Resolution of tractional macular edema following spontaneous detachment of residual vitreous cortex in a vitrectomized eye.

A 15-year-old boy underwent pars plana vitrectomy and lensectomy following blunt closed-globe injury. Postoperatively, he had visual distortion and a decline in visual acuity attributable to tractional macular edema caused by clinically apparent residual vitreous cortex. One month later, macular edema resolved and visual acuity improved following spontaneous vitreous separation. This case illustrates what has been termed the posterior bursa macularis and emphasizes its pathoetiologic role in vitreomacular disease.

Closed reduction with traction for developmental dysplasia of the hip in children aged between one and five years.

The treatment of developmental dysplasia of the hip diagnosed after the first year of life remains controversial. A series of 36 children (47 hips), aged between one and 4.9 years underwent gradual closed reduction using the Petit-Morel method. A pelvic osteotomy was required in 43 hips (91.5%). The patients whose hips did not require pelvic osteotomy were among the youngest. The mean age at final follow-up was 16.1 years (11.3 to 32). The mean follow-up was 14.3 years (10 to 30). At the latest follow-up, 44 hips (93.6%) were graded as excellent or good according to the Severin classification.

Unusual ocular complications of the HELLP syndrome: persistent macular elevation and localized tractional retinal detachment.

We report a case of HELLP syndrome with bilateral decreased vision at 25 weeks' gestation. Fundus examination revealed serous retinal detachment involving the macula. Systemic steroid and oral dorzolamide therapy was initiated and visual acuity improved. At 2 months, peripheral localized tractional retinal detachment was present in the left eye and argon laser photocoagulation was performed. Macular elevation did not resolve; therefore, in the right eye intravitreal triamcinolone acetonide injection was performed.

Analysis of patients with nonambulatory neuromuscular scoliosis surgically treated to the pelvis with intraoperative halo-femoral traction.

STUDY DESIGN: Retrospective case-control study. OBJECTIVES: To compare patients treated with and without intraoperative halo-femoral traction to assess neuromuscular spinal deformity correction as well as the safety of the technique. SUMMARY OF BACKGROUND DATA: Optimal sitting balance can be achieved in nonambulatory neuromuscular patients with pelvic obliquity by maneuvering a Galveston-type rod or inserting screws into the iliac wings; however, this is often clinically challenging because of the small, soft bone-stock in the pelvis of these patients. METHODS: A total of 40 patients with n…

[Mesenteric traction syndrome during the operation of aneurysms of the abdominal aorta--histamine release and prophylaxis with antihistaminics].

Mesenteric traction syndrome occurs during abdominal surgery and is described as sudden tachycardia, hypotension and flush. Among other etiological factors, eventeration or mesenteric traction of the small intestine may cause histamine release from mesenteric mast cells. Therefore, our hypothesis was that mesenteric traction syndrome could be positively influenced by prophylactic administration of H1- and and H2-antihistamines. Seventeen male patients (ASA groups III-V, 48-78 years old) were investigated in a randomised double blind study during elective abdominal aortic aneurysm (AAA) repair;…

Traction injuries of the brachial plexus: signal intensity changes of the posterior cervical paraspinal muscles on MRI.

PURPOSE: Our goal was to evaluate MR signal intensity change of the posterior cervical paraspinal muscles in brachial plexus traction injuries to determine the value of this finding in the diagnosis of nerve root avulsion. METHOD: MR images of 11 consecutive patients, 7 with nerve root avulsion and 4 without nerve root avulsion, were retrospectively analyzed, focusing on the signal intensity of the paraspinal muscles. RESULTS: Among the seven patients with nerve root avulsion, T2- or T2*-weighted MR images showed increased signal intensity in the deep posterior paraspinal muscles in five and…

Recognition of vitreoschisis in proliferative diabetic retinopathy. A useful landmark in vitrectomy for diabetic traction retinal detachment.

INTRODUCTION: In the late stages of proliferative diabetic retinopathy (PDR) the vascularized posterior cortical gel (PCG) contracts leading to a partial posterior hyaloidal separation, hemorrhage, and traction retinal detachment (TRD). "Additional epiretinal membranes" have been described previously. These are thin, usually transparent epiretinal membranes which extend anteriorly from the point of attachment of the elevated posterior cortical gel to the edge of the TRD. The origin and frequency of the occurrence as well as the clinical significance of such additional epiretinal membranes are …

An innovative model of system-linked community care: home-based traction as an alternative to institutional treatment.

Home-based traction is an alternative treatment to conventional hospital-based traction for children with orthopedic conditions such as congenital dislocated hip and Legg-Perthes disease. The application of a proposed theoretical model is used to describe the process of the home-based traction innovation. Creativity and innovation occasions a reconceptualization of stresses. The reconceptualization of stress, plus the operational support of coordinated in-home services, can help families and nurses in the community and in the hospital develop, implement, and evaluate this kind of innovative ca…

[Lucilia sericata infestation of the skin openings for the bone traction device in lengthening of the tibia: apropos of a case].

Fly larvae are responsible for animal disease well known by veterinarians. However, their finding in humans in nontropical countries is a rarity. Recently, we have found several larvae of Lucilia sericata in an achondroplastic child during the phase of lengthening of both tibiae with osteotax. We think that the myasis diagnosed in our patient is the first such case reported in the literature. For the time being, it should be considered a fortuitous finding and not as a complication of surgery.

[Application of isolation traction of aortic arch and upper part of the thoracic aorta in operation on cancer of the esophagus].

Isolation traction of aortic arch and upper part of thoracic aorta was performed by pulling and rotating them rightwards during operation on cancer located in the mid-segment of esophagus with serious adhesions. In 35 patients operated on, 33 cancers were resected with resection rate of 94%, including 21 by radical resection (60%) and 12 by palliative operation (34%). The method was to isolate the aortic arch and the upper part of thoracic aorta sufficiently and ligate the severed intercostal arteries and veins securely. While isolating the aorta, we pulled and rotated it with a strip of banda…

Application of in vivo traction-assisted resection of large gastric mucosal lesions: a case series (with video).

BACKGROUND: Traction-assisted techniques have been widely applied in gastric endoscopic submucosal dissection (ESD), enabling more stable and distinct visualization of the submucosa through mucosal elevation. However, there remains a notable scarcity of reports on their application in managing large gastric mucosal lesions. Here, we present a case series introducing the application of elastic ring traction-assisted resection for large gastric mucosal lesions and evaluate its efficacy and safety. METHODS: This prospective case series study enrolled 13 patients with large gastric mucosal lesion…
