The differential impacts of PEPFAR transition on private for-profit, private not-for-profit and publicly owned health facilities in Uganda.

While transition of donor programs to national control is increasingly common, there is a lack of evidence about the consequences of transition for private health care providers. In 2015, President's Emergency Plan for AIDS Relief (PEPFAR) identified 734 facilities in Uganda for transition from PEPFAR support, including 137 private not-for-profits (PNFP) and 140 private for-profits (PFPs). We sought to understand the differential impacts of transition on facilities with differing ownership statuses. We used a survey conducted in mid-2017 among 145 public, 29 PNFP and 32 PFP facilities reportin

Development of a Situation-Specific Theory for the Transition of Survivors of Stroke With Dysphagia.

The process by which stroke survivors move toward health while facing various difficulties can be construed as a "transition." Importantly, nurses need to understand and support this state of transition effectively. This study developed a situation-specific theory to explain post-stroke transition by integrating the findings of a qualitative study that explored the rehabilitation experiences of stroke survivors with dysphagia with a scoping review of qualitative studies using the theoretical framework of transition theory. This theory will help understand the transitions that stroke survivors

The adolescent and young adult with Klinefelter syndrome:ensuring successful transitions to adulthood.

As many adolescent and young adult males with Klinefelter syndrome move into young adulthood, they will require long-term medical and mental health care for their complex and specific health care needs. Adult general medical providers and endocrinologists may feel ill-prepared to meet the medical needs of this population. In turn, many pediatric primary care and subspecialty providers may find themselves struggling to determine how best to transition the older adolescent to adult-oriented services. Working with families and their teens to consider and address current and future medical, mental

Transitioning a large-scale HIV/AIDS treatment program from an international partner to a local Nigerian implementing partner: a before-after early outcomes assessment study.

BACKGROUND: HIV prevention, treatment, and care across the globe have been heavily dependent on donor funding and international aid, particularly across the Global South, with the highest contribution from the United States Government (USG) through PEPFAR. PEPFAR's shift towards HIV care sustainability involves transitioning HIV/AIDS treatment programs from international to local partners. Despite the increasing focus on these transitions, there is limited evidence of effective models and the outcome of such transitions, particularly in low- and middle-income countries like Nigeria. This study

The effectiveness of a transition programme based on the integrated theory of health behavior change in adolescents with chronic kidney disease: a quasi-experimental study.

BACKGROUND: To validate the effectiveness of the application of a transition programme for adolescents with chronic kidney disease based on the Integrated Theory of Health Behavior Change. METHODS: This study was a quasi-experiment study. We included 76 adolescents with chronic kidney disease, and based on their willingness to undergo transition intervention, participants were assigned to either the intervention group or the control group. The intervention group received the transition intervention guided by the Integrated Theory of Health Behavior Change, while the control group received sta

Assessment of transition readiness in adolescents in Thailand with rheumatic diseases: a cross-sectional study.

BACKGROUND: Most childhood-onset rheumatic diseases are chronic health conditions, which need long-term care throughout adulthood. A well-organized transition care is challenging and patient assessment of transition skills is needed for transfer preparation to an adult care setting. The Transition Readiness Assessment Questionnaire (TRAQ) is used to assess transition skills in chronically ill patients. Currently, limited transition skill assessment data exist in pediatric patients with rheumatic diseases, especially in Asian countries. This study aimed to determine the transition readiness ski

Adult patient perspectives on phenylketonuria care: Highlighting the need for dedicated adult management and services.

Phenylketonuria (PKU) is an autosomal recessive metabolic disorder characterized by an inability to metabolize the amino acid phenylalanine (Phe). If left untreated, an accumulation of Phe results in neurodevelopmental, neurological and psychological impairments. Advancements in detection and treatment of PKU have improved outcomes and life expectancy for these patients, emphasizing the need for life-long, specialized care. Due to the paucity of adult-focused PKU clinics, patients who are well into adulthood are still being treated in pediatric centers. This retrospective study evaluates the p

From fragmentation toward integration: a preliminary study of a new long-term care policy in a fast-aging country.

BACKGROUND: Taiwan, one of the fastest-aging countries in the world, started implementing version 1.0 of its long-term care (LTC) plan in 2008. In 2017, LTC Plan 2.0 began a new era with its goal to integrate Taiwan's fragmented LTC service system. LTC Plan 2.0 also aims to establish an integrated community-based LTC system incorporating both health care and disability prevention. This three-tier model consists of the following: two LTC services with a day-care center as their base and case management (Tier A), a day-care center and a single LTC service (Tier B), and LTC stations that provide

Pediatric to Adult Nephrology Transition in CKD.

CKD affects roughly 10% of the general population. Some adults with CKD were diagnosed with their condition during childhood. Because early CKD is often asymptomatic, obtaining reliable estimates of the true incidence of pediatric CKD (onset before age 18 years) is difficult due to the lack of routine screening for disease in children. With advancements in health care ( e.g ., antenatal imaging and genetic testing) and improved survival outcomes for premature infants and children with complex care needs, a growing number of children with CKD will eventually transfer their care to adult special

"Crafting a 'TransitionOmeter': A Proposed Framework for Developing and Honing Capabilities of Young People Transitioning to Adult Healthcare Services.".

This paper focuses on an innovative approach to preparing children and young people, with Spina Bifida, to move from child-centered to adult-oriented healthcare systems. Reflecting on our role in delivering a national nurse led service, we set to identify and critique international transition tools in use for this population. Specifically, we aimed to identify the core capabilities and indicators of progression to successful transition, so that holistic interventions could be planned to match the needs of individuals and their carers. There were two phases to the study, initially focusing on a

How parents of young adults with intellectual disability make sense of their experiences of transitioning from learning disability CAMHS to learning disability adult health service: An IPA study.

Whilst existing literature describes the experience of transitioning in mainstream mental health services, little is known about specialist learning disability services. The following study aimed to explore how parents of young adults, aged 17 to 21, with intellectual disability make sense of their experiences of transitioning from learning disability Child and Adolescent Mental Health Service (CAMHS) to adult community learning disability teams. Semi-structured interviews were conducted with four parents of individuals diagnosed with a learning disability who were receiving care in the adult

Transitional Pain Service: An Update.

PURPOSE OF REVIEW: Chronic Postsurgical Pain (CPSP) and the risk for long-term opioid dependency are known complications following major surgery. The idea of Transitional Pain Service (TPS) has been introduced as an interdisciplinary setting to manage pain in the perioperative continuum. We expand on the basic framework and principles of TPS and summarize the current evidence of the TPS and possible interventions to adress postoperative pain. Areas of future work in TPS-related research are discussed. RECENT FINDINGS: Several studies support the effectiveness of TPS in reducing opioid consump

Development of a Virtual Transition Program for Adolescents and Young Adults With Type 1 Diabetes: Three-Stage User-Centered Design Study.

BACKGROUND: Adolescents and young adults (AYAs) with type 1 diabetes (T1D) face a heightened risk of care gaps and preventable complications during the transition from pediatric to adult care. AYAs from low-income families are more likely to experience poor outcomes. Care guidelines for T1D now recommend a transition program beginning several years prior to anticipated independence. However, existing transition programs often require substantial resources, limiting their scalability and accessibility. OBJECTIVE: We used a human-centered design approach to develop and refine a low-cost, virtua
