MR imaging of tendon abnormalities of the elbow.

Tendon injuries about the elbow are among the most commonly encountered pathology in this articulation. These injuries are often the result of chronic repetitive microtrauma but can be seen in the setting of acute trauma. This article focuses on the imaging diagnosis of tendon pathology in the elbow and the associated findings that can be seen with tendon injury. A detailed understanding of common pathology and patterns of injury can facilitate the ease and accuracy of imaging diagnosis.

Achilles tendonitis in a patient with Behçet's disease.

Abstract A 56-year-old male patient had suffered from recurrent oral aphthae, acne-like rashes on the face, an erythema nodosum-like rash on the left lower leg, and severe heel pain on the left-hand side. Colonoscopy revealed six ulcerative lesions in the terminal ileum. Ultrasonography and magnetic resonance imaging showed an inflammatory lesion in the Achilles tendons, with greater inflammation on the left. Achilles tendonitis was considered to be a peripheral enthesopathy in this patient with Behçet's disease complicated by an ileal ulcer.

Peroneal tendon tears: a retrospective review.

Tears of the peroneal tendons are not uncommon but remain an underappreciated source of chronic lateral ankle pain. The purpose of this study was to identify the typical patient profile and nature of the injury, to analyze the course of treatment, and to determine the prevalence of complications seen with surgical repair. Forty patients with chronic pain over the peroneal tendons from the Foot and Ankle Institute at the Western Pennsylvania Hospital underwent peroneal tendon repair. During a 3-year period, a retrospective review was performed by evaluating medical records, surgical reports, an…

Extensor tendon repair: mobilise or splint?

OBJECTIVES: The aim of this study was to assess the static and dynamic hand therapy regimes used at Mount Vernon Hospital, following extensor tendon injury during 1995-2000 and compare them to the early active regimes published. METHODS: Sixty-five patients were included and their hand function recorded by calculating total active motion (TAM), percentage combined motion and extensor lag at 4-6 weeks and at 10 weeks postinjury. RESULTS: The results in two groups were good, with mean TAMs of 202 and 258 at 4-6 weeks and at 10 weeks, respectively, for the static regime and 214 and 245, respect

Management of muscle and tendon injuries in footballers.

BACKGROUND: Muscle and tendon injuries are among the commonest football injuries and can be very frustrating to treat because of the high ri … more conservative approach as the risk of prolonged missed playing time is greater. The rupture of tendons that have nearby agonists can often be managed conservatively, but rupture of isolated prime mover tendons, such as the Achilles and patellar tendons, should be managed surgically.

[Ruptures of the subscapular tendon. A diagnostic problem?].

Isolated ruptures of the subscapularis tendon are rare injuries which are often missed initially. After recent treatment of 3 patients treated in our clinic typical courses and pitfalls are demonstrated. Due to the traumatic genesis of subscapularis tendon ruptures, lesions of the rotator cuff must be excluded generally by an adequate diagnostic concept. Conventional x-rays of the shoulder in 2 views to exclude a bony lesion, clinical and ultrasound examination are seen as a standard because these methods will allow the correct diagnosis in most cases. Typical clinical signs for a subscapulari

Partial rupture of the distal biceps tendon.

We report on 7 cases of partial rupture of the distal biceps tendon. The mean patient age was 52 years (range, 38-58 years). There were 5 men and 2 women. The dominant arm was affected in all 7 patients. Pain was the chief complaint in all patients. Immobilization and physiotherapy were attempted in all patients, and 4 had at least 1 local steroid injection. No patient improved from the conservative treatment. All patients eventually underwent surgical debridement and reattachment of the biceps tendon with use of a 1-incision technique with suture anchors. After a mean follow-up of 31 months (

Response of the rabbit diaphragm to tendon vibration.

To evaluate the potential role of diaphragmatic muscle spindles in the act of breathing, we have recorded the electromyograms of the diaphragm and the external intercostal muscle in the third interspace during high-frequency mechanical vibration (50 Hz) of the central tendon in eight anesthetized, spontaneously breathing rabbits. Vibration induced a consistent, clear-cut increase in the inspiratory activity recorded from the external intercostal, thus indicating that the mechanical stimulus applied to the diaphragm was strong enough to trigger muscle spindles at distant sites. However, vibrati

Closed flexor tendon disruption in athletes.

