[Pars Plicata Vitrectomy in Premature Newborns for Retinal Detachment as a Result of Retinopathy of Prematurity, our Results].

INTRODUCTION: The aim of this work is to evaluate our own results of surgical treatment of retinal detachment in immature newborns. Retinopathy of prematurity (ROP) is one of the most complicated ocular disorders, both in terms of diagnosis and therapy. It is a potentially blinding illness that arises from the incomplete development of the bloodstream of the neuroretina of preterm infants. Currently, the most effective therapy is ablation of the avascular retina by laser photocoagulation or cryocoagulation. Despite this treatment, the immature retina may develop it´s detachment. Multi-drug resistant Bacteroides fragilis recovered from blood and severe leg wounds caused by an improvised explosive device (IED) in Afghanistan.

This report summarizes the case of a 23 year-old otherwise healthy male that was injured in an improvised explosive device (IED) blast in support of Operation Enduring Freedom (OEF). He sustained bilateral open tibia and fibula fractures in the setting of being exposed to water contaminated with raw sewage. Despite long-term carbapenem therapy, the patient's wounds were repeatedly noted to have purulent drainage during surgical debridement and cultures from these wounds were persistently positive for Bacteroides fragilis. Apparent clinical failure persisted despite the addition of metronidazol

Facet joint plus interspinous process graft fusion to prevent postoperative late correction loss in thoracolumbar fractures with disc damage: finite element analysis and small clinical trials.

BACKGROUND: The biomechanical mechanism of spinal three-column after interspinous process fusion remains unclear. The goal of this study is to assess the biomechanics and clinical effects of facet joint plus interspinous process graft fusion on preventing postoperative correction loss in thoracolumbar fractures with disc damage treated with posterior approach. METHODS: By simulating internal fixation device removal postoperatively, two surgical finite element models of the L1-L2 segments for facet joint plus interspinous process fusion (treatment group model) and single-level facet joint fusi

Anderson-Fabry disease: developments in diagnosis and treatment.

BACKGROUND: Lumbar interbody fusion has long been used in the treatment of degenerative disc disease. Lumbar spinal interbody fusion surgery traditionally is an open surgical technique. Although lumbar spinal interbody fusions using endoscopy have been reported, the endoscope was used partially for the interbody fusion. We are reporting a case where lumbar interbody fusion with discectomy was entirely done through direct visualization with the endoscope. METHODS: We report a case of a 55-year-old woman who underwent the transforaminal percutaneous full-endoscopic lumbar interbody fusion techn

Complications and Early-Term Radiographic Analysis of a Novel Active Compression Tibiotalocalcaneal Arthrodesis Nail With a Proximal Flexible Coil.

BACKGROUND: Tibiotalocalcaneal (TTC) arthrodesis is used among other methods as a salvage procedure for complex foot/ankle deformity. Fixation is usually achieved through plate/screw constructs or intramedullary (IM) nailing. Although IM nailing uses smaller incisions, fusion rates are variable and complications rates ranging from 25% to 55.7% have been reported. The Phantom ActivCore (AC) nail could serve as a viable alternative to traditional IM nailing by allowing for increased compression capacity and a potentially lower risk of stress fracture. However, no studies have described the early

A machine learning model for predicting postoperative complication risk in young and middle-aged patients with femoral neck fractures.

OBJECTIVE: Femoral neck fractures are the most common type of hip fracture, and the postoperative complications associated with these fractures significantly affect patients' quality of life and healthcare costs. The objective of this study was to develop a predictive model using machine learning (ML) techniques to assess the risk of postoperative complications in young and middle-aged patients with femoral neck fractures. METHODS: We retrospectively analyzed data from 899 young and middle-aged patients with femoral neck fractures who underwent surgical treatment between September 2019 and Ju

Proximal femoral bionic nail (PFBN) offers comparable functional and clinical outcomes to PFNA and intertan in the treatment of intertrochanteric fractures: a systematic review and meta-analysis.

BACKGROUND: Intertrochanteric fractures represent nearly 45% of hip fractures and are rising due to global aging. Surgical fixation, especially intramedullary nailing with PFNA or InterTAN, is the preferred treatment. However, complications like implant failure persist. The proximal femoral bionic nail (PFBN), designed to mimic natural femoral anatomy, has emerged as a promising alternative. This systematic review and meta-analysis aimed to compare PFBN with PFNA or InterTAN. METHODS: This study followed PRISMA guidelines and was registered on PROSPERO. A comprehensive search of PubMed, Scopu

Numerical evaluation of internal femur osteosynthesis based on a biomechanical model of the loading in the proximal equine hindlimb.

Femoral fractures are often considered lethal for adult horses because femur osteosynthesis is still a surgical challenge. For equine femur osteosynthesis, primary stability is essential, but the detailed physiological forces occurring in the hindlimb are largely unknown. The objective of this study was to create a numerical testing environment to evaluate equine femur osteosynthesis based on physiological conditions. The study was designed as a finite element analysis (FEA) of the femur using a musculoskeletal model of the loading situation in stance. Relevant forces were determined in the mu

Retrospective analysis of medium-term outcomes following anterior lumbar interbody fusion surgery performed in a tertiary spinal surgical centre.

