Ludwig's angina: report of two cases and review of the literature from 1945 to January 1979.

Two cases of Ludwig's angina treated successfully led us to review the main English language literature from 1945 to January 1979. The most common cause of Ludwig's angina is dental, and the organism cultured most often is streptococcus. However, other avenues of infection and numerous pathogens have been implicated in this disease. Penicillin remains the preferred primary antibiotic, with combinations of this drug and other antibiotics being used by various authors. In both the pre-antibiotic and antibiotic era, the airway remains the prime concern of the surgeon in the management of this pot

Regeneration of Pulp and Radicular Tissues in a Nonvital Avulsed Tooth with Open Apex: A Case Report.

AIM AND BACKGROUND: The management of avulsed immature teeth poses a significant challenge to clinicians, as prolonged dry time before reimplantation may lead to replacement resorption or ankylosis. CASE DESCRIPTION: Recent studies have shown promising results with the use of an intracanal proprietary pharmaceutical preparation, followed by sealing of the orifice with mineral trioxide aggregate (MTA). Angiography suggested the presence of vascularized tissue after 1 year, indicating successful regeneration of pulpal tissue in the radicular area. CONCLUSION: Despite an unfavorable crown-root

Intraoral surgical treatment of large complex odontoma with technology support in planning.

Complex odontoma or odontogenic hamartoma is the most common odontogenic tumor, diagnosed mainly in the first two decades of life, rarely exceeding 3 cm. This paper aim is report a clinical case of complex odontoma in the retromolar region that could induce an imminent risk of mandibular fracture. The patient reported local pain, discomfort, an area with hardened edema and absence of the lower right second molar. Using computed tomography, a three-dimensional model was printed and a mandibular reconstruction plate 2.4 mm pre-bent. Through intraoral access, osteotomy was performed to remove the

Good occlusal practice in children's dentistry.

The difference between paediatric dentistry and most other branches of dentistry is that in the child the occlusion is changing. Consequently 'Good Occlusal Practice' in children is a matter of making the right clinical decisions for the future occlusion. The clinician needs to be able to predict the influence that different treatment options will have on the occlusion when the child's development is complete.

Dental implants placed in a patient with multiple myeloma: report of case.

A 69-year-old male, under active treatment for multiple myeloma, fractured his left maxillary second premolar palatal abutment tooth and requested treatment because his upper partial denture no longer fit. Several other teeth were deemed hopeless, and construction of a maxillary complete denture presented problems. There are general contraindications regarding dental implant surgery in the terminally ill patient; however, in this case, the patient wished to maximize his quality of life and opted to undergo implant surgery. With close management, the treatment was successful.

Timing of orthodontic tooth movement in a case with traumatized and avulsed anterior teeth.

A traumatic injury to a patient undergoing orthodontic treatment resulted in avulsion, dislodgement, and fracture of upper anterior teeth. Deciding when to institute endodontic treatment and when to resume orthodontic treatment was based upon minimizing the changes for ankyloses and root resorption. Endodontic treatment was carried out two weeks after trauma, and orthodontic treatment continued three weeks after trauma. Radiographs taken six months, one year, and two years after orthodontic completion showed progressive root resorption to the avulsed tooth and minimum root resorption to the re

The Evaluation of Objective and Subjective Fate of Teeth in the Mandible Fracture Line and the Management-A Center's Experience.

OBJECTIVE: The decision to retain or extract teeth in the line of mandibular fractures has been a subject of debate in much of the scientific literature, and there is a need for further evidence to clear this issue. Thus, the aim of this study was to investigate both the positive and negative effects of teeth in the line of mandibular fractures provide more evidence in this field, as well as take into consideration patients' quality of life after the surgery. METHODS: Patients after trauma with teeth in the line of mandibular fractures were included in this study. Open reduction and rigid int

Influence of Cavity Designs on Fracture Resistance: Analysis of the Role of Different Access Techniques to the Endodontic Cavity in the Onset of Fractures: Narrative Review.

OBJECTIVES: In recent years, new endodontic access techniques have been proposed with the aim of preserving as much dental tissue as possible for subsequent prosthetic rehabilitation. It has indeed been demonstrated that the success of this therapy is essential and dependent on the proper execution of endodontic cavity access. The main objective is to provide a comprehensive and up-to-date overview of the new access techniques in endodontics in order to guide clinical practice toward a more precise and qualitative approach. As of today, there is still no universally standardized and recognized

Partial pulpotomy in crown-fractured permanent incisor with hyperplastic pulpitis: a case report.

