Central wedge resection is an alternative treatment, providing sufficien … desis, and central wedge resection are methods of treatment. Collateral ligament reconstruction and tendon transfer were unsuccessful in one patient. IP joint deviation of greater than 15 degrees may need treatment. If there is less than 10 degrees of motion associated with it, arthrodesis is indicated when the joint is skeletally mature. When opposition is impai …

PURPOSE OF REVIEW: Cubital tunnel syndrome is the second most common compressive neuropathy, next to only carpal tunnel syndrome in its inci … to prevent further axonal injury, until recently, the only option to restore intrinsic function was tendon transfers. Tendon transfers aim to restore thumb side pinch and control clawing with addition surgery. They also require the sacrifice of wrist extensors or finger flexors. In the past decade, nerve transfers to the distal portion of the ulnar nerve innervat …

The results of conservative and operative treatment of massive rotator cuff tears were compared. All 9 men and 5 women who were treated conservatively (group I) had a Hamada Gpoup 4 arthrographic tear. The mean follow-up period was 4 years. The operative group (group II) was composed of 23 men and 3 women who had cuff tears of more than 5.0 cm in longest dimension. The mean age range at surgery was 62 years. Following anterior acromioplasty (n = 26), additional procedures included tenorrhaphy (n = 12), fascial grafting (n = 8), laterl transfer of the long head of the biceps brachii tendon (n =

CASE: We present a 67-year-old woman with long finger extrinsic extensor tightness and a 56-year-old man with limited index finger flexion due to extrinsic extensor tightness secondary to tendon transfers for radial nerve palsy. Both patients underwent prior surgical procedures that led to limited range of motion (ROM). Subsequently, they elected for central tendon tenotomy (CTT), which demonstrated postoperative ROM improvement and satisfactory patient outcomes.
CONCLUSION: Surgical management of extrinsic extensor tendon tightness of the hand is generally addressed by performing tenolysis t…

Opposition is not grasp but a preposition for grasp that involves 3 components of thumb movements: abduction, flexion, and pronation. Thumb opposition is usually lost with paralysis of the thenar muscles innervated by the median nerve. Many opposition transfers have been described that differ in the donor tendon, route of transfer, and method of attachment to the thumb. No one transfer is applicable for every clinical condition, and each transfer has its advantages and disadvantages. Many factors must be evaluated to decide if surgery is likely to be beneficial and then decide on the optimum t

INTRODUCTION: Shoulder dislocations can cause acute and chronic instabilities that need to be addressed in order to restore joint functioning. The transfer of the coracoid process has become a feasible surgical procedure in patients with shoulder instability. Several concomitant injuries after recurrent dislocations have been described.
CASE PRESENTATION: A 32-year-old German man presented to our department with a history of recurrent shoulder dislocations. He was diagnosed with an avulsion fracture of the coracoid process and dislocation of an osseous piece with attachment to the conjoined t

OBJECTIVE: To evaluate the effectiveness of flexor hallucis longus tendon harvested using a minimally invasive technique in reconstruction of chronic Achilles tendon rupture.
METHODS: Between July 2006 and December 2009, 22 patients (22 feet) with chronic Achilles tendon rupture were treated, including 16 males and 6 females with a median age of 48 years (range, 28-65 years). The disease duration was 27-1,025 days (median, 51 days). Twenty-one patients had hooflike movement's history and 1 patient had no obvious inducement. The result of Thompson test was positive in 22 cases. The score was 5

BACKGROUND: Volar plate fixation for distal radial fractures is the most common cause of closed rupture of the flexor pollicis longus tendon (FPL). For treating a closed FPL rupture, transferring the fourth flexor digitorum superficialis or a tendon graft from the palmaris longus (PL) can be performed. This study reports the results of tendon grafting using the PL in closed FPL rupture and discusses the provision of optimal tendon tension.
METHODS: This retrospective study included 11 out of 20 patients who underwent PL tendon graft for closed FPL rupture between 2013 and 2022, with a follow-

Persistent axillary nerve palsy after reverse shoulder arthroplasty (RSA) results in profound deltoid dysfunction and loss of active shoulder elevation. Traditional reconstructive options such as nerve repair or transfer may not be feasible in chronic cases. Latissimus dorsi tendon transfer augmented with an interposed graft has been described to restore shoulder abduction in deltoid-deficient shoulders, but its application after RSA is not well documented. We present the case of a 73-year-old female patient who developed chronic axillary nerve palsy following RSA performed for complex proxima…

Ballistic injuries of the brachial plexus represent a challenging clinical dilemma. Given the unique pathophysiology of ballistic penetrating trauma, time-sensitive management of vascular and bony stabilization must be balanced with a nuanced understanding of nerve injury and recovery in both space and time. After immediate life and limb-saving interventions, an evidenced-based approach affords multiple options for irrecoverable nerve injury, including exploration and grafting, distal nerve transfers, and tendon transfers. An assessment of pertinent imaging and diagnostic studies, patient acce…

Postburn talipes equinovarus has long been a challenging problem for plastic surgeons, who usually replace the overlying scar in multistaged flap or tube transfer operations before lengthening the shortened Achilles tendon. This always requires long hospitalization and causes much discomfort for the patient because of postural immobilization. In 1975, I designed a new and simple procedure by Z-lengthening the scar tissue and the Achilles tendon as a whole in one stage. To date, the operative result in 10 cases has been excellent, without failure or recurrence.

Flexor digitorum longus transfer or augmentation is currently the most popular adjunctive procedure for the repair of an attenuated or ruptured tibialis posterior tendon. Although the procedure is efficacious, an important functional muscle is sacrificed. Results show that similar results can be achieved with a tenodesis procedure by way of a split anterior tibial tendon repair. The authors have modified the Cobb procedure, and do not create a hole through the medial cuneiform or navicular. The thick, fibrous periosteal tissue at the medial aspect of the cuneiform is a sufficient tunnel for se

Birth brachial plexus palsy (BBPP) can lead to significant functional impairment of the upper limb, particularly affecting hand function. Despite advancements in primary nerve reconstruction, many patients require secondary procedures to optimise hand use. This study evaluates surgical strategies aimed at restoring hand function in BBPP, emphasising the timing of intervention, nerve transfer options and reconstructive techniques. A comprehensive review of direct root transfers, secondary tendon transfers, along with an analysis of sensory re-education and long-term functional outcomes. Early n…