INTRODUCTION: Anterior lumbar interbody fusion (ALIF) can treat spondylolisthesis, degenerative disc disease and pseudoarthrosis. This approach facilitates complete discectomy, disc space distraction, indirect decompression of neural foramina and placement of large interbody devices. Several intra- and postoperative complications can be attributed to the anterior approach: vascular/visceral injury, hypogastric plexus injury and urogenital consequences. Spine-specific complications include implant migration, graft failure, pseudoarthrosis and persistent symptomology. METHODS: This retrospectiv

Biosensitive and antibacterial coatings on metallic material for medical applications.

Metallic materials are commonly used for load-bearing implants and as internal fixation devices. It is customary to use austenitic stainless steel, especially surgical grade type 316L SS as temporary and Ti alloys as permanent implants. However, long-term, poor bonding with bone, corrosion, and release of metal ions, such as chromium and nickel occur. These ions are powerful allergens and carcinogens and their uncontrolled leaching may be avoided by surface coatings. Therefore, bioactive glasses (BGs) became a vital biomedical material, which can form a biologically active phase of hydroxycarb

One-stage revision anterior cruciate ligament reconstruction in cases with excessive tunnel osteolysis. Results of a new technique using impaction bone grafting.

BACKGROUND: This level IV study describes a new one-stage procedure for revision ACL reconstruction in cases with extreme tunnel widening. METHODS: Eight consecutively treated subjects requiring ACL revision and presenting with excessive tunnel widening (87.5% to 250% tunnel enlargement) were included in this study. The graft-tunnel mismatch was resolved in this one-stage revision procedure by the use of custom-made eight to 10 mm cylindrical shaped bone allografts in a press-fit construct with the ACL-graft in combination with the usual fixation devices for ACL-reconstruction. All subjects w

Tendon transfers for drop foot correction: long-term results including quality of life assessment, and dynamometric and pedobarographic measurements.

BACKGROUND: Drop foot deformity is a common problem with severe restrictions in quality of life and impairment of daily activities. A technique of posterior tibial tendon transfer through the interosseus membrane and fixation to the anterior tibial and the long peroneal tendon "Bridle procedure" (stirrup-plasty) offers a physiological alternative to surgical correction. METHODS: Data of 53 consecutive patients treated by stirrup-plasty were acquired from patient's charts; 31 were interviewed with standardized questionnaires; 20 were examined physically; 19 received pedobarography, and 8 under

Is Removal of Implants Mandatory Following Minimally Invasive Percutaneous Screw-Rod Stabilization Without Fusion for Mono-Segmental Thoracolumbar Fractures in Elderly Patients?

PURPOSE: Despite the lack of evidence, the current standard of care following posterior pedicle screw-rod stabilization for spinal trauma includes instrumentation removal. This retrospective cohort study aimed to assess the necessity of implant removal in patients aged ≥65 years who underwent minimally invasive pedicle screw-rod fixation for AO type A and B thoracolumbar fractures. METHODS: We evaluated the clinical and radiological outcomes of 57 patients aged ≥65 years with mono-segmental AO type A and B thoracolumbar fractures treated with percutaneous short-segment pedicle screw fixation,

Osteogenic and Regenerative Potential of Free Gingival Graft.

We studied immunolocalization of CD29, CD44, osteocalcin, and TGF-β1 in the bone tissue of the mandible of miniature pigs with extra-bone fixation of a free gingival graft. Three months after surgery, neoosteogenesis foci with high expression of the studied markers were found in the contact area of the free gingival graft with the alveolar bone. The markers were localized in the layer of external circumferential lamellae, on the surface of concentric lamellae of the growing osteons, and in the connective tissue of the Haversian canals. TGF-β1-immunopositive cells predominated in the connective

A clinical decision-making model for repeat surgical treatment of pectus Bar displacement: distance measurement after nuss procedure.

BACKGROUND: Bar displacement is one of the most common and serious complications after the Nuss procedure. However, measurements of and factors affecting bar displacement have not been reported. The objectives of this study were to develop a decision model to guide surgeons considering repeat treatment and to estimate optimal cut-off values to determine whether reoperation to correct bar displacement is warranted. METHODS: From July 2011 to August 2013, ninety bars were inserted in 61 patients who underwent Nuss procedures for pectus excavatum. Group A did not need surgical intervention and G

Salvage of infected intramedullary knee arthrodesis with vascularized free fibula and staged fixation.

Intramedullary knee arthrodesis is indicated when an infected total knee arthroplasty revision is failed. There is a high risk of infection after this procedure and the only option of treatment considered in these cases is AK amputation. We present an alternative to AK amputation in intramedullary arthrodesis infected cases that has been successful in all patients we have treated. From 2008 to 2012 five patients (4 men and one woman) were treated. The treatment protocol was staged: (1) debridement, removing intramedullary implant and cement, PMMa as spacer and placement of a double-frame exter

Mesh cages for spinal deformity in adults.

Correction of deformity including restoration of sagittal plane malalignment in patients with spinal disorders is facilitated by placement of structural allografts and cages into the anterior column. The effectiveness of these implants will be reviewed with a focus on anterior column implantation of titanium mesh cages. In a consecutive study of adult patients who had spinal fusions for sagittal plane deformities using structural titanium mesh cages, cage status, fusion status, and outcome were assessed. Followup ranged from 24 to 62 months (average, 33 months). No cage failure of extrusion wa
