[Malignant pleural diseases: diagnosis and treatment].

OBJECTIVE: To evaluate efficacy of diagnostic procedures, results of surgery, and complications in malignant pleural diseases. MATERIAL AND METHODS: From 1999 to 2006, 169 patients underwent treatment in the Department of Thoracic Surgery and Oncology, Institute of Oncology, Vilnius University. Patients were divided into two groups: group I, patients with primary pleural malignant diseases (93 patients, 55.0%), and group II, secondary pleural tumors (76 patients, 45%). Of the 76 patients, 40 patients (52.6%) were diagnosed with metastatic pleural tumors and 36 patients (47.4%) with tumors inv

Thoracoscopic management of thoracic duct injury: Is there a place for conservatism?

Thoracic duct injury is a rare but serious complication following chest surgeries and major neck dissections. Clinically, it can present as cervical chylous fistula, chylothorax or chylopericardium. Without treatment, the mortality is up to 50% and thus, early aggressive therapy is indicated. Traditional conservative management includes low-fat diet, parenteral nutrition, careful monitoring of fluid and electrolytes, and drainage of the neck wound or chylothorax. Patients with failed conservative management require definitive treatment in the form of ligation of the thoracic duct, which has tr

Video-assisted open repair of proximal thoracic aortic pseudoaneurysm.

Open surgery remains an essential alternative for descending thoracic aortic aneurysms repair when anatomical constraints preclude endovascular intervention. We present the case of a 63-year-old woman with a complex pseudoaneurysm located at the junction of the distal aortic arch and proximal descending aorta (zones 3/4), approximately 7 mm distal to the origin of the left subclavian artery. The absence of a proximal landing zone rendered thoracic endovascular aortic repair unfeasible. A limited left thoracotomy was performed at the fourth intercostal space. Cardiopulmonary bypass was establis

Left superior pulmonary venous thrombosis complicated with splenic infarction after video-assisted thoracoscopic left upper lobectomy.

A 72-year-old man underwent video-assisted thoracoscopic left upper lobectomy for small cell lung cancer. After 16 days, he experienced epigastric abdominal pain and vomiting, and was taken by ambulance to our hospital. Contrast-enhanced computed tomography (CT) showed a propagation of thrombus in the stump of the left superior pulmonary vein (LSPV) complicated with splenic infarction. The patient received anticoagulation therapy with heparin and warfarin, and further progression of the thrombus or any systemic embolic event was not observed during hospitalization. Here, we report a patient pr

[Video-assisted thoracoscopic thymectomy--preliminary results].

Video-assisted thoracoscopic surgery (VATS) provides a new approach to thymectomy. From April 1999 to December 1999, we performed a total of 10 video-assisted thoracoscopic thymectomies for myasthenia gravis (MG). There were one male and nine female patients with ages ranging from 8 to 59 years. Thymoma was present in one of the ten patients. We considered that complete thymectomy was accomplished in all cases by examination of the thymic bed and of the resected specimen. There was no mortality or intraoperative complications. The median postoperative hospital stay was 4 days. Clinical improve

[A Case of Refractory Chylothorax after Surgery for Esophageal Cancer in Which Lymphangiography and Thoracic Duct Ligation Was Useful].

Chylothorax after esophagectomy is a relatively rare complication that can be difficult to manage. Here, we report a case of refractory chylothorax after surgery for esophageal cancer treated with lymphatic duct lipiodol imaging by inguinal lymph node puncture to confirm patency of the thoracic duct and thoracic duct ligation. A 71-year-old female with esophageal cancer(cT3N0M0)underwent video-assisted thoracoscopic esophagectomy with 2-field lymph node dissection, intrathoracic gastric tube reconstruction, and an enterostomy. A chylothorax appeared when we started enteral nutrition on the day

Not So Vats: How Early Is Too Early in the Operative Management of Patients with Traumatic Hemothorax?

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is a practical resource in the management of traumatic hemothorax. However, it carries inherent risks and should be mobilized cost-effectively. In this study, we investigated the ideal VATS timing using cost analysis. METHODS: 617 cases of unilateral traumatic hemothorax from 2012 to 2022 were identified in our trauma database. We extracted encounter cost, length of stay (LOS), and operative cost information. Using Kruskal-Walli's test, we compared the cost and LOS for patients who underwent VATS or continued nonoperative management in t

The Use of Thoracoscopic Release in the Surgical Correction of Thoracic Scoliosis in Children.

INTRODUCTION: Recently, the use of thoracoscopic methods in spinal deformity surgery has increased, however, the issue of the effectiveness of combining anterior release with posterior spinal fusion compared to single-stage posterior instrumentation remains controversial, which raises the issue of the effectiveness and feasibility of their use. PURPOSE: To evaluate the effectiveness of endoscopic anterior release for thoracic scoliosis in children. MATERIAL AND METHOD: A retrospective analysis of the results of surgical treatment of 48 patients with idiopathic scoliosis of the thoracic spine

[Intrathoracic lipoma resected by video-assisted thoracoscopic surgery; report of two cases].

