas chromatography or high-performance liquid chromatography coupled with mass spectrometry were applied for the PS evaluation. Over twenty substances could be characterized simultaneously. Cocaine concentrations up to 17ngm(-3) were observed in Latin America, while this substance was abs

Screening for drugs of abuse (II): Cannabinoids, lysergic acid diethylamide, buprenorphine, methadone, barbiturates, benzodiazepines and other drugs.

Requirements for the provision of an efficient and reliable service for drugs of abuse screening in urine have been summarized in Part I of this review. The requirements included rapid turn-around times, good communications between requesting clinicians and the laboratory, and participation in quality assessment schemes. In addition, the need for checking/confirmation of positive results obtained for preliminary screening methods was stressed. This aspect of the service has assumed even greater importance with widespread use of dip-stick technology and the increasing number of reasons for whic

Utility of the Time-Line Follow-Back to assess substance use among homeless adults.

Assessing substance use of homeless persons is a critical task. This study examines the test-retest reliability, concurrent validity, and sensitivity to change of the Time-Line Follow-Back interview, a calendar instrument used to assess days and quantities of alcohol use and days of illicit drug use, in the multisite Collaborative Program to Prevent Homelessness (CPPH). The Time-Line Follow-Back was reliable for assessing use during the past month and the recent 6 months. Results from the Time-Line Follow-Back were correlated with other self-reports of use, with research diagnoses of substance

[Atypical bipolar disorders].

Some epidemiologic data reveal how difficult detecting atypic bipolar disorders is: 9 years of progression before the diagnosis is properly established and a specific treatment is initiated, and intervention of 4 to 5 different specialists. Incomplete symptomatology, impulsive actions, periodic alcohol abuse, compulsive buying behaviors, acute delusional episodes, medicolegal actions and comorbidities can hide or modify bipolar symptomatology. Bipolarity should be systematically screened for in case of substance abuse (40 to 60 percent of bipolar disorders), anxiety disorders (panic disorder,

Results of random drug testing in an adolescent substance abuse program.

OBJECTIVE: The objective of this study was to estimate from a random urine drug-testing program for adolescents the proportion of drug tests that are susceptible to interpretation errors. METHODS: This was a secondary analysis of a clinical database and chart review from an adolescent outpatient substance abuse program at a large children's hospital. We analyzed from 110 adolescent patients (13-21 years of age) all 710 urine drug test results that were collected between December 2002 and July 2005 and 85 original laboratory reports for tests that were collected between December 2002 and May 2

Munchausen syndrome by proxy (MSBP): an extreme form of child abuse with a special forensic challenge.

Munchausen syndrome by proxy (MSBP) is a special form of child abuse in which an adult repeatedly produces symptoms of illness in a person under his/her care. In most cases the perpetrators are mothers who repeatedly and in different ways produce or feign symptoms of illness in their children in order to obtain medical treatment for them. MSBP is thus a special form of child abuse that is also of importance in the field of forensic medicine and a particular challenge to the medicolegal expert. We report two cases of poisoning with different substances (clozapine and clonidine) detected by toxi

Urban wastewater analysis as an effective tool for monitoring illegal drugs, including new psychoactive substances, in the Eastern European region.

The use of illicit drugs causes unquestionable societal and economic damage. To implement actions aimed at combating drug abuse, it is necessary to assess illicit drug consumption patterns. The purpose of this paper was to develop, optimize, validate and apply a procedure for determining new psychoactive substances (NPSs) and classic drugs of abuse and their main metabolites in wastewater samples by using solid phase extraction (SPE) and high-performance liquid chromatography coupled with tandem mass spectrometry (HPLC-MS/MS). Moreover, detailed validation of the procedure was conducted. The d

Exploring gender differences among treatment-seekers who use opioids versus alcohol and other drugs.

Identifying clinical differences between opioid users (OU) and alcohol and other drug users (AOD) may help to tailor treatment to OU, particularly among the majority of OU who are not on opioid agonist treatments. Given the dearth of research on these differences, this study explored gender differences in demographic and clinical characteristics between OU and AOD. Participants (N = 506) were from a multisite, randomized controlled clinical trial of an Internet-delivered psychosocial intervention conducted in 2010-2011. Logistic regression models explored differences in demographic and clinica

A prospective study of substance use and housing stability in a homeless population.

OBJECTIVES: This study examined self-report and urine test data about homeless substance use over time, prospectively comparing substance use with attainment of stable housing. METHODS: 400 homeless people systematically sampled from shelters and streets in St. Louis, Missouri were assessed with structured diagnostic interviews and urine substance testing annually over 2 years. Nearly two-thirds (n = 255) completed all three assessments, constituting the sample for this prospective study. RESULTS: More than half (55%) of this homeless sample had detectable cocaine use during the study. Most

Association Between Substance Use and Trauma Outcomes in Adolescents.

INTRODUCTION: Adolescent substance use and substance use disorders are significant public health issues. Our goal was to evaluate the association between adolescent substance use, detected via blood alcohol levels and urine drug screens, and trauma-related outcomes at a Level I pediatric trauma center. Most of the literature is focused on adult trauma patients with limited data in the pediatrics. METHODS: In this retrospective cohort study, we analyzed data from adolescent trauma patients 13-17 years of age presenting to a Level I pediatric trauma emergency department (ED). Demographic data,

Disparities in Healthcare Providers' Substance Use Detection Practices for Pregnant Patients.

OBJECTIVE: To assess providers' substance use screening/testing practices in patient care and identify disparities in their application. We hypothesized that patient race and social vulnerability (SV) are independently and jointly associated with increased rates of provider self-reported substance use screening/testing. METHODS: A 2 × 2 factorial vignette design was used to survey OB/GYN, Midwifery, and Family Medicine providers. The patients' age and medical characteristics were identical in each vignette, but two elements varied dichotomously: (1) the patient's race (Black vs. White) and (2

On the Value of Measuring Recent Drug Use by Self-Reports and Urinalysis in Clinical Trials.

Background: Valid measurement of drug use in patients enrolled in clinical trials that treat substance use disorder is vital to determine the trial's outcome. Self-reports are often used but their validity has been studied with mixed results. Urinalysis may sometimes be employed as an alternative or supplement to self-reports. Objectives: This study examined how estimating drug use by either method would affect the results from a randomized clinical trial conducted in a methadone treatment program. At the initial Baseline interview and four follow-up interviews, participants were asked about t