Hyperplastic pulpitis, which is regarded as one type of asymptomatic irreversible pulpitis, occurs usually in young teeth where pulps are exposed by caries or trauma. If apices are mature, root canal therapy is recommended. In this case, the success of partial pulpotomy in a crown-fractured permanent incisor with hyperplastic pulpitis, which had been contaminated with oral microflora for six months is reported.

Displacement of maxillary third molar into the lateral pharyngeal space.

Iatrogenic tooth displacement is a rare complication during extraction of impacted molars, but displacement of a maxillary third molar into the maxillary sinus, infratemporal fossa, buccal space, pterygomandibular space, and lateral pharyngeal space has been reported. Currently, 6 published reports describe third molar displacement into the lateral pharyngeal space, only 1 of which involved the loss of a maxillary third molar into this area, which occurred after an attempted self-extraction by the patient. There have been no reported cases of iatrogenic displacement of the maxillary third mola

Multiple cemental tears.

A cemental tear is a pathologic condition in which a complete or incomplete separation of the cementum occurs along the root surface and is usually accompanied by a deep periodontal pocket. Past articles report that the incidence of cemental tears has usually been limited to 1 tooth per individual. We encountered a clinical case with cemental tears involving 14 teeth in 1 individual. Multiple cemental tears in 1 individual have not been previously described in the dental literature. We present the clinical and pathologic features of this rare case and suggest that the probable cause of multipl

Replantation of three avulsed permanent incisors with complicated crown fractures.

AIM: To report a rare case of multiple avulsions of permanent teeth associated with complicated crown fractures. SUMMARY: The case describes a 3-year follow-up of the treatment of three avulsed maxillary incisors with complicated crown fractures. The teeth were replanted after an extra-oral time of 90 min (extra-oral dry time 6 min). After root canal treatment, the fractured maxillary incisors were restored with a layered hybrid composite material to re-establish aesthetics and function. Follow-up visits showed signs of replacement resorption affecting the maxillary central incisors but only

Attractive magnets for orthodontic extrusion of crown-root fractured teeth.

A subgingival crown-root fracture presents the clinician with a difficult restorative problem, including reaching the fracture line, and is complicated by the need to maintain the periodontal tissues in good health. The treatment options up to now have usually been limited to extrusion of the remaining root with a conventional orthodontic appliance, surgical intraalveolar transplantation of the root or extraction with bridge replacement. In this report, a new method of orthodontic extrusion with attractive magnets is presented. One or two neodymium-iron-boron magnets were attached to the remai

Management of intrusive luxation injuries.

Traumatic intrusion of permanent teeth is a relatively infrequent but serious type of dental injury, due to the complicated picture it involves. Various treatment approaches have been suggested, so far, regarding management of intrusive luxation. Techniques aiming to reposition the intruded tooth include observation for spontaneous reeruption, surgical as well as orthodontic repositioning. However, development of complications such as pulp necrosis, inflammatory root resorption, replacement resorption and ankylosis and loss of marginal bone support makes selection of the most favorable techniq

Seventeen-year follow-up of hypophosphatasia diagnosed in middle-aged siblings harboring a novel intronic and a rare missense ALPL gene mutation.

Hypophosphatasia (HPP) is the rare inborn-error-of-metabolism that features impaired mineralization of the skeleton and teeth due to a deactivating mutation or mutations of the gene ALPL which encodes the tissue-nonspecific isoenzyme of alkaline phosphatase (TNSALP). We report 17-year follow-up of twin sisters and a brother referred in middle-age for painful proximal femoral "stress fractures" and then diagnosed with HPP. They reported generalized muscle and bone pain, metatarsal fractures, arthropathy and, since childhood, tooth loss. Their concordant findings were explained by compound heter

Use of an intraoperative navigation system for retrieving a broken dental instrument in the mandible: a case report.

BACKGROUND: A fracture of root canal instruments, with a fractured piece protruding beyond the apex, is a troublesome incident during an endodontic treatment. Locating and retrieving them represents a challenge to maxillofacial surgeons because it is difficult to access due to the proximity between the foreign body and vital structures. Although safe and accurate for surgery, radiographs and electromagnetic devices do not provide a precise three-dimensional position. In contrast, computer-aided navigation provides a correlation between preoperatively collected data and intraoperatively encount