We experienced 2 cases of intrathoracic lipoma arising from the chest wall and posterior mediastinum. The 1st case was a 65-year-old female, and another case was a 60-year-old male. The 1st case had a 15 cm tumor at posterior mediastinum, and another case had rapidly growing 6 cm tumor at chest wall. Both tumors were surgically removed completely by video-assisted thoracoscopic surgery (VATS), and the diagnosis of lipoma was established pathologically. VATS resection was an useful procedure for intrathoracic lipoma. Computed tomography (CT) and magnetic resonance imaging (MRI) were useful for

Thoracic actinomycosis with mainly pleural involvement.

A 61-year-old man, who had been diagnosed with pleuritis 5 months earlier, was admitted to our hospital to determine if a tumor shadow that appeared in his right lower lung field on March 2002 was a localized pleural mesothelioma. Although a CT-guided lung biopsy was performed, no definite diagnosis was made. However, because the tumor shadow continued to increase in size, we could not rule out the possibility of a malignant thoracic tumor, and performed video-assisted thoracoscopic surgery (VATS). Histological examination of the surgically resected tissue led to a diagnosis of thoracic actino

[Minimally invasive video thoracoscopic correction of pectus excavatum].

Pectus excavatum, the most common congenital deformity of the chest wall, has esthetic, psychological and social repercussions as well as a slight impact on pulmonary function. Treatment is surgical and is carried out for esthetic purposes in most cases. The most commonly applied surgical technique is based on the one originally described by Ravitch: sub-perichondrial, bilateral chondrectomy and sternal osteotomy. In 1997, however. Nuss described a minimally invasive approach to correction by means of a support bar. Our objective was to perform minimally invasive correction of pectus excavatum

Minimally invasive repair of a left diaphragm hernia after debulking surgery for advanced ovarian cancer.

Eighty percent of women with ovarian cancer have advanced disease (FIGO stage III or IV) at the time of diagnoses and require extensive upper abdominal surgery to obtain complete gross resection (Minig et al., 2015, Eisenhauer et al., 2006). A diaphragmatic hernia is defined as abdominal contents bulging into the thoracic cavity (Spellar and Gupta, 2020). While rare following primary debulking surgery (PDS), these present with a variety of symptoms and are often misdiagnosed. Computed tomography (CT) is the diagnostic gold standard (Vertaldi et al., 2020). This video demonstrates repair of a l

Routine Immediate Postoperative Chest Radiography After Minimally Invasive Pulmonary Lobectomy Has a Low Impact on Clinical Management: A Value-Based Imaging Analysis.

BACKGROUND/OBJECTIVES: Despite the widespread adoption of minimally invasive video-assisted thoracoscopic surgery (VATS) for pulmonary lobectomy and Enhanced Recovery After Surgery (ERAS) protocols, routine chest X-rays (CXRs) in the Post-Anesthesia Care Unit (PACU) after VATS pulmonary lobectomy remain common. Evidence questioning their clinical utility in asymptomatic patients is growing. This study aimed to evaluate the therapeutic yield of routine PACU CXRs following VATS lobectomy, in a homogeneous cohort (PACU time point). METHODS: This single-center retrospective cohort study included

Thoracoscopic diagnosis of tuberculous pleuritis mimicking pleural malignancy with chest wall involvement: a challenging case report.

BACKGROUND: Tuberculous pleuritis is a common form of extrapulmonary tuberculosis that may closely mimic pleural malignancy, particularly when associated with loculated effusion, pleural thickening, or chest wall involvement. This diagnostic challenge is amplified in resource-limited settings. CASE PRESENTATION: We report a 41-year-old male with a 30 pack-year smoking history who presented with a two-month history of pleuritic chest pain, dyspnea, night sweats, and significant weight loss. Imaging revealed a large loculated right-sided pleural effusion with diffuse pleural thickening and asso

Video-assisted thoracoscopic excision of a bronchogenic cyst with a pericardial defect and an aortic feeding artery.

Bronchogenic cysts are rare congenital foregut anomalies, often detected incidentally. We report the case of a 24-year-old female who was incidentally detected during a routine health check-up to have a mediastinal mass. Preoperative chest computed tomography imaging suggested a benign bronchogenic cyst. Minimally invasive video-assisted thoracoscopic surgery was planned for its excision. Intraoperatively, a pericardial defect and a feeding artery arising from the aortic arch were noted. The patient recovered uneventfully and was discharged on postoperative day 3. This case demonstrates the ut

Intercostal Nerve Reconstruction for Severe Compensatory Hyperhidrosis: The Gebitekin Technique.

Compensatory sweating (CS) is the most common and disabling complication of endoscopic thoracic sympathectomy and represents an unmet clinical challenge. Our surgical hypothesis is to generate a parallel pathway to the damaged part of the sympathetic nerve, similar to the Kuntz nerve, by reconstructing the 2 healthy intercostal nerves, thus treating CS. Here, we present a novel videothoracoscopic technique involving bilateral intercostal nerve reconstruction in patients with severe CS after endoscopic thoracic sympathectomy.

Thoracoscopically assisted ligamentous release of the thoracic spine: a cadaver study.

The surgical treatment of thoracic kyphosis frequently requires an anterior release, which can be carried out by an open or endoscopic technique. This study concerns the reference points which are essential from the anatomic view for minimizing the operative risks. Furthermore we wanted to find the most convenient video-assisted thoracoscopic surgery (VATS) technique for transection of the anterior longitudinal ligament (ALL). Transection of the ALL, as well as discectomy, was performed using this technique. The extensive anatomic dissection of 12 fresh cadavers was performed to define possibl