Closed tendon injuries of the hand and wrist are very common in the athletic population. Most of these injuries, especially those involving the extensor tendons, can be treated successfully with nonoperative management if seen acutely. Acute closed flexor tendon injuries, however, usually require operative intervention for the best result. Although evaluation and diagnosis of flexor tendon injuries are relatively straightforward, diagnosis is still commonly delayed secondary to both delayed presentation and missed diagnoses. If diagnosis is delayed, operative intervention becomes less optimal …

Extensor tendon: anatomy, injury, and reconstruction.

Although seemingly simple in its anatomy and function, the extensor mechanism of the hand is actually a complex set of interlinked muscles, tendons, and ligaments. A thorough understanding of the extensor anatomy is required to understand the consequences of injury at various levels. Reconstructive options must restore normal function. Whereas primary repair of anatomic structures is frequently possible in acute injury, it is rarely possible in chronic situations. Technically exacting procedures may be necessary to restore function.

Tendon entheses of the human masticatory muscles.

Tendons attach to the limb skeleton via chondral-apophysary or periosteal-diaphysary entheses. It was the aim of the present study to investigate the tendon entheses of the temporal, the masseter, as well as the medial and lateral pterygoid muscles, considering the biomechanics and the mode of osteogenesis at the attachment sites. The origin and insertion zones of the four masticatory muscles were studied histologically and by polarization light microscopy in six halves of human heads. Contrary to the limb skeleton no causal relationship between the histological structure of the tendon enthese…

Disorders of the long head of the biceps tendon.

Without a clear understanding of the functional role of the biceps tendon, treatment recommendations have been a subject of controversy. An objective review of the available information would suggest that some humeral head stability may be imparted through the tendon. However, the magnitude of this function is likely to be small and possibly insignificant. In contrast, the symptomatic significance of the long head of the biceps is less controversial, and it has become increasingly recognized as an important source of persistent shoulder pain when not specifically addressed. When present, persi

Blood supply of the tibialis anterior tendon.

Injection techniques and immunohistochemical methods (antibodies against laminin) were performed to uncover the vascular pattern of the human tibialis anterior tendon with regard to spontaneous rupture of this tendon. Proximally, the blood supply of the tibialis anterior tendon mainly arises from the anterior tibial artery. Distally, the tendon is supplied by branches of the medial tarsal artery. Blood vessels enter the peritenon via vinculae from the posterior side. From the peritenon, the blood vessels penetrate the tendon and anastomose with a longitudinally orientated intratendinous networ

Triceps tendon rupture in weight lifters.

Triceps tendon avulsion injuries are rare. We report four weight lifters with triceps tendon raptures, two of whom had received local steroid injections for pain in the triceps. All four patients had taken oral anabolic steroids before injury. All patients had closed avulsion of the triceps tendon from its insertion into the olecranon. Three patients were injured while bench pressing heavy weights, and one patient was injured while swinging a baseball bat. Satisfactory results were achieved after surgical reinsertion of the tendon.

Staged extensor tendon reconstruction in the finger.

Staged extensor tendon reconstruction using a silicone implant followed by tendon grafting was done to restore proximal interphalangeal (PIP) joint extension in 6 fingers with severe injuries to the dorsal skin and extensor mechanism. Abrasions to the joint capsule and cortical surfaces were also present. To avoid finger stiffness, the reconstruction was delayed and range of motion exercises were initiated early. The skin injury was managed by split-thickness skin grafting or allowed to heal by secondary intention to avoid prolonged immobilization. During surgery, the peritendinous fascia of t

Etiology and pathophysiology of tendon ruptures in sports.

Of all spontaneous tendon ruptures, complete Achilles tendon tears are most closely associated with sports activities (1-3). Schönbauer (3) reported that 75% of all ruptures of the Achilles tendon are related to sports. In Plecko & Passl (2) the number was 60%. In our material of 430 cases, the number of sports-related Achilles ruptures was very similar (62%), while only 2% of ruptures of other tendons were sports-related (P < 0.001) (1). Also, the majority of Achilles reruptures occurred in sports. The ruptures occurred most often in soccer (34%), track and field (16%) and basketball (14%). T…
